Autism CARES Act of 2019 impacts on nursing essay

    Assignment 1

Select a House of Representatives or Senate bill (national or state level), either currently in process or one that has been passed within the last year that impacts mental health care operations, advanced practice nursing, or mental health care financing.

The bill has been identified as H.R. 1058 (116
    th
): Autism Collaboration, Accountability, Research, Education, and Support Act of 2019. It is cited as Autism CARES Act of 2019. The legislation reauthorized some provisions of the Public Health Service Act (PHS Act) with the intention of improving autism management. It is a federal legislation that specifically focuses on amending the PHS Act to enhance the activities of the National Institution of Health with regards to research and management of autism spectrum disorders (ASD). In addition, the legislation reauthorizes the provisions of PHS Act that are concerned with education, early detection and intervention targeted at ASD. Besides that, the legislation mandates the Interagency Autism Coordinating Committee (IACC) to continue with its activities. The legislation offers three other advantages not included in the prior version of PHS Act. The first advantage is that the changes target all persons with ASD rather than targeting only the youth, an inherent shortcoming in the previous framing of PHS Act framing Autism CARES Act of 2019 impacts on nursing essay. The second advantage is that it focuses funding on shortages identified in personal health services. The third advantage is that it reduces health-outcome disparities across diverse populations (GovTrack, 2020).

The Autism CARES Act of 2019 was introduced into the 116
    th
    congress on 7
    th
February 2019 and sponsored by Rep. Christopher “Chris” Smith of New Jersey’s 4th congressional district. It was then referred to the Health Committee that held hearings on 25

    June 2019 and 11
    th
June 2019. The legislation was ordered reported by the committee on 17
    th
July 2019 with the recommendation that the full chamber consider the bill further. The house committee on energy and commerce reported on the bill on 23
    rd
July 2019, and offered helpful exclamatory background information on the bill’s intentions and the issues it addressed. The Congress voted to pass the bill on 24
    th
July 2014. The Senate voted to pass the bill on 19
    th
September 2019. The bill was signed into law by the president on 30
    th
    September 2019 (GovTrack, 2020).

Its legislative history dates back to 2006 when President George W. Bush signed it into law as Combating Autism Act (P.L. 109-416). The intention of the law was to offer coordinated federal response to the dramatic increase in persons diagnosed with autism spectrum disorder in the country. It increased public investment to address the growing needs of persons diagnosed with autism spectrum of disorders, while authorizing the IACC to coordinate federal efforts targeted at the disorder. The legislation was reauthorized in 2014 as the Autism CARES Act (P.L. 113-157). It had to be reauthorized by September 2019 for the Federal activities conducted under the legislation to continue (Autism Society, 2020).

Discuss your support or opposition of the Bill based on policy, legal, and ethical considerations, and the Christian worldview.

The Autism CARES Act of 2019 is a good legislation that addresses legitimate policy concerns that improve autism management at the federal level in the USA. The legislation offers three advantages from the policy perspective. Firstly, it reauthorizes Combating Autism Act of 2006 thus ensuring that the gains made over the years are continued and not lost. Secondly, it expands the provisions presented in previous legislation so that federal intervention has greater impact Autism CARES Act of 2019 impacts on nursing essay. Thirdly, it ensures that there is federal support for research, prevalence tracking, services and other government activities targeted at autism. Fourthly, it increases the annual federal budget for autism management efforts with $1.8 billion allocated to fund efforts through 2024. The funding is intended to primarily support research grants that advance scientific understanding of ASD, and develop treatments for medical conditions associated with ASD. Also, the funding helps address the needs of persons affected by ASD. Fifthly, it expands the focus of federal-based intervention efforts by ensuring that support is provided for persons with ASD while responding on their well-being and health throughout their entire lifespan. Finally, it reauthorizes and expands the IACC (GovTrack, 2020) Autism CARES Act of 2019 impacts on nursing essay.

In addition to having policy implications, the legislation has ethical implications. It acknowledges that autism is a lifelong condition. The previous legislation targeted the youth so that other age groups were left with unmet needs. The new legislation support the existing activities, and expands them so that health disparities are reduced even as services are provided throughout the lifespan (GovTrack, 2020). The health disparities exist along lines of demographic groups and they are ethically wrong because they exemplify injustices. Through focusing on the youth, the previous legislation presented ethical concerns as other age groups were ignored although they had needs. Utilitarian and Kantian ethics all offer theoretical reasons for considering health disparities as ethically wrong, as do many other ethical principles. The declaration of human rights further supports (ethically) efforts to address health disparities. Every American with ASD has the right to reasonable equal access to health care, a right that was violated by the previous legislation that only targeted the youth. The new legislation condemns unjust practices while promoting just practices, and avoids doing harm to others through ensuring that every person with ASD has an equal opportunity to receive care. Besides that, it is ethically problematic that there were persons with ASD who bore the societal burdens of the medical but who did not receive the same basic benefits that youths with ASD received (Sang & Dearholt, 2018). Overall, the new legislation ensures that all persons with ASD experience reasonably equivalent health outcomes thus addressing the ethical concern of health disparity presented in previous legislation.

Also, the legislation has legal implications. The previous legislation were due to expire with the implication that the activities they supported would halt unless new legislation was presented. Presenting the new legislation guarantees that federal autism management programs and activities have the legal foundation to continue (Pokorski, 2018).

