  • Case Study: An Asian American Woman. Diagnosis-Bipolar Disorder

    Bipolar Therapy

    Client of Korean Descent/Ancestry

    BACKGROUND INFORMATION

The client is a 26-year-old woman of Korean descent who presents to her first appointment following a 21-day hospitalization for onset of acute mania. She was diagnosed with bipolar I disorder.

Upon arrival in your office, she is quite “busy,” playing with things on your desk and shifting from side to side in her chair. She informs you that “they said I was bipolar, I don’t believe that, do you? I just like to talk, and dance, and sing. Did I tell you that I liked to cook?”

She weights 110 lbs. and is 5’ 5”

    SUBJECTIVE

Patient reports “fantastic” mood. Reports that she sleeps about 5 hours/night to which she adds “I hate sleep, it’s no fun.”

You reviewed her hospital records and find that she has been medically worked up by a physician who reported her to be in overall good health. Lab studies were all within normal limits. You find that the patient had genetic testing in the hospital (specifically GeneSight testing) as none of the medications that they were treating her with seemed to work.

Genetic testing reveals that she is positive for CYP2D6*10 allele.

Patient confesses that she stopped taking her lithium (which was prescribed in the hospital) since she was discharged two weeks ago.

    MENTAL STATUS EXAM

The patient is alert, oriented to person, place, time, and event. She is dressed quite oddly- wearing what appears to be an evening gown to her appointment. Speech is rapid, pressured, tangential. Self-reported mood is euthymic. Affect broad. Patient denies visual or auditory hallucinations, no overt delusional or paranoid thought processes readily apparent. Judgment is grossly intact, but insight is clearly impaired. She is currently denying suicidal or homicidal ideation.

The Young Mania Rating Scale (YMRS) score is 22

Assignment: Assessing and Treating Patients With Bipolar Disorder

Bipolar disorder is a unique disorder that causes shifts in mood and energy, which results in depression and mania for patients. Proper diagnosis of this disorder is often a challenge for two reasons: 1) patients often present as depressive or manic but may have both; and 2) many symptoms of bipolar disorder are similar to other disorders. Misdiagnosis is common, making it essential for you to have a deep understanding of the disorder’s pathophysiology. For this Assignment, as you examine the patient case study in this week’s Learning Resources, consider how you might assess and treat patients presenting with bipolar disorder.

    To prepare for this Assignment:

Review this week’s Learning Resources, including the Medication Resources indicated for this week.

  • NRS-440VN Topic 1 DQ 1 nursing shortage causes

    Topic 1 DQ 1

Discuss the events that have contributed (or will continue to contribute) to the nursing shortage, or that contribute to a shortage in a region or specialty. Discuss at least one way that the nursing profession is currently working toward a resolution of this problem. In replies to peers, offer different examples of how the nursing shortage has been addressed in your state, community, or specialty area.

    post

According to Whitney (2018), the American Association for Colleges of Nursing (AACN) has identified many factors that have contributed to the nursing shortage:

55% of RNs were 50 years or older in 2013, meaning many RNs will be retiring in the near future and at that rate, nursing school enrollments are not increasing fast enough to compensate for the demand

There is a nursing faculty shortage which has caused the restrictions on nursing program enrollments

Patients demographics are changing, people are living to be older and have more complex heath needs which indicates the need for more nurses

Due to insufficient staffing, job satisfaction is reduced, and nurses are getting burnt out – these stress levels are causing more nurses to leave the profession

High nurse retirement and turnover rates are directly impacting access to health care

The Institute of Medicine (IOM) established a goal that 80% of RNs are to obtain a BSN by 2020. The expectation of this goal was that the educational system would be seamless from one degree to the next while in turn, increasing diversity of students would create a workforce which was prepared to meet the demands of diverse population (Whitney, 2018). According to Boivin (2017), the profession is a ways off from meeting that goal, the number of BSN prepared nurses has doubled and now exceeds the number of ADN prepared nurses, so progress is being made. Nurses are encouraged to take this next step as some employers are offering incentives for their RNs to further their education. In addition, more hybrid or online RN to BSN completion programs are being offered which provide convenience to their students, allowing them to continue working full-time while completing their education from home.

The Healthcare system today in the US faces many challenges and obstacles, from technology to patient age. But few of the challenges facing in the US healthcare system are as serious as the nationwide nursing shortage. The lack of qualified nurses in the workforce has huge implications for patient care across America, especially as more people need nursing care and the demand for nurses grows all around the country(WGU,2020).

There are several elements that are contributing to the nursing shortage which includes growing Baby Boomer populations. As the largest generation in the country continues to age, there are more Baby Boomer nurses that are retiring, and they simply are not being replaced quickly enough by new nurses. Secondly, advancements in modern medical care that prolong life. While this is a good thing, it also means that people are living much longer than before and needing more care as they age. The nursing workforce simply is not large enough to handle the older people who are living longer due to better health care. Thirdly, lack of current resources to train new nurses. Insufficient numbers of nurse faculty and classroom space are leading to many nursing student applicants being turned away. Finally, many nurses leaving the field. Because there is a shortage of incoming nurses, there is a high demand for current RNs. Many RNs find they are scheduled many hours with lots of patients to cover for the shortages. This can lead to more stress and lower levels of job satisfaction and can drive nurses away from the profession (WGU,2020).

One of the solutions to overcome the nursing shortage is to use an onboarding program to make new nurses feel welcome. Studies and surveys show that community, group cohesion, and nurse autonomy reduce employee turnover among nurses, and nurses who feel they have a sense of community at work report higher levels of job satisfaction. When hospitals bring in new nurses, making these nurses feel welcome with an onboarding program can help encourage employee retention. A good onboarding program can ease new nurses into the job, so they are less overwhelmed in the first few weeks at a new job. A good example is to avoid assigning new nurses to problem patients right away. Another example is to consider events that help new nurses get to know current nurses to encourage a sense of community. Nurses that feel like they are welcome, and part of a community not only stick with the job, but they are also more likely to bring along their friends (Parker.S,2020).

    Week 4:
Therapy for Patients With Major Depressive Disorder (MDD
    )

Mood disorders can impact every facet of a human being’s life, making the most basic activities difficult for patients and their families. This was the case for 13-year-old Jeanette, who was struggling at home and at school. For more than 8 years, Jeanette suffered from temper tantrums, impulsiveness, inappropriate behavior, difficulty in judgment, and sleep issues
    .

