_**NEW** quality improvement initiative based on a data analysis.  This will be a PowerPoint presentation of 7-10 slides and should address the following points:_

    ·

    ·
_Provide actual data indicating the need for the QI initiative._

    ·
_Develop a QI proposal base on the identified health issue and data analysis that focuses on improvement._

    ·

Make a quality initiative proposal (7-10 PowerPoint slides) through a presentation, interpreting and communicating dashboard data to support the proposal.

    Collapse All

    **Introduction**

Health care providers are perpetually striving to improve care quality and patient safety. To accomplish enhanced care, outcomes need to be measured. Next, data measures must be validated. Measurement and validation of information support performance improvement. Health care providers must focus attention on evidence-based best practices to improve patient outcomes.

Health informatics, along with new and improved technologies and procedures, are at the core of
_all_ quality improvement initiatives. Data analysis begins with provider documentation, researched process improvement models, and recognized quality benchmarks. All of these items work together to improve patient outcomes. Professional nurses must be able to interpret and communicate dashboard information that displays critical care metrics and outcomes along with data collected from the care delivery process.

    **Overview**

_A basic principle of quality measurement is: If you can't measure it, you can't improve it._

**_–Agency for Healthcare Research and Quality (2021)_**

In the previous assessment, you analyzed the effectiveness of an existing quality initiative. Now that you've done that, this assessment gives you experience interpreting and communicating dashboard data for the purpose of making a quality initiative proposal of your own. And you'll make your proposal through a PowerPoint presentation, including using speaker notes in the Notes section of the slides, much like you might in your health care setting.

Health care providers are on an endless quest to improve both care quality and patient safety. This unwavering commitment requires hospitals and care givers to increase their attention and adherence to treatment protocols to improve patient outcomes. Health informatics, along with new and improved technologies and procedures, are at the core of virtually all QI initiatives. The data gathered by providers, along with process improvement models and recognized quality benchmarks, are all part of a collaborative, continuing effort. As such, it is essential that professional nurses are able to correctly interpret and effectively communicate information revealed on dashboards that display critical care metrics.

    **Instructions**

In this assessment, you will make a QI initiative proposal based on a health issue of professional interest to you. This proposal will be based on an analysis of dashboard metrics from a health care facility. You have two options:

    **Option 1**

    If you
**have** access to dashboard metrics related to a QI initiative proposal of interest to you:

    · Analyze data from the health care facility to identify a health care issue or an area of concern. You will need access to reports and data related to care quality and patient safety. If you work in a hospital setting, contact the quality management department to obtain the data you need.

    · You will need to identify basic information about the health care setting, size, and specific type of care delivery related to the topic that you identify. You are expected to abide by standards for compliance with the Health Insurance Portability and Accountability Act (HIPAA).

    **Option 2**

    If you
**do not have** access to a dashboard or metrics related to a QI initiative proposal:

    · You may use one of the links in the following reading list to access a dashboard or metrics for a health care facility to develop the QI initiative proposal:

    ·
    [Assessment 3: Resources](https://capella.alma.exlibrisgroup.com/leganto/public/01CAPELLA_INST/lists/2475584570006731?auth=SAML&section=3429472480006731)

    · You will follow the same instructions and provide the same deliverables as your peers who select Option 1.

Complete the following steps for your proposal:

    · Analyze data to identify a health care issue or an area of concern as it relates to a state, national, or accreditation benchmark requirement relevant to your professional setting.

    · Evaluate the quality of the data.

    · Outline a QI initiative proposal based on the selected health issue or area of concern and supporting data analysis to improve identified dashboard metric. The interactive activity Designing a Quality Improvement Initiative can get you going on the first steps of a QI process and your assessment.

    · Identify the target areas of improvement and outcome measures.

    · Include the QI model that will be utilized.

    · Specify evidence-based strategies that will be utilized.

    · Integrate interprofessional perspectives and actions to lead quality improvements in patient safety, cost-effectiveness, and work-life quality.

    · Specify roles and responsibilities.

    · Apply effective collaboration strategies to promote QI of interprofessional care.

    · Include specific communication tools.

    · Deliver a persuasive, coherent, and effective audiovisual presentation. Integrate relevant sources to support arguments, correctly formatting citations and references using current APA style.

Be sure that your proposal, at minimum, addresses each of the bullet points. You may also want to read the Data Analysis and Quality Improvement Initiative Proposal Scoring Guide to better understand the performance levels that relate to each grading criterion. Additionally, be sure to review the

Assessment 01 [Group 3] – Enhancing Quality and Safety

For this assessment, you will develop a 3-5-page paper that examines a safety quality issue in a
health care setting. You will analyze the issue and examine potential evidence-based and best-
practice solutions from the literature as well as the role of nurses and other stakeholders in
    addressing the issue.

Before you complete the instructions detailed in the courseroom, first select one of the safety
quality issues below to analyze for this assessment. After you have selected one of the issues
from the list below, return to the detailed instructions in the courseroom to complete your
    assessment.