The Autism CARES Act of 2019 is aligned with the Christian worldview that focuses on stewardship and compassion. Persons with ASD cannot take care of themselves, and it is only right that the government intervenes in facilitating the management of their medical condition. This is affirmed in the Bible when Jesus instructs his disciples and followers to take care of others as his substitutes (Mathew 25:40). Given that the federal government has extensive public resources and is in office with the primary objective of addressing public concerns, then it is not hard to accept that they are stewards. As stewards, the federal government can use its powerful means to ensure that persons with ASD to get care. Overall, the legislation ensures that the federal government serves the needs of its citizens rather than being focused profit, thus matching the Christian worldview that advocates for compassion and stewardship (Shellenbarger, 2019). Discuss how the bill impacts, or will impact, the role of the PMHNP. Discuss the impact on health care quality and outcomes.

The Autism CARES Act of 2019 impacts resource allocation to support the delivery of nursing care. It highlights the role of PMHNP to influence policy through their responsibility and capacity to influence health care delivery systems. The PMHNP practice is based on the science of caring and science of human health. It operates from a perspective that values every member of the society equally and holistically while seeking to foster and advance public health across all society levels and throughout the lifespan. To achieve these objectives, PMHNP understand that legislation must define and integrate the appropriate standards for care delivery while addressing the necessary condition for delivering care. Through policy work, nurses can identify and present the shortcomings of the legislation, and influence the appropriate changes to assure high care quality with regards to nursing processes and standards. In essence, the legislation highlights the role of PMHNP as policy advocates who can influence policy while helping to shape care. Similarly, the American Nurses Association and National League for Nursing highlight the role of PMHNP as policy advocates by noting that it is reasonable for PMHNP to identify problems with the policies affecting their practice and galvanize other stakeholders to make the appropriate changes (Christensen & Simmons, 2019). In addition, the legislation identifies the role of PMHNP in conducting the activities identified in the PMHNP. Through the legislation, there is an expectation that PMHNP will engage in ASD research activities, manage surveillance activities, and facilitate care delivery with regards to public education, early detection and intervention (Christensen & Simmons, 2019).

The legislation has a profound impact on health care quality and outcomes. It is the primary source of federal funding for autism monitoring, training, services and research. It requires and supports research and programs to benefit persons with ASD. It manages a center of excellence in autism surveillance and epidemiology. It supports the IACC and its plan. Besides that, it development treatments for ASD. Also, it trains medical personnel to detect and diagnose ASD. Additionally, it monitors ASD prevalence (GovTrack, 2020). ASD is a present concern since management efforts are hindered by a dearth of information that would be best addressed through research. The legislation ensures that significant advances would be reported in three areas. Firstly, research efforts are well coordinated through supporting research funded by the National Institute of Health. Secondly, data and surveillance is streamlined through building infrastructure at the CDC to monitor ASD. Finally, training efforts are supported to address the reported shortages of personnel who facilitate screening, diagnosis and treatment efforts (Autism Society, 2020). As a result, the legislation improves health care quality and outcomes. Autism CARES Act of 2019 impacts on nursing essay

    NURSE MANAGER BUDGET ANALYSIS

Analysis of the Nursing Unit Budget by the Nurse Manager and the Formulation of a Future Plan of action Based on the Same

This budget presentation shows the budget allocation for the 20-bed nursing unit for the month of June in 2013 as USD 285,000. However, the cruel expenses for the unit in the same month came to USD 309,000. This gives a variance or shortfall of USD 24,000 for the month of June 2013. In the same month, the projected patient days were 360. However, the actual patient days came to a total of 347 for the month (less by 13 patient days). A patient day refers to the total number of patients in the unit’s beds by midnight (Waxman, 2015). There was also an increase in both the projected productive and non-productive (compared to the actual) by USD 20,000 and USD 3,000. The total personnel cost was therefore higher in the actual expenses of the unit by USD 23,000. The overheads and supplies expenses in the same month of June were also higher than projected by USD 1,000. In all, the total increment in expenses for the unit came to USD 24,000 higher than allocated for the month of June 2013.

Productive time is time actually worked by the nurses and paid for. Non-productive time is time that is paid for by the organization while the nurse is not working, such as during sick time, holidays, and paid time off (Gealan, 2013; Brown et al., n.d.). Looking at the budget comparison between 2012 and 2013, it is clear that the patient days increased by 76 days from 3,164 to 3,240. The costs or expenses (personnel remuneration, overheads, and items) also rose, marginally, by USD 3,000 from USD 1,770,000 in 2012 to 1,773,000 in 2013.

The funding allocated to every nursing department is that department’s budget. There should therefore be an appropriate mix of nurses to match the actual patient care hours for quality and effective nursing care (Gealan, 2013; Brown et al., n.d.). In this unit, the future plan of action will be to properly titrate the personnel costs to the patient days every month to avoid having non-essential personnel on duty when the patient days are low. This will be achieved by properly making use of full-time equivalents or FTEs month by month. A full time nurse is taken to work for 8 hours a day and 5 days a week. This gives a total of 2,080 hours of personnel time a year (Brown et al., n.d.). If necessary, locum employees can also be used to reduce costs as they will only work when needed. Their cost will also be based on the FTE model. Lastly, there will also be need to increase efficiency in the use of overheads like electricity and supplies like syringes and catheters by preventing wastage.