As a PNP working with pediatric patients, you must be able to assess whether these symptoms are caused by psychological, social, or underlying growth and development issues. You must then be able recommend appropriate therapies.

This week, as you examine antidepressant therapies, you explore the assessment and treatment of three populations: pediatrics, adults, and geriatrics. The focus of your assessment tool, a decision tree, will specifically center on one of the most vulnerable populations, pediatrics. Please remember, you must also consider the ethical and legal implications of these therapies. You will also complete a Quiz on the concepts addressed throughout this module.

    Learning Objectives

    Students will:

Assess patient factors and history to develop personalized plans of antidepressant therapy across the lifespan

Analyze factors that influence pharmacokinetic and pharmacodynamic processes in pediatric, adult, and geriatric patients requiring antidepressant therapy

Synthesize knowledge of providing care to pediatric, adult, and geriatric patients presenting for antidepressant therapy

Analyze ethical and legal implications related to prescribing antidepressant therapy to patients across the lifespan

    Assignment:
Assessing and Treating Pediatric Patients With Mood Disorders

When pediatric patients present with mood disorders, the process of assessing, diagnosing, and treating them can be quite complex. Children not only present with different signs and symptoms than adult patients with the same disorders, they also metabolize medications much differently. Yet, there may be times when the same psychopharmacologic treatments may be used in both pediatric and adult cases with major depressive disorders. As a result, psychiatric nurse practitioners must exercise caution when prescribing psychotropic medications to these patients. For this Assignment, as you examine the patient case study in this week’s Learning Resources, consider how you might assess and treat pediatric patients presenting with mood disorders.

IOM Future of Nursing Report and Nursing

    IOM Report

Different entities in the medical field have put efforts to try and improve the condition in terms of the advancement in the nursing workforce, nursing practice and education. Robert Wood Johnson Foundation Committee Initiative is an entity that venture in deliberations of this kind. The essence of this paper is to describe Robert Wood Johnson foundation contributions to the issues in the medical field. It highlights the benefits of the Institute of Medicine. Also, the paper gives an overview of Arizona action coalitions. IOM Future of Nursing Report and Nursing.

The Robert Wood Johnson Foundation Committee Initiative was a wise idea towards directing the whole outline for work. The initiative aimed at the search for a completely reformed health care system to speak about the United States health care concerns. Based on the present day debate about the Affordable Care Act in Congress, there was a necessity and claim for quality care. Therefore, the foundation decided to enhance health care access to culturally diverse residents in the United States. The foundation was a shareholder in the health care industry.  It believed that health care providers need support in their roles. Particularly, it influenced nurses’ roles from the pharmacists and primary. Subsequently, in their report “Future of Nursing”, IOM and the foundation foresee a future that has primary care and prevention as the key facilitators of the health care system (Fineberg & Lavizzo-Mourey, 2013). The foundation committee and IOM concede that certain health elements must be reformed such as the nursing profession. The nursing workforce has the most significant percentage of people in health sector.

The foundation approached the Institute of Medicine to create and begin a strategy based on the future of nursing. They released a report that recommended for action-based solution for future nursing. The action-based solution was to involve an adjustment in the institutional and public rules at the local and national levels. The committee, in their report, suggested specific actions after the examination. The actions were to point out the important roles of nurses in designing and enacting an effective and efficient health care delivery system (Fineberg & Lavizzo-Mourey, 2013). The Robert Wood Foundation further proposed understanding nurses’ roles in the context of societal issues, workforce sector, present, and future technology. Also, the committee noted about growing the nursing faculty by restructuring the whole nursing education and the capacity in nursing school to produce adequate and competent nurses. In doing so, they would cater to the present and future concerns in health. Examination of health professional education and health care delivery solution’s with improvement element is considered a main feature in the recommendation list. Lastly, the committee was concerned with attracting and sustaining well-baked nurses in the health care settings.

IOM took note of the above commendation after concurring and appreciating the unique roles of nurses within the healthcare settings. IOM report is important to nursing practice because nurses are the health care providers who spend the most time with patients and stay close to them (Dzau, 2016). Nurses have the scientific comprehension of health care procedures in the healthcare field. With that, they can redesign and improve the health care system. For a fact, the commendation had the intention of decreasing the gap between coverage and access. IOM Future of Nursing Report and Nursing.

The first main message from the IOM report suggested that advanced practice registered nurses ought to perform their duties to the maximum magnitude of their training and education. Most nurses work as a registered nurse. Some of the registered nurses are advanced practice registered nurses (APRNs) having passed certification examination. The committee awards licenses to nurse anesthetists, clinical nurse specialists, and nurse practitioners. The report also challenged the strict nursing practice and licensing rules that vary depending on the state. IOM did not exempt the rule concerning the practice scope that defined roles of a qualified nurse. IOM is significant to nursing practice since it provides recommendations for shareholders from the department of state lawmakers to uphold that nurses can conduct their responsibility to the full capability of their education and schooling (Borger, 2012). The government influences the transformation of the initially recommended regulations on the range of practice by giving incentives to adopt better practices.

With relation to nursing education, the IOM report set a goal with the purpose of expanding the index of the baccalaureate of nurses to eighty-six percent by 2025 (Kershaw, 2012). Also, it aims at elevating the number of nurse staff with a doctorate that would represent the demand from a more educated workforce to take care of the present and future population. IOM dwells on forming student-centered policies. The policies comprise of common curriculums and standard competencies. As a result of such policies, nurses with the will to continue with their education do need to refresh themselves on some of the courses. Leadership has realized the need for more nurse practitioners. IOM also has engaged the American Nurses Credentialing Center to commence providing a plan that boosts the numerical figures of baccalaureates prepared nurses.

In nursing workforce development, the Institute of Medicine has displayed undoubted significance in its provision. The report recommends a better-quality infrastructure and explores workforce information as a requirement for assessment by the workforce (Fineberg & Lavizzo-Mourey, 2013). Besides, the report assigned the formation of a National Health Workforce to help approximate the claims from health care workers. It supports the nursing staff in data collection. IOM also has enabled the monitoring of the recruitment of healthcare providers across many disciplines.

