    Surgical Package and Global Period

General surgeons have a wide variety of responsibilities that include treating injuries, correcting deformities, and treating diseases primarily through operations. General surgeons routinely perform many kinds of operations. Knowledgeable and highly trained in anatomy and a range of diseases, general surgeons perform operations on the skin to other body organs.

Imagine that you’re working for a busy general surgeon’s office. You are responsible for the scheduling and coding for many of the surgeries done in this office. The weekly reading provides you with some very useful information on the surgical package and global period.

Apply coding concepts related to surgical package and Global Period.

Include the following aspects in the discussion:

    · Provide a definition of “surgical package” and include what services might be included in addition to the actual surgical procedure.

    · Review CMS categorizes surgeries as major and minor and established 3 “global periods”.

    · What are the three global periods (not already provided in 3-2-1 Code It or mentioned in another student's posting)

    · Provide a surgery CPT code and description example for each of the three

    ·
Discuss why the Centers for Medicare and Medicaid Services (CMS) established these descriptions in 1992.

    WK3 QUESTION2 HP-216

Instructions: Assign the most appropriate CPT procedure code(s) including any needed modifiers.

    Question

    1) GALLBLADDER ULTRASOUND

REASON FOR EXAM: Right upper quadrant abdominal pain.

Multiple B-mode scans of right upper quadrant reveal good visualization of the gallbladder. There are several small, dense echoes in the dependent portion of the gallbladder with acoustic shadowing defects distally. The findings are suggestive of cholelithiasis. The rest of the abdominal cavity organs appear normal in size and appearance; this includes the liver, pancreas, spleen, and kidneys.

Assign CPT code(s): FILL IN BLANK

    2) X-RAY OF CERVICAL SPINE, ANTEROPOSTERIOR AND ODONTOID VIEWS; X-RAY OF LUMBOSACRAL SPINE, ANTEROPOSTERIOR, ODONTOID, AND OBLIQUE VIEWS

REASON FOR EXAM: Rule out lumbar fracture. Lumbar back pain.

No fractures or dislocations are evident. A total of four views are obtained of the lumbar spine, which show the lumbar vertebrae to be normal. The alignment is anatomical. The disc spaces and intervertebral foramina are well maintained.

IMPRESSION: Normal lumbar and cervical spine

Assign CPT code(s): FILL IN BLANK, FILL IN BLANK

    3) X-RAY, RIGHT RIBS

    REASON FOR EXAM: Rib pain.

Two views reveal no evidence of any rib fracture. The underlying lung is normal. The heart is moderately enlarged, and there is evidence of coronary artery surgery. Incidentally demonstrated is a 4-cm laminated gallstone in the right upper abdomen.

Assign CPT code(s): FILL IN BLANK

    4) X-RAY, LEFT WRIST; X-RAY, LEFT SHOULDER

REASON FOR EXAM: Pain, left shoulder. Check left wrist closed (lower end) fracture healing and left humerus (upper end) closed fracture healing.

LEFT WRIST: Anteroposterior and lateral views taken through plaster show that the fracture of the distal radius remains in excellent alignment.

LEFT SHOULDER: Multiple views of the left shoulder show that the fracture of the upper end of left humerus is in excellent alignment.

Assign CPT code(s): FILL IN BLANK, FILL IN BLANK

    5) X-RAY, CHEST, TWO VIEWS

REASON FOR EXAM: Shortness of breath.

The lungs are clear. No pleural effusion. The heart is normal. Pneumothoracic scoliosis is noted. In comparison with the previous examination of 10/10/YY, there is no essential interval change.

The patient has known pneumothoracic scoliosis and has long experienced shortness of breath. The patient's primary care provider monitors increased shortness of breath via chest X-ray to determine if it has become lung-related. For this encounter, the shortness of breath remains related to pneumothoracic scoliosis. Reason for chest x-ray is shortness of breath.

    SUMMARY: Negative.