A nurse manager is responsible for overseeing monthly budget amounts for the unit, a duty that not only entails personnel but often also includes evaluating equipment and supply needs and expenditures. The manager may test new products and/or oversee demonstrations. A nurse manager will report to budget meetings with other department managers to share information, exchange ideas and engage in problem-solving for the health care facility.

This week you are being supplied a budget report showing projected expenses and actual expenses for the past two years. You are asked to present the report during this months budget meeting. After reviewing the budget report, write a description of what you would report at the meeting to explain the report and your plan of action for the following year. Translational research involves gathering knowledge from various clinical experiments and using it to address the critical medical needs of a particular population. Translational research aims at improving the health outcomes of the target population. Consequently, this type of research has become very popular in the field of medicine. The study conducted by Hardy et al (2015) on childhood obesity utilizes T2 transitional research, which focuses from clinical practices to the population. This type of research aims at using clinical knowledge to address health issues facing the target population.

Transitional research is best for this study. Particularly, this type of research illustrates how community-based obesity treatment programs play a significant role in addressing the issue of childhood obesity. It indicates how healthcare practitioners use community-based obesity treatment programs in managing the weight and behaviors of obese children.

Additionally, transitional research is the most appropriate approach to childhood obesity. Research shows that one in every four children aged between 5 and 14 years is struggling with obesity (Hardy et al, 2015). This number represents approximately 716,000 children. The high number makes it relatively difficult for the pediatric to meet their treatment needs. Additionally, brief counselling on healthy feeding habits and screening services offered by primary care team does not assist them to lower their weight. Furthermore, obesity among children is a key public health challenge (Arteaga et al, 2018). About 18.5% of children aged between 2 and 19 years are overweigh (Arteaga et al, 2018). The high weigh exposes them to weigh-related ailments. The drastic increase in childhood obesity in the country necessitates for transitional research, thus enabling healthcare practitioners to come up with more effective clinical practices for addressing this problem. NUR 550 Translational Research Nursing Essay example

Weekly discussion question for NUR 550- In your own words, define
    translational research
and how it connects to your role, either individually or in a collective practice. Describe how you might use it in your current or future setting (currently-operating room manager, future setting-Masters in nursing with emphasis in education)

    Translational Research

Translational research refers to the application of knowledge from the basic science, research findings and clinical trials to address the medical needs and improve health outcomes. Translational research promotes multidirectional incorporation of basic research, population-oriented research, and patient-focused research to improve public health (Curtis et al., 2017). In addition, translational research integrates the needs of the general public and the community members are actively engaged to identify their health needs that need to be addressed through research (Titler, 2018). Using translational research, the best available medical and health practices to address the medical needs are then implemented according to the research findings.

As an operating room manager, translational research comes in handy, especially when implementing the best-practices and care delivery in the operating room. The patients in the operating room have different medical needs and therefore as the manager, I always ensure that the best-practices adapted for the care of these patients are informed by the research findings. Therefore, in my current setting translational research can be useful in addressing the medical needs of the surgical patients and ensuring that the patients are actively engaged and involved throughout the research process. NUR 550 Translational Research Nursing Essay example

In my future role as a nurse educator, I plan to use translational research to develop knowledge base valuable in preparing competent nurses capable of promoting and maintain wellness and care for patients using transformative transitions. Moreover, I plan to use transformational research to guide the designing of courses and programs in nursing education and also develop the best learning and teaching practices.

    Credit Hours

    3 credits in 11 weeks

Walden University assigns credit hours based on the number and type of assignments that enable students to achieve the course learning objectives. In general, each semester credit equals about 42 hours of total student work and each quarter credit equals about 28 hours of total student work. This time requirement represents an approximate average for undergraduate work and the minimum expectations for graduate work. The number and kind of activities estimated to fulfill time requirements will vary by degree level and student learning style, and by student familiarity with the delivery method and course content.

    Course Description

    Primary and Secondary Sources

Review the following information prior to selecting resources for assignments.

    Primary:
A primary source is an original document that is the first account of what happened. A research report is primary, and you can tell because it includes materials and methods demonstrating how the research was done. Some creative work is also primary, such as poetry, novels, and interviews of people who experienced something firsthand. In nursing, which is an evidence-based discipline, we strive to use primary research that is published in scholarly, peer-reviewed journals.

    Scholarly, peer-reviewed journal:
Scholarly journals publish papers by professional authors and experts in the field using a peer-review process to review the work and assure quality before publishing. The focus of a scholarly journal is to provide accurate information for scholars and other researchers. The focus is on content rather than advertising, a direct contrast to popular media. Scholarly journals publish both primary and secondary papers, the former usually noted as original research and the latter as reviews and commentaries. Letters to the editor may also be published but should be recognized as opinion pieces.

    Note:
When selecting articles for course assignments, you are advised (unless you are referencing seminal information) to focus on work published within the past five years.