The initiatives in Arizona Action Coalition are the campaign for patient-centered health care and action to facilitate the provision of high-quality care, with nurses practicing to their maximum capacity. Arizona also works to enact the commendations of the IOM report. The campaign functions to create and sustain reforms required for improvement in health care for all United States citizens. One barrier to the advancement in Arizona is the high turnover rate of nurses. A majority of nurses below fifty years leave the healthcare profession because of reasons such as family and personal issues, or burnout during the profession. Nursing advocates should suggest that the scope for Advanced Practice Registered Nurses’ be widen in a manner that they could perform their duties to the full degree of learning and schooling in spite of variations of policies in different states. By doing so, nurses’ duties would be decreased hence, patients would get more quality efficient care. Since the patient is an essential individual in healthcare, he or she should get care at the optimum level. Also, Arizona can retain nurses if it eases the ability of nurses to acquire advanced degrees as they work. Accessibility of education would heighten their competency and morale. IOM Future of Nursing Report

    Introduction

Nurses comprise the biggest fragment of the health care profession, with around three million registered nurses in the US.  They offer a range of services involving health promotion, direct patient care, coordination of care, and patient education.  They are researchers, serve in leadership roles, and work towards the improvement of health policy. As the health care delivery system goes through reformation, partly as a result of the ACA, the profession of nursing is having a widespread impact by offering accessible and patient-centered at an affordable price. The IOM report offers recommendations for nursing the reformed health care system. The paper discusses the IOM report has influenced nursing education and training, practice, and role development and the role of state-based action coalitions in promoting implementations contained in the report.

The work of the Robert Wood Johnson Foundation Committee Initiative that led to the IOM report

In 2008,  the RWJF entered into a partnership with the IOM to institute a two-year study to assess the profession of nursing, recognizing that redesign of the health care system would require nurses to be active, and visibly due to their numbers.   According to Altman et al. (2019), The RWJF committee was given the task of examining if the nursing workforce was in a position to fulfill the demands or patients of a transformed health care system.  It was to develop a series of recommendations, entailing recommendations that address the provision of nursing care a transformed health care system, and the education system to adequately prepare nurses.

The four “Key Messages” that structure the IOM Report recommendations

     First key message

The first key message is that nurses should practice at the top of their training and education. The committee performing the study established that policy, regulatory and historical barriers have hindered nurses from performing a wide range of tasks for which their training and education have prepared them.  Kimuyu (2018) indicates that the committee devised recommendations for state legislatures, Congress as well as several federal entities on actions that could be carried out to eliminate barriers to practice. Also, the committee established that new nurse graduates could gain from further help in transitioning to practice, and proposed the   creation of residency programs to assist newly graduated nurses to further refine their skills NRS-440VN – IOM Future of Nursing Report and Nursing

This message has influenced nursing practice by enhancing the removal of practice barriers to enable nurses to practice at the top of their training and education. In 2013, fifteen had made changes in their laws to grant NPs full authority to practice and prescribe (Hansen-Turton, 2015).

    Second key message

The second key message is that nurses should attain higher levels of training and education via an improved education system that supports flawless academic progression.  According to Altman et al. (2026), the committee proposed that a high number of nurses advance their education so that by the year 2020, 80%  of the nursing workforce nurses would possess a baccalaureate degree, with the with a doctoral degree doubling. Additionally, the committee proposed that nurses embark on lifelong learning all through their careers, and the diversity of the workforce be increased.

The message has led to a rise in baccalaureate degree programs and the number of students enrolling in and graduating with baccalaureate degrees. Additionally, there has been development or expansion of residency programs in different levels and settings. For instance, over 130 health systems and hospital s across the United States are presently participating in the AACN/UHC program, with yearly participation having risen to 9,000 in 2014 from 3,579 in 2010.   In 2002, AACN and the university healthsystem consortium (UHC) have created a residency program for nurses who have completed their baccalaureate degree programs (Altman et al., 2016).

    Third key message

The third key message is that nurses should fully partner with physicians and other health professionals in revamping health care in the US. According to Altman et al. (2016), the study committee established that for nurses be capable of fully take part in reforming the health care system, it will be essential for them to take leadership roles and work in collaboration with other health professions’ leaders. The committee proposed that education programs, financiers, along with health care organizations offer, fund, and expand opportunities for the nursing workforce to build up leadership skills and take on leadership roles, and that people who make health care decisions make sure that nurses leaders in both in management teams and boards.

There has been development and expansion of opportunities for leadership development by nursing associations, education programs, and private organizations.  As indicated by Altman et al. (2016), nursing associations vigorously support programs designed to enable nurses to skills in leadership and entrepreneurship.  For instance, the American Organization for Nurses executives offers a variety of nursing leadership education along with leadership development programs, for example, the nurse manager fellowship.

    Fourth key message

The last key message is that effective planning of the workforce and making of policy needs an improved collection of data along with enhanced information technology. The committed found that planning and preparation of essential transformations in the system, necessitates granular and reliable data on the health care labor force.  The committee suggested that the National Health Care Workforce Commission work together with the Health Resources and Services Administrations in improving data collection and analysis on the health care labor force (Altman et al., 2016).

The role of state-based action coalitions

The Center for Champion Nursing in America and the Association of Retired people launched state-based organizations referred to as Action coalitions in all states as well as the District of Columbia as the foundation for the implementations of the IOM recommendations. State-based action coalitions play the role of advancing the recommendations contained in the IOM report at local and state levels (Friberg, 2019).  Action coalitions are regarded as the driving force for the Future of Nursing (FON) campaign since they are capable of working as a network to enhance transformation at the local level. NRS-440VN – IOM Future of Nursing Report and Nursing.

The initiatives on which the California action coalition is working

The California Action Coalition is the impetus for the execution of recommendations of the IOM report. The Action Coalition has partnered with HealthImpact to increase the sustainability of both groups and perform the universal goal of positively influencing health care is delivered in California. The coalition has also engaged in working to promote leadership opportunities for nurses.  There is an active mentoring program in the state, which is administered by the Association of California nurse leaders.  These initiatives advance the nursing profession by advancing public health and recognizing the role of nurses as leaders in the transformation of health care.

Barriers to advancement that currently exist in California

Lack of full prescriptive and practice authority are barriers to advancement in California. NRS-440VN – IOM Future of Nursing Report and Nursing. According to Phillips (2020), nurse practitioners in California must adhere to standardized protocols when carrying out medical functions or when ordering or prescribing devices and drugs.  The protocols are created and consented by the physician, nurse practitioner, and the management of a health care organization in which the practitioner is employed. Opposition from physicians and lack of awareness by the patients and the general population on nurses’ competencies are other barriers to nursing advancement

Nursing advocates overcome thing barriers by lobbying policymakers to amend the current laws to allow nurse practitioners to practice and prescribe medications without being supervised by physicians. The advocates are also taking the lead in informing and educating physicians about the scope of nurse practitioners and their levels of education and educating patients and the general public about the responsibilities and roles of nurse practitioners.