    • Alarm Fatigue: Alarm fatigue occurs when nurses become desensitized to frequent
alarms, leading to delayed or missed responses to critical alerts. Nurses need to
prioritize and manage multiple alarms effectively. Failure to respond to critical alarms
can result in patient harm. Understanding and addressing this issue is essential:

o Training & Education: Ongoing training on alarm management and the
significance of different alarms is essential. Proper training helps nurses
respond appropriately and reduce alarm fatigue. Ongoing training on alarm
management and the significance of different alarms is essential. Proper training
helps nurses respond appropriately and reduce alarm fatigue

o Technology Implementation: Utilizing advanced alarm systems that reduce
false alarms can help mitigate alarm fatigue. Improved technology can enhance
patient safety and reduce nurse stress.

o Policy Development: Developing policies to manage alarm settings and
response protocols can standardize practices. Clear policies ensure consistent
    and effective alarm management.

o Patient Safety and Quality of Care: With fewer staff members, each individual
is often required to manage a larger number of patients. This increased
workload can lead to oversights, missed care, delayed responses to patient
needs, and a higher likelihood of errors in medication administration or treatment

    5

    • Workplace Violence: Workplace violence, including physical and verbal abuse, is a
significant issue in healthcare settings, affecting the safety and well-being of nurses and
can trickle down to patient care. Nurses need training on how to de-escalate potentially
    violent situations and protect themselves.

o Patient Safety and Well-being: An alarming consequence of workplace violence
is the potential harm to the patient. Distraction and stress can lead to medication
administration errors, missed alarms, increased fatigue leading to other types of
    medical or nursing errors.

o Loss of Trust: When healthcare professionals experience or witness workplace
violence, their trust in the healthcare system can be severely eroded. This loss of
trust can make nurses seek other employment, lead to more absences and more
stress in the workplace, impacting patient care.

o Reporting Mechanisms: For healthcare professionals, establishing clear
reporting mechanisms encourages nurses to report incidents of violence.
Accurate reporting helps identify patterns and implement preventive measures.

o Operational Disruptions: Workplace violence can disrupt the normal flow of
operations in a healthcare setting. This may impact the flow of patient care,
leading to additional tests, procedures, or interventions to correct any errors
    leading to delays and inefficiencies.

o Regulatory and Accreditation Implications: Healthcare facilities are subject to
regulations and standards set by accrediting bodies. Workplace violence can lead
to errors, which can lead to penalties, increased scrutiny, and potential challenges
    in maintaining accreditation.

For this assessment, you will develop a 3–5 page paper that examines a safety quality issue in a healthcare setting. You will analyze the issue and examine potential evidence-based and best-practice solutions from the literature as well as the role of nurses and other stakeholders in addressing the issue.

The role of the baccalaureate nurse includes identifying and explaining specific patient risk factors, incorporating evidence-based solutions to improving patient safety and coordinating care. A solid foundation of knowledge and understanding of safety organizations such as Quality and Safety Education for Nurses (QSEN), the Institute of Medicine (IOM), and The Joint Commission and its National Patient Safety Goals (NPSGs) program is vital to practicing nurses with regard to providing and promoting safe and effective patient care.

You are encouraged to complete the Identifying Safety Risks and Solutions activity. This activity offers an opportunity to review a case study and practice identifying safety risks and possible solutions. We have found that learners who complete course activities and review resources are more successful with first submissions. Completing course activities is also a way to demonstrate course engagement.

As a baccalaureate-prepared nurse, you will be responsible for implementing quality improvement (QI) and patient safety measures in healthcare settings. Effective quality improvement measures result in systemic and organizational change, ultimately leading to the development of a patient safety culture.

Consider the hospital-acquired conditions that are not reimbursed under Medicare/Medicaid, some of which are specific safety issues such as infections, falls, medication errors, and other concerns that could have been prevented or alleviated with the use of evidence-based guidelines.

The purpose of this assessment is to better understand the role of the baccalaureate-prepared nurse in enhancing quality improvement (QI) measures to address patient safety risk at a healthcare setting of your choice. You will do this by exploring the professional guidelines and best practices for improving and maintaining patient safety in healthcare settings from organizations such as QSEN (Quality and Safety Education for Nurses) and the IOM (Institute of Medicine). Looking through the lens of these professional best practices to examine the current policies and procedures in place at your chosen organization and the impact on safety measures for patients, you will consider the role of the nurse in driving quality and safety improvements. You will identify stakeholders in QI improvement and safety measures as well as consider evidence-based strategies to enhance quality of care and promote safety in your chosen healthcare setting.

Everyone should have a working knowledge of the terminology used for personal medications that may be prescribed. Healthcare workers need to know this as well for patients. The patient needs related to pharmacological therapies are endless and as a point-of-care health professional, you may very well be that person.

Medications are given by route. Unlike driving routes by road, in this case, routes refer to the part of the body used to get the drug in. Indeed, every single body orifice you know about (and a few you probably do not know about) can be used to administer medication. Create a handout to give strategies for effective communication, and a separate narrative to evaluate communication strategies most appropriate for your chosen disaster or catastrophic event.

    Collapse All

    **Introduction**

Imagine you're feeling good about the disaster management plan you formulated, and it was well received in the meeting at which you presented. But you know full well the best-laid plans can come apart in the heat of the moment if community organizations and agencies aren't communicating effectively with each other. So you decide to offer some suggestions via a handout to increase the chances of success.

    **Preparation**

If you haven't already, use the Capella library to research team dynamics for disasters and public health emergencies, along with strategies for effective communication. In this assessment, you'll need to evaluate these so you can recommend the most appropriate strategies, given your disaster management plan.