    Secondary:
A secondary source is one step removed from the original source. This work interprets and often compiles other work, and it includes review articles, textbooks, fact sheets, and commentaries about a topic. It also includes news reports of original research. Secondary work is more prone to error and bias than primary work because it is being filtered through an additional person or persons. Review papers can be useful to glean information about a topic and to find other sources from the reference list, but it is the original, primary research that should be relied on most heavily in demonstrating scholarship, depth, and validation of factual information.

    Course Assignments

    Participation in weekly Discussions:
The exchange of ideas among colleagues engaged in scholarly inquiry is a key aspect of learning and is a requisite activity in this course. You are expected to participate each week by posting a response to a prompt or question in the weekly Discussion area. In addition, you are expected to respond to your fellow students’ postings. To count as participation, responses need to be thoughtful; that is, they must refer to the week’s readings, relevant issues in the news, information obtained from other sources, and/or ideas expressed in the postings of other class members. You may ask questions or offer further information or links about the subject. Please pay attention to grammar and spelling, as consistently poorly written posts will receive grade penalties. In grading the required Discussion postings, your Instructor will be using the Discussion Posting and Response Rubric, located in the
    Course Information
     area.

    Note:
Unless otherwise noted, initial postings to Discussions are due on or before Day 3, and response postings are due on or before Day 6. You are required to participate in the Discussion on at least three different days (a different day for main post and each response). It is important to adhere to the weekly time frame to allow others ample time to respond to your posting. In addition, you are expected to respond to questions directed toward your own initial posting in a timely manner.

    Assignments:
The Assignments provide you with the opportunity to apply the skills and knowledge gained through the Learning Resources and the practicum experience. See the Assignment area of specific weeks for detailed descriptions of the assignments. In grading the required Assignments, your Instructor will be using rubrics located in the Course Information area.

    Note:
The course Assignments will require that you completely and accurately demonstrate critical thinking via assimilation and synthesis of ideas when using credible, outside and course specific resources (i.e. video, required readings, textbook), when comparing different points of view, highlighting similarities, differences, and connections, and/or when lending support to your Assignment responses.

    Portfolio Assignment:
Each course in the Master of Science in Nursing (MSN) program for the following specializations includes a Portfolio Assignment: Nursing Education, Nurse Executive, Nursing Informatics, and Public Health Nursing. The Portfolio Assignment is designed to measure specific professional knowledge and skills as outlined in the American Association of Colleges of Nursing (AACN) Master’s Essentials. Students submit the Portfolio Assignment in the online classroom and a criterion-based scoring rubric is used to grade the assignment. The rubric is aligned with American Association of Colleges of Nursing (AACN) Master’s Essentials and provides specific and informative feedback on your performance. The Portfolio Assignment is evaluated by the course Instructor.

Grading Criteria and Total Components of a Grade

Course grades will be based on participation (postings) and completion of assignments listed below.

Letter grades will be assigned as follows:

    90%–100% = A

    80%–89% = B

    70%–79% = C

< 70% = F Please see below for the policy on Incomplete (I) grades. Assignment Total Points Percentage (Weighted) Discussions Participation in Discussions* (5 @ 100 pts. each) 500 25% Quizzes (2 @ 100 pts. Each) 200 10% Assignments Module 1 Assignment (100 pts.) Module 2 Assignment (100 pts.) Module 3 Assignment (100 pts.) Module 4 Assignment (100 pts.) Module 5 Assignment (100 pts.) Module 6 Assignment (100 pts.)** 600 65% Total 1300 100% * Each Discussion requires that you make one initial posting and at least two response postings to colleagues. See the Discussion Posting and Response Rubric for posting details. **This is the Portfolio Assignment for this course. Incomplete Grade Policy Per University policy, Incomplete grades can be granted only to students who have already met the minimum criteria for active weekly participation in a course (including weekly postings in online courses) and have completed at least 80% of other coursework. Incompletes can be awarded when, because of extenuating circumstances, a student has not met additional course requirements, including but not limited to written assignments, group projects, and research papers, as applicable. All Incomplete grades are awarded at the discretion of the course faculty. (Reproduced from Student Catalog) Students who are eligible for an Incomplete must contact the Course Faculty to request the grade as soon as possible. Students who do not meet the criteria listed above will not be allowed to earn an Incomplete. If the Incomplete is approved, the Faculty Member will work with the student to outline the due date(s) for remaining work. Under no circumstances will the new due dates extend beyond 50 days from the last day of the term. Faculty will then have 10 days to assess the work and post the permanent grade before the University-allotted Incomplete time limit of 60 days expires. All Incomplete grades not resolved within the time allotted will convert to permanent grades of F. Instructor Feedback Schedule The Instructor will log in to the course during the week to monitor the weekly Discussion area. Feedback will be provided via the My Grades area, the Discussion area, and/or the Announcements page. Instructor feedback and explanation is provided whenever full credit is not achieved. Depending on the nature of the feedback, Instructor responses may be posted to the Discussion area or included in the My Grades area. The goal of your Instructor is to act as a discussion and learning facilitator rather than a lecturer. The Instructor will not respond to every posting by every individual, so please feel free to ask your Instructor if you would like some personal feedback on a particular assignment posting or any time you have any questions regarding your assignments or your grade. For most assignments, you can expect your grades and/or feedback to be posted within five calendar days after the due date. Some assignments may require more than five days for your instructor to provide you with quality feedback. Course Procedures All class Discussions take place in the weekly Discussion areas. You are encouraged to post course-related questions to the Contact the Instructor area as they may be of interest to all; however, if your question is urgent, it is often best to e-mail the Instructor. If your e-mailed question is thought to be of benefit to all, it may be responded to by the Instructor via e-mail to all or posted as an announcement. Instructor feedback on content and writing issues that is thought to be of benefit to the entire class may be posted to the Contact the Instructor area; however, most personal critique will be done privately in the Grade Center. Be sure to check the Grade Center for comments every week even if you received full credit. Please feel free to use the Class Café to initiate and participate in conversations not directly related to the course. This is an excellent opportunity to get to know other students better. The Instructor will browse the Class Café occasionally but generally will not respond to conversations posted there unless students have specific questions for him or her. Check the e-mail account you use for official Walden University business on a regular basis. The expectation is that you are checking this e-mail account daily during the week. If you experience difficulty sending or receiving Walden e-mail, please contact the Student Support Team right away. Contact information for the Student Support Team is located in the Student Support area. Review all materials in the Course Information area, as well as the materials contained under each of the weekly buttons. Note:  There are Optional Readings located within the Learning Resources section of each week in the course. You are encouraged to explore these readings, as needed, in order to enhance your understanding of the course content. Preferred Methods for Delivering Assignments Be sure that you post to the correct Discussion area each week. Do not e-mail postings to the Instructor. For all initial Discussion postings, make sure that the first sentence of your posting reads Main Question Post. For your responses to others’ response postings, make sure that the first sentence of your response reads Response. These actions will ensure easily identifiable subject lines for your postings and responses. Application Assignments are submitted to the SafeAssign link and named according to the week in which the Assignment is submitted. Directions for naming each Application Assignment are included in each week’s Assignment area. Please be sure that all written Application Assignments are saved and submitted as a “.doc” file. All e-mail correspondence must contain in the subject line “NURS 6003-XX-NAME” (XX is the section number) followed by a brief description of the subject. This subject line convention ensures that your e-mail will be easily identified and responded to in a timely manner. It is required that the e-mail contain a signature that matches the official name used in the course. Late Assignment Policy Students are expected to submit assignments by the due dates noted in the course. In extenuating circumstances, such as illness, the student must contact the Instructor as soon as possible to discuss the situation. In those circumstances, faculty will determine the appropriate course of action for the student. Depending on the situation, these actions may include recommendations to drop the course (if within the University drop/withdrawal period), acceptance of some or all of the overdue assignments with or without penalties, or failure to accept assignments. Assignments submitted late without prior agreement of the Instructor, outside of an emergency absence, or in violation of agreements for late submission, will receive a grade reduction for the assignment amounting up to 20%. After 5 days, the assignment will not be graded. Students should be aware that late assignments may not receive the same level of written feedback as do assignments submitted on time. Keeping Your Coursework You will have access to the course and your coursework from the course start date until 60 days after the course ends. After this time, you will no longer be able to access the course or related materials. For this reason, we strongly recommend that you retain copies of your completed assignments and any documents you wish to keep. The University is not responsible for lost or missing coursework. Course Evaluation At or near the end of the course, you will receive an e-mail inviting you to submit an online evaluation of the course and instruction. All submitted course evaluations are confidential, and only aggregate data and comments will be shared with the Instructor and Program Director. Your feedback is vitally important to Walden University in its efforts to continuously improve programs. Students With Disabilities Students in this course who have a disability that might prevent them from fully demonstrating their abilities should contact the director of disability services at  disability@waldenu.edu  or at 1-800-925-3368, ext. 312-1205 and +1-612-925-3368 or  https://www.waldenu.edu/contact  for international toll-free numbers as soon as possible to initiate disability verification and discuss accommodations that may be necessary to ensure full participation in the successful completion of course requirements. Classroom Participation In accordance with U.S. Department of Education guidance regarding class participation, Walden University requires that all students submit at least one of their required Week 1 assignments (which includes posting to the Discussion Board) within each course(s) during the first 7 calendar days of class. For courses with two-week units, posting to the Discussion Board by Day 7 meets this requirement. The first calendar day of class is the official start date of the course as posted on your myWalden academic page. Assignments submitted prior to the official start date  will not  count toward your participation. Financial Aid cannot be released without class participation as defined above. Students who  are taking their first class with Walden  and do not submit at least one of their required Week 1 assignments (or at least one Discussion post) by the end of the 7th day will be administratively withdrawn from the university. Students who have already taken and successfully completed  at least one  or more class(es) with Walden, and who  do not participate  within the first 7 days, will be dropped from that class. If you have any questions about your assignments, or you are unable to complete your assignments, please contact your Faculty Member. Checklist The weekly course checklist below outlines the assignments due for the course. For full assignment details and directions, refer to each week of the course. All assignments are due by 11:59 p.m. Mountain Time (MT) on the day assigned (which is 1:59 a.m. Eastern Time (ET) the next day). Shadow Health Assessment Assignment

You will complete all assessment assignments using the
    Shadow Health
      virtual reality simulation platform.