    Conclusion

The IOM report explores how the education, responsibilities, and the roles of nurses should significantly be changed to meet the high demand for care that will occur as a result of a reformed health system and to promote improvements in the United States health system. The report contains key messages related to nursing practice, nursing education, partnerships for progression, and date on health care labor force. . State action coalitions play the role of advancing the IOM recommendations at local and state levels.  The report has greatly influenced nursing education and practice. NURS 6512 Building a Health History week 1 discussion

Good communication techniques can encourage patient disclosure and break the barriers that block and encourage patient apprehension. It can cause the patient to recuse themselves from seeking and receiving medical care. Practitioners should be versatile in their approach and interviewing techniques to allow the foundations needed to develop the patients’ health history, especially those from diverse cultural backgrounds. Some techniques that can be used are self-disclosure, expressing empathy, and involving the patient in the decision-making process. Finding a commonality allows for open dialogue between the patient and the provider. It enables the patient to talk about their fears without judgment and for both to be able to ask open-ended questions; offers the provider to practice patient-centered care and to actively listening to both parties’ wants and needs (Padela & Zaidi, 2018).

In this case, the patient is a 26-year-old Lebanese female living in graduate student housing who visits the medical clinic. Understanding the Lebanese culture and their practices will help the practitioner provide care and treatment (Attum, 2020). Cultural competency plays a major role in defining the medical relationship between provider and patient. It is defined as having the ability to provide care to those with differing values, beliefs, and behaviors by incorporating the practice of providing care that meets the patient’s needs (Hembley, 2015). Recognizing your patients’ cultural demographics is an empathetic approach to treatment and shows that the practitioner acknowledges and values that individual patient. It allows a trusting connection to be formed, enabling pertinent information to be exchanged and received between the provider and patient on a scale that meets the patient’s overall needs (Hembley, 2015). Once the patient connects, they experience reduced stress during and post-visits. They have better adherence to treatment, which allows for better outcomes; the practitioner also has a renewed sense of meaning and fulfillment and fewer burnout experiences (Hembley, 2015). When a provider’s approach is expressed through sensitivity to their patients’ cultural differences, it sets up a patient-centered relationship. For adequate diagnosis and treatment, the patient has to open up and discuss health-related issues that may disrupt their cultural beliefs and values. For example, many women from this region of the world view gynecological examination as an invasion against their modesty and virginity for unmarried women (Padela & Zaidi, 2018). Patient education will only be accepted if the patient is willing and motivated to participate in the learning process. For providers, the first step is to find a way to connect and be accepted by the patient; this can be accomplished by showing empathy towards their belief system.

There is an unmet need for guidelines for young women’s health and wellness practices in general from this population, making it imperative to evaluate the patient’s health risks and hazards. A health risk assessment helps both patient and doctor identify areas of concern (Hembley, 2015). For successful care and treatment, a trusting relationship between the patient and provider is essential to their needs. It allows the patient to be accountable for their actions and be an active participant in their health and well-being. Enabling them to identify health-related issues as they arise and immediately address them with their provider for swift treatment and better outcomes.

According to the Centers for Disease Control (CDC), Lebanese women’s reproductive health needs to be improved. The CDC states that more than 50% of Lebanese women between the ages of 18 and 45 do not use any form of birth control. Women from this region are more prone to developing abnormalities such as dysmenorrhea, severe pelvic pain, and injury, along with other preventable and treatable medical comorbidities like anemia, hypertension, and diabetes (CDC, 2017).

There are premeditated barriers to receiving reproductive or gynecological care due to different cultural values and beliefs. Some of the clinical challenges for many of these women are based on having same-sex providers, their need for modesty in the clinical setting, and respect for their religious obligations (Padela & Zaidi, 2018). These barriers can be detrimental to this population if patient-doctor relationships and communication are not improved. Finding common ground is imperative for preventing pathologies as cervical cancer, infertility, and sexually transmitted diseases (Padela & Zaidi, 2018). Because of the barriers preventing women from this region to seek medical care, providers need to be competently aware of the risks associated with a lack of knowledge of cultural diversity and values.

    Discussion: Building a Health History

Effective communication is vital to constructing an accurate and detailed patient history. A patient’s health or illness is influenced by many factors, including age, gender, ethnicity, and environmental setting. As an advanced practice nurse, you must be aware of these factors and tailor your communication techniques accordingly. Doing so will not only help you establish rapport with your patients, but it will also enable you to more effectively gather the information needed to assess your patients’ health risks.

For this Discussion, you will take on the role of a clinician who is building a health history for a particular new patient assigned by your Instructor.

    To prepare:

With the information presented in Chapter 1 of Ball et al. in mind, consider the following:

By Day 1 of this week, you will be assigned a new patient profile by your Instructor for this Discussion.
    Note
: Please see the “Course Announcements” section of the classroom for your new patient profile assignment.

How would your communication and interview techniques for building a health history differ with each patient?

How might you target your questions for building a health history based on the patient’s social determinants of health?

What risk assessment instruments would be appropriate to use with each patient, or what questions would you ask each patient to assess his or her health risks? NURS 6512 Building a Health History week 1 discussion

NURS 6512 Building a Health History week 1 discussion

    Building a Health History

The patient is a 26-year-old Lebanese female student living in graduate student housing. Communication techniques would first be to establish preferred primary language to communicate in. According to Ball et al. (2019) states, “maintain eye contact and use a conversational tone” (p.1). Communication should be aimed at building a patient-centered rapport and obtaining a clear outlook of the patient’s condition and healthcare morale. My communication approach would be respectful and polite to build an ideal nurse-patient relationship successfully. I would then assess the patient for learning needs and learning requirements aimed at health promotion by using a language line interpreter. The patient’s learning needs, abilities, preferences, and readiness to learn are then assessed. Major assessment parameters would target social determinants such as cultural and religious practices, emotional barriers, desire and motivation to learn, physical and cognitive limitations, financial concerns, and identifying language barriers. I would update learning methods so the entire multidisciplinary team that teaches the patient can be aware of the education assessment entirety and refer to this before teaching the patient and family.