Assign CPT code(s): FILL IN BLANK

    6) X-RAY, LEFT MANDIBLE

REASON FOR EXAM: Pain in jaw.

Three views of the left mandible show no definite evidence of a fracture. However, the mandible is not optimally visualized in this study.

Assign CPT code(s):FILL IN THE BLANK

    7) LIVER AND SPLEEN SCAN, WITH VASCULAR FLOW

REASON FOR EXAM: Abnormal liver function studies per laboratory report.

The liver and spleen were imaged under vascular flow. The liver is slightly enlarged, but good flow was seen. The spleen appears normal.

    Assign CPT code(s)FILL IN BLANK

    8) CT SCAN OF LUMBAR SPINE

REASON FOR EXAM: Lumbar back pain.

Axial images were obtained throughout L3, L4, and L5 levels using magnification technique. There is disc material protruding from the posterior aspect of L4 level on the right sde. This is causing almost complete obliteration of the nerve canal and measures roughly 6 mm in size. The remainder of this disc as well as L3 and L5 disc levels appear normal.

CONCLUSION: Findings consistent with a herniated disc at L4 level on the right side.

    DIAGNOSIS: Herniated disc, L4, right.

Assign CPT code(s) FILL IN BLANK

    9) X-RAY, LEFT ELBOW

REASON FOR EXAM: Elbow pain, left.

Anteroposterior and lateral views are normal.

Assign CPT code(s FILL IN BLANK

    10) X-RAY, LEFT SHOULDER

REASON FOR EXAM: Pain, left shoulder. Rule out fracture.

Anteroposterior and lateral views reveal a comminuted, slightly angulated, closed fracture of the neck of the humerus. I do not see any other acute abnormality. There is an old healed fracture of the left clavicle.

DIAGNOSIS: Comminuted nondisplaced closed fracture, neck of left humerus. (Initial encounter for closed fracture.)

Assign CPT code(s): FILL IN BLANK

    **## EvidenceBasedPracticePrepAssignment.docx**

    Step 1: Evidence Based Practice
    **Prep** Assignment

    · This will be due shortly after your first clinical week/weekend. See due date document for specifics.

    · You will need to answer and discuss each question below
**ON THIS TEMPLATE** and submit it to the appropriate assignment folder in D2L. Your clinical faculty will then indicate paper topic approval or disapproval. This is a REQUIRED pass/fail assignment.

    · Purpose: To gain skills in research. To determine EBP paper topic and receive instructor feedback as to whether or not topic is reasonable and acceptable for formal EBP paper.

    Assignment Overview:

    1. Choose a nursing intervention commonly used in nursing practice.  Use your clinical time to identify an intervention that you have seen that may not be supported by current evidence or that is not consistently followed. A suggestion might be to explore interventions listed in Nurse’s Pocket Guide in Nursing Central. There is also a resource in D2L of some possible EBP topics.

    · Example: The issue of testing pH of gastric contents for confirmation of NG placement is well researched and the problems of auscultation for insufflation of air are well documented, yet the practice continues, why?  Is the practice based on research, if not, why does it continue?

    · You will be doing research to determine what literature says SHOULD be done and if it’s not being done, analyze why it is not being done.

    ·
    **List your topic here:___________________________________________________________**

    2. Find and analyze one scholarly, research-based article that supports or challenges the use of this intervention and find and analyze one scholarly, research-based article that identifies a new evidence-based intervention to address the issue. (Minimum of 2 articles total)

    Article #1:

    Article #2:

    3. What key words did you use in the search bar?

    4. How many results came up during your initial search?

    5. How did you change/refine your initial search to get the information you needed?

    6. Is your topic too narrow or too broad? How will you expand or limit your scope in order to complete the requirements of the paper?

    7. Obtain approval of topic by instructor via D2L
    _PRIOR_ to beginning project.

    8. If the topic is not approved, work with instructor to refine and approve topic to begin paper.