    Introduction and Pre-brief

Two days after a minor, low-speed car accident in which Tina Jones was a passenger, she noticed daily bilateral headaches along with neck stiffness. She reports that it hurts to move her neck, and she believes her neck might be swollen. She did not lose consciousness in the accident and denies changes in level of consciousness since that time. She states that she gets a headache every day that lasts approximately 1-2 hours. She occasionally takes 650 mg of over the counter Tylenol with relief of the pain. This case study will allow you the opportunity to examine the patient’s optic nerve via use of the ophthalmoscope as well as assess her visual acuity. You will need to document your findings using appropriate medical terminology. Careful assessment of documentation of EACH cranial nerve is integral to performing a comprehensive neurological assessment. Be sure to assess for foot neuropathy using the monofilament test. NR509 Shadow Health SOAP Note Template

    Tips and Tricks

By now you are very familiar and comfortable with navigating the Shadow Health virtual learning environment. The simulated patients are similar to actual patients and can respond to over 70,000 initial and follow-up questions. Your patients will never get frustrated when you ask multiple questions and they will never get embarrassed or withhold information if you address sensitive subjects, like sexual activity.

Be sure to practice asking interview questions in Shadow Health using the talk-to-text feature and the Google Chrome browser. This will assist with reducing the time commitment for each assignment and enhance the fidelity of your patient-provider experience.

When writing up your physical examination findings, it is insufficient to simply document that the cranial nerve assessed was “intact” or “normal”. What does this mean? Document exactly what you assessed and the findings. Documentation of pertinent negative findings, which denote what you expect to find during the examination and not an abnormality, are just as important as pertinent positive, or abnormal, findings.

    Purposes

The purposes of the Shadow Health Physical Assessment Assignments are to: (a) increase knowledge and understanding of advanced practice physical assessment skills and techniques, (b) conduct focused and comprehensive histories and physical assessments for various patient populations, (c) adapt or modify your physical assessment skills and techniques to suit the individual needs of the patient, (d) apply assessment skills and techniques to gather subjective and objective data, (e) differentiate normal from abnormal physical examination findings, (f) summarize, organize, and appropriately document findings using correct professional terminology, (g) practice developing primary and differential diagnoses, (h) practice creating treatment plans which include diagnostics, medication, education, consultation/referral, and follow-up planning; and (i) analyze and reflect on own performance to gain insight and foster knowledge NR509 Shadow Health SOAP Note Template.

    Due Date

    Sunday 11:59 PM MT
     at the end of
    each respective week.

Students are expected to submit assignments by the time they are due. Assignments submitted after the due date and time will receive a deduction of 10% of the total points possible for that assignment for each day the assignment is late. Assignments will be accepted, with penalty as described, up to a maximum of three days late, after which point a zero will be recorded for the assignment.

In the event of an emergency that prevents timely submission of an assignment, students may petition their instructor for a waiver of the late submission grade reduction. The instructor will review the student’s rationale for the request and make a determination based on the merits of the student’s appeal. Consideration of the student’s total course performance to date will be a contributing factor in the determination. Students should continue to attend class, actively participate, and complete other assignments while the appeal is pending.

    Total Points Possible: 75 Points

    Assignment

Step One: Complete the designated Shadow Health (SH) Assignment on the SH platform.

Step Two: Document your findings on the
    Fillable Soap Note Template
     or the
    Printable Soap Note
    .

Step Three:  Upload the Lab Pass and completed SOAP Note as separate documents to the same assignment tab in the gradebook.

    Requirements

NOTE: Before initiating any activity in Shadow Health, complete the required course weekly
    readings and lessons
     as well as review the
    introduction and pre-brief.

Complete the Shadow Health Concept Lab (Weeks 2, 4, and 5) prior to beginning the graded assignment.

Gather subjective and objective data by completing a focused, detailed health history and physical examination for each physical assessment assignment.

Critically appraise the findings as normal or abnormal. NR509 Shadow Health SOAP Note Template

Complete the post activity assessment questions for each assignment .

Complete all reflection questions following each physical assessment assignment.

Digital Clinical Experience (DCE) scores do not round up. For example, a DCE score of 92.99 is a 92, not a 93.

You have a maximum of two (2) attempts per Shadow Health assignment to improve your performance. However, you may elect not to repeat any assignment. NOTE: If you repeat an attempt, ONLY the second attempt will be graded, regardless of the DCE score. Please refer to the grading rubric categories for details.

Download the Lab Pass for the final attempt on the assignment.

    On the Canvas Platform:

Summarize, organize, and appropriately document findings using correct professional terminology on the SOAP Note Template.

Identify three (3) differential diagnoses and provide ICD-10 codes and pertinent positive and negative findings for each diagnosis.

Create a comprehensive treatment plan for each assignment. Must address the following components: Diagnostics, Medication, Education, Referral/Consultation, and Follow-up planning. If no interventions for one or more component, document “none at this time” but do not skip over the component.