    Risk Assessment

It is important to identify the appropriate health risk assessment strategies in patients with different cultures and religions related to their definition of health promotion and disease prevention. Based on the information is given, I would use a healthy female adult risk assessment to ask questions to identify healthy female habits and practices related to Lebanese women’s culture. The purpose of the risk assessment is to build a collection of data related to the patient condition and risk for developing related medical conditions. Questions are aimed at demographics, lifestyle, personal health history, family health history, and biometrics (such as blood pressure, weight, cholesterol, etc.); other data includes genetic/genomics and individual preferences, according to Wu and Orlando (2015) NURS 6512 Building a Health History week 1 discussion.

    Potential Health-Related Risk

Language barriers and culture should always be considered as they can pose a potential health-related risk due to the misinterpretation or misunderstanding of information and strategies provided. Sah et al. (2019), “many migrants may be unfamiliar with the norms and practices of a new country where sociocultural and geographical circumstances can have a huge impact on their health (p. 65). Effective communication is the primary goal of preventing immigrants’ health-related issues and producing positive outcomes associated with potential health risks. It leads to better health outcomes and wiser allocation of resources, and greater patient and family satisfaction.

    Five Targeted Questions

?When was your last OBgyn visit?

    ?Last normal menstrual cycle?

    ?Any pregnancies?

    ?The use of contraceptives?

    ?Sexual history?

NURS 6512 week 1 Discussion: Building a Health History

Developing or building an accurate and detailed patient history is a cornerstone of initiating an appropriate plan of care for any patient and is a skill that is essential for the APRN. In this way, we get to know our patient’s, their past concerns and issues, and how past problems impact current functioning. A complete health history is also necessary when collaborating with other healthcare clinicians or when there is a transfer of care. Factors such as age, gender, ethnicity, and living environment all should be considered since they greatly impact a patient’s health status. When interviewing a patient, whether it is a first encounter or follow-up care, communication, interviewing techniques, and other tools are necessary to ensure that the necessary information is gathered and understood to meet the needs of the patient. For the purpose of this discussion, the patient in question is a 14 year old biracial male living with his grandmother in a high-density public housing complex.

Whenever meeting with any patient, I prefer to use a method called AIDET which is as follows: Acknowledge and greet the patient by name, make eye contact, smile, and include any others who are present; Introduce yourself, your position, and background or experience; the Duration that the interview and exams will take; Explain what steps are next and what the patient can expect from the visit; and lastly, Thank the patient and any family or friends for their time and consideration (Studer Group, 2020).. These steps can be completely in a few sentences, take less than five minutes, and can provide reassurance to nervous patients who don’t know what to expect.

While AIDET is an appropriate tool that can be used for most patient and situations, other communication and interview techniques may differ depending on the specific patient. For example, since this patient is an adolescent, I would first let him and his grandmother know that I will be speaking with them together and privately. While the grandmother may be necessary for background information, especially when the boy was younger, most adolescents are able to give an accurate health history and may speak more freely without the guardian if they are given the opportunity. However, adolescents may have difficulty choosing their words or may feel embarrassed so they should be given time to express themselves without confrontation, to which they do not respond well (Bell, Dains, Flynn, Solomon, & Stewart, 2019). In addition, there are specific topics that should be discussed with patients and their caregivers which are age-appropriate and referred to as anticipatory guidance (Sullivan, 2019). For example, topics to focus on for patients aged 10 to 14 years include safety issues, nutrition, dental hygiene, peer pressure, puberty, safe sex/contraception/STD prevention, safety rules with adults, communication, screen time, self-control, depression/anxiety, tobacco/alcohol/substance use, educational goals and activities, and after school activities, and supervision. NURS 6512 week 1 Discussion: Building a Health History

When interviewing any patient, social determinants of health should be considered and questions should be targeted towards identified areas of concern. Social determinants of health are socioecological factors such as gender, religion, ethnicity/race, sexual orientation, mental health, geographical location, education, income, employment, disabilities. Discrimination related to these factors cause disparities that negatively impact health and outcomes (Tebb, Pica, Twietmeyer, Diaz, & Brindis, 2018). Questions for this particular patient would be aimed towards factors such as his biracial status, being reared by a grandparent, and highly crowded public housing. Specific issues related to diseases in overcrowded environments such as tuberculosis should be assessed. I would also carefully monitor growth, milestones, and dietary habits since many individuals in urbanized areas live in food deserts without ready access to fresh, healthy, unprocessed foods.

When assessing a patient’s risk factors, especially once disparities in social determinants of health are identified, it is helpful to have a specific screening tool tailored to the demographic in question. For example, studies show that adolescents tend to have more social risks than medical ones. Therefore, tools such as The Guidelines for Adolescent Preventative Services (GAPS) was developed that targets primary and secondary interventions to prevent adolescent morbidity and mortality by improving health-care delivery (Sullivan, 2019). Risk taking behaviors in teens are identified, such as smoking, drinking, or unprotected sex, by 24 topics that examine health guidance, screening, immunizations, and health-care delivery. Another tool that was developed to assess drug and alcohol use in adolescents is the CRAFFT Questionnaire. CRAFFT refers to car, relax, alone, forget, friends, and trouble (Ball et al., 2019).

As mentioned above, adolescents tend to have more social risks than medical ones. Therefore, an assessment tool such as the HEEADSSS is helpful in obtaining an accurate psychosocial history for this age group. The acronym HEEADSSS stands for home, education/employment, eating, activities, drugs, sexuality, suicide/depression, and safety and encompasses many of the same social determinants (Sullivan, 2019).

Five specific questions targeted towards my patient to assess his health risks and build his health history are as follows: 1) Do you know anyone who has committed suicide? While suicide can be an uncomfortable subject, it is a common cause or mortality in teens, particularly in males, and should be assessed (Bell et al., 2019). This is a non-threatening question that may open the conversation and lets the teen know that this is a safe topic; 2) Are you sexually active? Adolescence is a time for experimenting and risk-taking behaviors. Children are becoming sexually active at younger ages and should be educated regarding safe sex, contraception, and disease prevention; 3) Do you safe at home? This question is a great open-ended question that can start the conversation and lead to a variety of safety-related issues. For example, does the boy feel safe with his grandmother? Answers to the contrary could indicate abuse or neglect. Maybe he does not feel safe in his living environment due to crowding in a public housing complex; 4) Tell me what you had for breakfast this morning? This question not only assesses the patient’s memory recall, but also may give an indication of nutrition and dietary habits; 5) Is there anything we have not talked about that you think I should know? Oftentimes the patient, especially ones who are young, may expect the clinician to guide the interview and answer questions that are posed. They may be more reluctant to initiate their own topics or concerns and this gives them to opportunity to have the floor to ask or discuss anything in a safe place.