Provide rationales and citations for diagnoses and interventions. NR509 Shadow Health SOAP Note Template

Include at least one scholarly source to support diagnoses and treatment interventions with rationales and references on the SOAP note. Only scholarly sources are acceptable for citation and reference in this course. These include peer-reviewed publications, government reports, or sources written by a professional or scholar in the field. The textbooks and lessons are NOT considered to be outside scholarly sources. For the threaded discussions and reflection posts, reputable internet sources such as websites by government agencies (URL ends in .gov) and respected organizations (often ends in .org) can be counted as scholarly sources. The best outside scholarly source to use is a peer-reviewed nursing journal.  You are encouraged to use the Chamberlain library and search one of the available databases for a peer-reviewed journal article.  The following sources should not be used: Wikipedia, Wikis, or blogs.  These websites are not considered scholarly as anyone can add to these. Please be aware that .com websites can vary in scholarship and quality.  For example, the American Heart Association is a .com site with scholarship and quality. Each student is responsible for determining the scholarship and quality of any .com site.  Ask your instructor before using any site if you are unsure. Points will be deducted from the rubric if the site does not demonstrate scholarship or quality. Current outside scholarly sources must be published with the last 5 years.  Instructor permission must be obtained BEFORE the assignment is due if using a source that is older than 5 years.

For this assignment you will explore, identify, and critically analyze, a theory that could be used as a framework for directing the future actions you identified in the
    Ways of Knowing
assignment.  In addition, you will professionally reflect on your critical analysis and describe how exposure to a theory may lead an individual to think differently about their own experience, professional role, and future directions of their work.
Note:  The content of this assignment could serve as draft for a discussion related to ”Theory” in Chapter 3 of the final capstone document.

    Assignment Guidelines:

To begin this assignment you will first select a theory.  The theory can be either borrowed or from the discipline of nursing.  After you select the theory, you will address each of the items below in a written scholarly paper.  The items reflect the processes involved in forming a complete description of a theory as discussed in Chapter 8 of Chinn and Kramer.

Begin this assignment with an introduction. The introduction should include a discussion that provides:

A brief succinct overview of the professional experience and future actions discussed in the
    Ways of Knowing

A statement that explains the purpose of the assignment (e.g., The purpose of this assignment is to……).

After your introduction, discuss the following content using appropriate APA level headings:

Description of the Theory – Provide the name of the theory. Identify if the theory is borrowed, or if from the discipline of nursing.  Then, identify the author of the theory, and the date the theory was established. Utilize primary rather than secondary resources. NUR 700 Theory Selection and Critical Analysis

Purpose of the Theory – Provide a descriptive summary of the purpose of the theory. In your summary specifically speak to the context and situations in which the theory is useful.  In your summary clearly identify how the purpose of this theory is linked to the future actions you identified in the
    Ways of Knowing

Concepts of the Theory – Provide a descriptive summary of the concepts (words or groups of words that represent objects, properties, and / or events) within the theory. In your summary clearly identify how the concepts are linked to the future actions you identified in the
    Ways of Knowing

Definitions in the Theory – Provide a clear definition for each concept that was identified. The definitions should be presented in conceptual terms within the context of the theory.

Relationships in the Theory – Provide a descriptive summary of the linkages among and between the concepts associated with the theory.

Structure of the Theory – Provide a clear description of the structure of the theory (choose from one of the examples on pg 195 of Chinn and Kramer). In your description clearly identify how you arrived at your conclusion.

Assumptions in the Theory – Provide a clear description of the assumptions associated with the theory. In your description clearly identify if the assumptions are factual assumptions or value assumptions.  Also in your description clearly speak to how the exposure to differing assumptions may open new perceptions with regards to going “outside of the box” of common practice and procedures.

Application to Professional Practice – Provide a descriptive summary your critical analysis findings. In your summary thoroughly discuss the application of the theory in terms of providing guidance for the future actions identified in the
    Ways of Knowing

End your paper with a conclusion. The conclusion should present a brief summary of the key takeaway points of the paper.  The conclusion should not present any new information. NUR 700 Theory Selection and Critical Analysis

    Introduction

During my shift, I was debriefed by the RN of a patient was refusing vital signs, was mute upon presentation and the caregiver was available to give collateral information even though visibly anxious, almost in tears while describing patient’s behavior in the past month characterized with refusal of medication for blood pressure and psychotropic drugs. The patient is paranoid, exhibits bizarre behaviors and bad eating habits coupled with stopping to talk. Patient has a diagnosis of paranoid schizophrenia, which is a serious mental illness (SMI).  Paranoid schizophrenia is the most common form of schizophrenia, a type of brain disorder that can make it difficult for you to tell the difference between reality and fantasy. Prior to approaching the patient, I had reviewed her chart.  I noted she had her last refills in September of the previous year and had not followed up at the clinic for several months. The

In mental healthcare, providers experience a challenge when people with mental health conditions suddenly stop taking their medication, or to altogether avoid seeking a prescription. This choice can be profoundly frustrating for loved ones and clinicians alike, particularly when the disorder is one that is often easily treated with the right medication (Garcia-Cabrera, I., Diaz-Caneja, C.M., Ovejero, M, & de-Portugal, E, 2018). While the decision to avoid medication isn’t always a wise one, understanding the motives behind patients’ decisions to stop taking medication can help clinicians, community organizations, friends, and family work together to encourage healthy choices without undermining the autonomy of people struggling with mental illness (Garcia-Cabrera et al., 2018). As such, this paper seeks to select a theory related to the following case scenario and present a critical analysis of the same.

    Lewin’s Change Theory

The Lewin’s change theory was developed using unfreezing, changing and refreezing steps (Kurt, 1947). This theory was developed in 1940s by Kurt Lewin. Kurt is a German-American psychologist popularly known as the father of psychology for pioneering the social, organizational and applied psychology in the United States. He was born in 1890 in Prussia by a Jewish family on September 9
    th
and died in February 12, 1947 at the age of 56. He studied medicine from University of Freiburg and later biology from University of Munich and later in his career, appointed as a Psychological Institute of the University of Berlin in 1921. His change theory is a nursing theory that has since then become the most influential in management. It was developed using three steps which include the unfreezing, changing and refreezing and sought to bring an understanding of the change process in simplified form (Kurt, 1947).

    Purpose

The purpose of the Lewin Change theory in mental health is to initiate organizational change. As such it is essential in developing and implementing interventions that promote the drug adherence among the patients (Belcher, DiBlasio, Siegfried & Turnquist, 2017). However, it is also used in team building trajectories as an excellent strategy of bringing about the change of mentality among the employees. This involves improving the provider’s attitude towards the not adhering patient and getting to encourage them on how to approach such patients in a manner that does not lower their self-esteem. It also helps in creating awareness of the advantages of change in an organization or an individual. In this case, the purpose of the theory will be to impose changes towards modifying the patient’s behavior to improve their adherence to his care plan.

    Concepts

The Lewin’s change theory has three concepts. These include the driving forces, restraining forces, and equilibrium. Driving forces are considered to be the factors that push towards the direction of the change. They cause a change to take place. In this case, these forces will push the client towards adherence to medication as desired for them to recover from their mental illness. On the other hand, the restraining forces are factors that counter the driving forces. These forces discourage the patient from adhering to medication and can be the side effects of medication or other personal reasons (Sabin, 2016). These factors shift the equilibrium in the direction that opposes change while the driving forces shift the equilibrium in the direction of the desired change. The equilibrium is a state of balance between the driving forces and the restraining forces whereby no change occurs. However, the patient’s recovery is influenced by these two forces. NUR 700 Theory Selection and Critical Analysis.

    Definitions in the theory

The definitions presented by the Lewin’s theory include behavior which is defined in this model as “a dynamic balance of forces working in opposing directions (Teguh, Hariyati & Muhaeriwati, 2019).” The theory also defines equilibrium as a state of no change due to balanced driving and restraining forces. The change theory is also defined as a three-stage model of change that is known as the “unfreezing-change-refreeze model”

    Relationships in the theory

The Lewin’s theory of change informs multiple relationships. These include the nurse patient relationship whereby, it seeks to develop a trusting link between the two parties in order to facilitate initiation of changes. Another relationship in this model is the one between the nurses and the organization which supports adaptation and successful implementation of change in the clinical setting. Moreover, collaborative relationships among the nurses as the theory develop positive attitudes towards understanding particular concepts through inquiry and building of teamwork (Salinas, Johnson, Conrardy, Adams & Brown, 2019). A relationship between the nurse leaders and the staff members is also evident when change is suggested and during the implementation.

    Structure of the theory

The theory is composed of unfreezing, changing and refreezing components. The unfreeze feature of the theory ensures that the employees are ready for the change. Recognizing the need for change and creation of awareness about the change takes place here. in this case, the RN recognized the need for adherence to the treatment plan and encouraged the replacement of the old behavioral patterns and attitudes with new positive behaviors. This follows change whereby the execution of the intended change is conducted. Finally, the refreeze feature of the model entails ensuring that the change that is implemented becomes permanent.

    Assumptions in the theory

The Lewin’s change theory has various assumptions. These assumptions are related to conflicts and motivational processes in human nature and they create the bases for the three stages of the model NUR 700 Theory Selection and Critical Analysis. They include (Teguh, Hariyati & Muhaeriwati, 2019):

An Individual or group performance is prone to regression unless some measures are taken to institutionalize the improved performance level

There is a tension in person whenever a psychological need or intent exists, and the tension is released when the need or intention is fulfilled.

This tension may be positive or negative, and under conflict situation this is identified as “force field”. Hence the term is known as force field analysis so as to evaluate the tension between positive or facilitating forces and negative or constraining forces the given change plan.

Other assumptions related to conflict include:

Individuals stand mid-way between two positive goals of approximately equal strengths; for e.g. Individual has to choose between two good systems, so which one to buy.

Individuals find themselves between two approximately equal negative goals; for e.g. if an individual has to make a choice between two things which he dislikes, that is a choice of lesser evil.

Individuals are equally exposed to opposing positive and negative forces

    Application to professional practice

Lewin’s change theory has attracted many applications in the current professional practice. It is applied in finding a better understanding of how change impacts the organization. This is because; it seeks to introduce change patterns that seek to improve the outcomes of the patients. It is also applied in determining the barriers or restricting factors to achieving successful implementation of change in the organization and among patients with diverse medical needs and treatment plans (Umotong, 2016). Moreover, the model identifies the forces that drive the positive modification of human behavior as well as the opposing forces towards recovery. This helps in developing strategies that overcome resistance and facilitate collaboration between the nurses and the patients.

    Conclusion

Refusal of drug adherence can result in fatal consequences. However, the application of Lewin change theory in such scenarios can assist in developing an organizational culture of encouraging patients through positive behavioral changes. This theory is essential in recognizing the need, implementing change and ensuring the sustainability of the change through its structural components. NUR 700 Theory Selection and Critical Analysis