Week 1: Building a Comprehensive Health History

According to a 2011 Gallup poll, nurses are ranked as the most trusted professionals in the United States. One of the most admired nursing skills is the ability to put patients at ease. When patients enter into a healthcare setting, they are often apprehensive about sharing personal health information. Caring nurses can alleviate the hesitance of patients and encourage them to be forthcoming with this information.

The initial health history interview can be an excellent opportunity to develop supportive relationships between patients and nurses. Nurses may employ a variety of communication skills and interview techniques to foster strong bonds with patients and to effectively facilitate the diagnostic process. In conducting interviews, advanced practice nurses must also take into account a range of patient-specific factors that may impact the questions they ask, how they ask those questions, and their complete assessment of the patient’s health.

This week, you will consider how social determinants of health such as age, gender, ethnicity, and environmental situation impact the health and risk assessment of the patients you serve. You will also consider how social determinants of health influence your interview and communication techniques as you work in partnership with a patient to gather data to build an accurate health history.

    Learning Objectives

    Students will:

Analyze communication techniques used to obtain patients’ health histories based upon social determinants of health

    Analyze health-related risk

My patient is a 40-year-old black male who is a recent immigrant from Africa. He currently does not have any medical insurance. Ball et. al. (2019) mentions how the practitioner should create an environment during the examination that promotes comfort and a connection with the patient. Since the patient is from another country, he might not be to familiar with how the process works in America. I am sure he has many barriers in his mind about seeking out health care. It is the responsibility of the practitioner to try to make the patient at ease in the interview and ask questions that get the best information from the patient’s medical history. NURS 6512 week 1 Discussion: Building a Health History

I would start by asking the patient open-ended questions. Ball et. al. talks about many ways to ask such questions. A question I would start with is, ‘How can I help you today?’ I think this is a strong starting point for this patient. It gives enough room with the patient to start speaking about that on his mind. Since the patient has no insurance, he might have questions to how he is going to pay for this visit. Sebastia et. al. (2016) talks about how immigrants that come to another country from a different country can have social and economic disparities. Related to this. I will ask the patient if they have any questions about finical assistance programs. Another point to ask is how does his current finical situation effect his willingness to seek medical help in the future. NURS 6512 week 1 Discussion: Building a Health History

In Africa, there is a major problem with HIV infections. Belachew et. al. (2020) discusses how a large potion of infants with HIV can be found in East Africa. I would use the five Ps of sexual history mentioned by Ball et. al. (2020). It is important to see if the patient might have been in contact with a HIV positive individual. One of the questions include how many sexually partners the patient as had. Another question is if the patient used protection. during the sexual encounters. Its is important to establish is safe zone with the patient before hand so he is ok with talking about the subject. NURS 6512 week 1 Discussion: Building a Health History

    case study

TJ, a 32-year-old pregnant lesbian, is being seen for an annual physical exam and has been having vaginal discharge. Her pregnancy has been without complication thus far. She has been receiving prenatal care from an obstetrician. She received sperm from a local sperm bank. She is currently taking prenatal vitamins and takes over the counter Tylenol for aches and pains on occasion. She has a strong family history of diabetes. Gravida 1; Para 0; Abortion 0

    Discussion post

It has been predicted that by 2050, 50% of the US population will consist of minorities and unfortunately, today’s model of healthcare has been noted to have persistent racial and ethnic discrepancies (Sharma et.al, 2016). Culturally competent patient care includes knowledge, attitudes, and skills that support caring for people across different languages and cultures. In healthcare, it is important to know how to provide quality patient care for all patients from different race’s, disability, socioeconomic status, sexual orientation, gender, nationality, education, religion, ethnicity, veteran status, and physical characteristics (Whitman, 2018).                   Assessment Needs

Lesbians and bisexual women face the same health issues as heterosexual women but there can be difficulty when accessing appropriate care (Carlson, (2020). An assessment for this patient is the same as it is for a patient that is not a lesbian. The assessment paperwork should have language about sexual orientation, sexual activity, relationship status, gender identity and sex assigned at birth included. All educational materials that are provided for patients should have inclusive images and languages for all ages, genders, races, sexual orientation, and literacy levels as well.

Quality patient care can be improved by recognizing any barriers that could exist for our patients so that we can create a therapeutic relationship with them by ensuring that an inclusive and nonjudgmental history is completed (Friedman, (2020). During the assessment process, it is always important to maintain a standard systematic inquiry that includes, social activities, surgeries, past medical history, current medications, vaccinations, etc. It is also important that patients are provided with privacy, removing distractions, communicate with patients with open-ended questions, and be prepared to provide clarification as needed. Enacting CORE communication with all of our patients will help to establish trust and transparency. CORE stands for Caring – feel and exhibit concerns for others, Open – desire to listen and receive information; and understand another person without prejudice and judgment, Respectful – treat others with consideration, courtesy and high regard and Empathetic – understand others experiences and communicate understanding (Friedman, (2020).

    Risk Factors

Statistically, there is an elevated risk for stigmatization and discrimination that can pose to be a threat to this patient’s health and wellbeing which can lead to her being at a higher risk for suicide, becoming a victim of crime, substance use and participating in perceived culturally less accepted behaviors (Friedman, (2020).  It is important to maintain positive interactions and be sensitive to our body language as we respect word choices to avoid creating any barriers that would hinder the patient’s transparency (Friedman, (2020).

This case study indicates that the patient is a lesbian and she is pregnant through donor insemination which is a fertility treatment using a sperm donor (American Pregnancy Association, 2020). It is important during the assessment not to assume anything. When asking the patient to tell her story it is important to inquire about her sexual history and sexual behavior that includes sexual safety practices, age of menarche and if the patient has any sexual concerns she would like to discuss.  It is important to have the patient discuss who she is intimate with since it may not be congruent with sexual orientation or gender identity. We should not assume the parameters of sexual behavior based on the knowledge provided about the patient’s relationship status and identity.

    Patient Complaints and Possible Diagnosis

This patient is complaining of having vaginal discharge. Inquiring when the discharge began, what it looks like if there is an odor, and if there are symptoms such as burning, itching or urinary frequency will help to narrow down the different causes and determine what tests may need to be completed and what treatments. There are several different causes for the discharge to explore. The number one cause that needs to be ruled out is related to the body’s natural response to the pregnancy. There is a normal vaginal discharge that can vary in consistency, thickness, frequency, and amount during pregnancy called leukorrhea (Healthline, 2020). Leukorrhea is thin, clear or milky white, and mild smelling and as the pregnancy progresses the discharge becomes more noticeable as it becomes heavier (Healthline, 2020).  It is important to make sure that the assessment questions regarding the discharge characteristics are accurate since the abnormal discharged that could signal complications during pregnancy needs to be ruled out. For instance, if the discharge is bright red and exceeds an ounce it could indicate complications of the pregnancy such as placenta previa or placental abruption (Healthline, 2020).

There are other causes for an increase in vaginal discharge which can be explored such as Candidiasis (yeast infection), sexually transmitted diseases (STDs) like Bacterial vaginosis (BV), and Trichomoniasis). Yeast infections during pregnancy are not uncommon especially during the second trimester and can be caused by the hormonal changes that are occurring. It is difficult for the body to keep up with the chemical changes in the vagina as there is more sugar in the vaginal secretion where yeast can feed (American Pregnancy, 2020). Bacterial vaginosis (BV) is more common in lesbian and bisexual women than heterosexual women and frequently occurs in both members of lesbian couples (WebMed, 2020). When the normal bacteria in the vagina become out of balance BV can develop and present itself with or without symptoms. Symptoms can include vaginal discharge with a fishy odor and vaginal itching. BV can increase a woman’s chance of getting other STDs such as
    HIV
    ,
    chlamydia
    ,
    gonorrhea
    , and
    pelvic inflammatory disease
(WebMed, 2020). Trichomoniasis is passed from one person to another during sexual contact but has a yellow, green, or gray vaginal discharge that is foamy and has a strong odor.

    Conclusion

Making sure that the assessment is thorough and all bases are covered will help to ensure that the patient is properly cared for and her pregnancy continues without complications. Providing the patient with appropriate outside support if deemed necessary through the assessment and making sure all paperwork and materials are supportive and compatible with her needs will ensure patient confidence and transparency in the care that is being provided.

Week 2: Functional Assessments and Cultural and Diversity Awareness in Health Assessment

Diversity is not about how we differ. Diversity is about embracing one another’s uniqueness.

    —Ola Joseph

Countless assessments can be conducted on patients, but they may not be useful. In order to ensure that health assessments result in the necessary care, health assessments should take into account the impact of factors such as cultures and developmental circumstances.

    Learning Objectives

    Students will:

Analyze diversity considerations in health assessments

Apply concepts, theories, and principles related to examination techniques, functional assessments, and cultural and diversity awareness in health assessment

    Discussion: Diversity and Health Assessments

May 2012, Alice Randall wrote an article for
    The New York Times
on the cultural factors that encouraged black women to maintain a weight above what is considered healthy. Randall explained—from her observations and her personal experience as a black woman—that many African-American communities and cultures consider women who are overweight to be more beautiful and desirable than women at a healthier weight. As she put it, “Many black women are fat because we want to be” (Randall, 2012).

Randall’s statements sparked a great deal of controversy and debate; however, they emphasize an underlying reality in the healthcare field: different populations, cultures, and groups have diverse beliefs and practices that impact their health. Nurses and healthcare professionals should be aware of this reality and adapt their health assessment techniques and recommendations to accommodate diversity.

In this Discussion, you will consider different socioeconomic, spiritual, lifestyle, and other cultural factors that should be taken into considerations when building a health history for patients with diverse backgrounds. Your Instructor will assign a case study to you for this Discussion.

    To prepare:

Reflect on your experiences as a nurse and on the information provided in this week’s Learning Resources on diversity issues in health assessments.

By Day 1 of this week, you will be assigned a case study by your Instructor.
    Note
: Please see the “Course Announcements” section of the classroom for your case study assignment.

Reflect on the specific socioeconomic, spiritual, lifestyle, and other cultural factors related to the health of the patient assigned to you.

Consider how you would build a health history for the patient. What questions would you ask, and how would you frame them to be sensitive to the patient’s background, lifestyle, and culture? Develop five targeted questions you would ask the patient to build his or her health history and to assess his or her health risks.

Think about the challenges associated with communicating with patients from a variety of specific populations. What strategies can you as a nurse employ to be sensitive to different cultural factors while gathering the pertinent information?

By Day 3 of Week 2

    Post
an explanation of the specific socioeconomic, spiritual, lifestyle, and other cultural factors associated with the patient you were assigned. Explain the issues that you would need to be sensitive to when interacting with the patient, and why. Provide at least five targeted questions you would ask the patient to build his or her health history and to assess his or her health risks.

Respond on or before Day 6

    on 2 different days
    to at least
     two
of your colleagues who were assigned a different patient than you. Critique your colleague’s targeted questions, and explain how the patient might interpret these questions. Explain whether any of the questions would apply to your patient, and why.

Diversity is the description of patient population characteristics that refers to a combination of certain dimensions of differences such as socioeconomic, spiritual, lifestyle, and other cultural factors (Celik et al., 2008). TJ represents a diverse population as the National Survey of Family Growth statistical data suggests that 1.1%  of women identify as lesbian (Martos, et al., 2017).  During this healthcare appointment, since it is helpful and important to recognize the client as a unique individual, and respect their diversity, I would build a health history by focusing questions without bias and framing them to be sensitive to the patient’s background, lifestyle, and culture (Saha, 2014).  It is important to recognize that barriers in communication or feelings of discrimination already exist within this diverse population (Martos et al., 2017). and current healthcare practices are structured in ways that support the neutral approach and keep dimensions of diversity invisible and hidden but acknowledging that differences matter can have clinical implications that are immensely helpful to patient outcomes (Celik et al, 2008). Some diagnosis gets missed based on the lack of identifying that differences matter, and overall, the most important concern is the patient’s healthcare.

As a clinician, I would be kind and compassionate and create a comfortable interview environment for TJ noting that this diverse patient population already has a higher level of unpredictable social stress than are others because of the stigmatization of homosexuality in American culture (Mays & Cochran, 2001).  Additionally, being in a healthcare exam is already incredibly stressful enough (Ball et al., 2019). My questions would tailer around comfort and unbiased care and concern to identify and treat the patient’s concerns. Since this is an annual physical exam, and she is followed closely with obstetrics, I would focus on her needs that she currently discusses and ask her five basic questions that would help in this area. NURS 6512 Discussion: Diversity and Health Assessments

    1. How would you prefer to be addressed (Ball et al.,  2019)?

    2. Are there any concerns that you want me to focus on that will help you today? I noticed you identified that you have been having vaginal discharge, can you tell me more about this (de Fonseca et al., 2013)?

    3. Tell me about your support system for your pregnancy (Appelgren et al., 2018)?

    4.  Do you have any health problems or medical conditions not related to pregnancy?

    5. Are you eating well and exercising to maintain mental and physical health for you and the baby (Weiss & Elmore, 2017).

Diversity care is associated with communicating with patients from a variety of specific populations will be sensitive to different cultural factors when gathering pertinent information (Laureate, 2019). By being able to fully communicate the issues of this patient and ask non-bias questions, but aware of cultural sensitivity to come up with the best plan for this patient. Introduction

Cultural competency is essential in healthcare as it will help develop a relationship of trust with the patient. Being a culturally competent provider means that you have a curiosity and respect for a patient’s beliefs and values and includes being reflective and aware of your feelings, biases, experiences, and lack of knowledge. The healthcare provider must be mindful of their cultural background and the cultural background of their patients. According to Ball, Dains, Flynn, Solomon, and Stewart (2015), culturally competent healthcare providers can adapt to patients’ unique needs from different cultural backgrounds, building a trusting relationship between providers and patients. For this purpose of this discussion, the first case study’s cultural factors will be discussed, and sensitive issues and appropriate targeted questions to ask this patient. Factors

There are concerns with the overall healthcare of these patients when assessing this patient’s cultural factors. The socioeconomic factor related to this patient is that the patient lives with and relies on his daughter, a single mother, for physical and financial support. The patient is 86 years old and is likely unable to contribute to the household financially or physically. It is also unclear if the daughter can support the patient beyond his medical needs. In the Asian culture, it is expected that adult children care for their aging parents; this includes providing financial support. However, younger generations interpret the expectation of caring for the elderly differently, and many have a difficult time financially supporting their aging parents (Yan, Chan, & Tiwari, 2014). According to Vega (2014), 12.3 percent of Asian-Americans over 65 years of age live in poverty, compared to 9.1 percent of all Americans over 65 years of age. The suicide rate for Asians over 75 years of age is almost twice that of others the same age.

    Sensitive Issues

The fact that the patient relies on his daughter for support and the patient’s statement about not wanting to be a burden is concerning, and the topic should be discussed with the patient sensitively. According to Carteret (2010), Asians place a strong emphasis on family, and family loyalty is expected. Therefore, the father may not be open about his home situation and relationship with his daughter out of fear of shame, dishonor, or disgrace to himself or his family. The APRN should be aware of this cultural factor when discussing why the patient feels that he is a burden to his daughter. Additionally, it would be appropriate to speak with the patient about in-home services to help care for him and address any financial concerns that the patient may have regarding his medications and managing his comorbidities.  The APRN would need to be culturally sensitive  not offend the patient due to the strong family beliefs and fear of shaming the family.

    Targeted Questions

When asking targeted questions to the Asian patient, it is crucial that the APRN recognize and understand that particular culture and how they communicate. When conversing with Asian cultures, preserving harmony is as important as how questions are asked and the body language being used (Carteret, 2010). Direct questions may not be the most appropriate, especially when addressing sensitive issues such as the patient’s home life. The following questions would be asked.

Can you tell me about your current health problems?

Do you ever feel sad or depressed?

You mentioned that you don’t want to be a burden to your daughter, can you tell me about your relationship with your daughter?

Do you feel safe at home?

Would you be open to exploring resources to help you at home?

    Conclusion

Healthcare providers must be aware of the different cultural factors that influence patient care. Providers must be open to different cultures and sensitive to the patients’ cultural beliefs and norms. A culturally competent provider can adapt to the patient’s needs, therefore, building a foundation of trust (Ball, Dains, Flynn, Solomon, & Stewart, 2015). When concerns arise when caring for patients from different cultures, the provider should address those concerns in a culturally sensitive way so as not to offend the patient or family and provide the patient with the care they need.

NURS 6512 Diversity and Health Assessments Discussion

Functional Assessments and Cultural and Diversity Awareness in Health Assessment

Xiaodong et al. (2017), in an issue of Healthy China 2030, “the focus should be on disease prevention and encouraging people to adopt healthy lifestyles. Simultaneously, the public health service system and the health services industry should be improved to facilitate early detection, early diagnosis, and early treatment of diseases, as these are of great significance to reduce the mortality rate”(p. 112). The essentials of nursing is having compassion and autonomy. To immediately identify cultural diversity as patient status is of major importance in the development of an efficient risk assessment. Patient care should be holistic and patient-centered regardless of cultural factors, spirituality, ethnicity, demographics, or socioeconomic status. Active-listen, allowing time for responses promotes a trusting environment that sets the mode of building an interactive nurse-patient relationship. Good rapport with the patient will promote comfort to make information easy and forthcoming. Trust allows the nurse to adequately determine the patient’s needs to develop a focused health assessment effectively.

    Cultural Sensitivity

The patient is an at-risk 86-year-old male, physically and financially dependent on his daughter, a single mother with little time for all his health needs. Patient families are significant in implementing the patient’s beliefs and values into their treatment plan. In some cases, the family can make adjustments related to patient beliefs on health promotion to comply with the care regimen, and cultural diversity should always be considered. Nursing has evolved throughout the years, focusing on essential patient care theories and philosophies (Andersson, Willman, Sjostrom-Strand, & Borglin, 2015). Some theories and philosophies focus on understanding and analyzing your cultural competence related to patient and caregiver family cultures. Cultural diversity is about respecting and appreciating an individual’s society, interests, skills, and talents; culture sensitivity is about being aware of one’s cultural differences, according to (Brown, 2020). To establish a professional understanding of cultural diversity/sensitivity, it is important to maintain competences, a set of functional capabilities to effectively as an individual and an organization to efficiently work within the context of the cultural beliefs, behaviors, and needs presented by customers and their communities (CDC, 2015). Another issue to address with this scenario is caregiver strain. According to Menon et al. (2017), “the lack of support system in various countries makes the burden of caregiving to fall on the family members, a single individual in most cases. The sudden nature of the disease leaves the family unprepared to deal with consequences which include psychological and financial burden”(p. 418). Single home caregivers can be stressful for the individual as well as the patient. Patient concerns with overwhelming ad becoming a burden on a loved one can cause underlying issues of mental strain and distress in the home that can alter patient outcomes. Target Question

Are there any changes or symptoms related to your health?

What are your concerns about becoming a burden to your daughter?

Do you feel safe in your home environment?

Who all lives in the home?

Are there any concerns about your health needs being met?
