    Week 2: Interprofessional Collaboration Strategies

Question: In today’s changing healthcare environment, it is more important than ever for professional nurses to be skilled in collaborating with professionals from other healthcare disciplines. What communication strategies can professional nurses use to specifically promote collaboration with other healthcare disciplines and professionals?

There are many forms of communication that nurses can promote collaboration with other members of the interdisciplinary healthcare team. The advanced technology we have today has provided us with many different avenues for communication. We can communicate verbally via in person, over the phone, or by using a handheld communication device such as Vocera. We can also communicate in writing via secure email, fax, instant messaging, clear and concise communication notes in a medical record, written report, etc. NR 351 Week 2 discussion: Interprofessional Collaboration Strategies essay examples

According to page 32 of the Massachusetts Department of Higher Education Nursing Initiative, “The Nurse of the Future will interact effectively with patients, families, and colleagues, fostering mutual respect and shared decision making, to enhance patient satisfaction and health outcomes” (Massachusetts Department of Higher Education Nursing Initiative, 2016, p. 32). I agree with this statement. Nurses are innovative! And are always thinking of new and better ways to communicate effectively with patients and members of the healthcare team.

Hood (pg 108) states that patient safety and quality of care depend on effective communication among all members of the healthcare team. Effective communication skills are essential in healthcare regardless of the position you hold within that team. Whether you are the physician, the nurse, an STNA, or in dietary. If we cannot communicate with each other, we cannot collaborate, and thus, we cannot provide the best care for our patients. In my current role, we not only deal with internal members of the healthcare team, we also deal with external team members. Essentially, my team members act as the lead collaborator to assure the health and welfare and the needs of their patients are being met. My team members communicate with the individuals they serve to see what their specific needs are and then collaborate across different functions to assure the health and welfare of the individuals they serve. This involves a multitude of communication strategies as some providers prefer written communication via fax, others are not permitted to email, etc. NR 351 Week 2 discussion: Interprofessional Collaboration Strategies essay examples.  This can be challenging as we have to be able to effectively communicate using many different methods of communication while being mindful of HIPAA. “True collaborative practice involves cooperation and consultation among healthcare providers of different disciplines by working together to achieve goals” (Kearney-Nunnery, 2016).

For team work and collaboration to be effective, nurses have to first of all build a relationship of trust and care with the patients and their families. The three stages of the nurse client relationship are; facilitators, advocate and coordinator. (Hood .2010 ,pg 81). Once the trust is established between the nurse and patient/patient family, it is easy to effectively communicate with them.  According to Ledder and Pepper, nurses rely on communication during nurse-client, collegial, and inter-professional   interactions.(Hood 2010 pg 84). Developing a professional partnership helps foster a relationship based on mutual respect to achieve a common mission while each participant lives out his or her life’s purpose. (Hood .2010, pg 102). Ledder and Pepper also suggests that, to foster a healthy work relationship amongst care members, meaningful conversations needs to occur via the following principles; intention, listening, advocacy, inquiry and silence.(Hood 2010, pg 102) NR 351 Week 2 discussion: Interprofessional Collaboration Strategies essay examples

As part of a nursing team, nurses are responsible for open, professional communication. Regardless of the setting, a professional environment is one in which staff members respect each other’s ideas, share information, and keep one another informed. For example, keeping colleagues informed about patients with emerging problems, physicians who have been called for consultation and unique approaches that may help prevent further problems. Another way to foster a great interdisciplinary collaboration is treating our colleagues with respect, listening to their ideas without interruption, being honest, and direct while communicating.  Part of a good communication is clarifying what others are saying to avoid errors and miscommunication. An efficient team is able to count on each other when needs arises that includes sharing expectations of what, when, and how to communicate is a step toward establishing a strong work team.

Effective communication and collaboration among health professionals is imperative to providing patient centered care (AACN, pg 22). During communication we have to identify patient preferred method of communication, the intellectual ability and any language barrier that may impede communication. Sometimes, client’s inability to speak or understand English language is interpreted as client been confused or non verbal. This is where nurses step in as patient advocate to make other team members aware of client’s communication barriers and ensure plan of care is communicated to client appropriately. NR 351 Week 2 discussion: Interprofessional Collaboration Strategies essay examples. Some of the communication tools we use at my job for patients with language barriers are the two way phone where there is a translator at the other end of the line, hired translators, colleagues who speak the same language, family members (client spokes person/responsible party), road maps (this are printed plan of care which tells client or family what labs were done, what procedure is scheduled etc) and also picture chart. For patients who are not mentally capable of communicating, we have to make sure we keep their decision makers well informed of the plan of care.  According to the nursing of the future, nursing core competencies, one must identify one’s own strengths, limitations, and values in functioning as a member of a team, while respecting the unique professional and cultural attributes that members bring to a team (Massachusetts, slide 31). Team work amongst team members ensures the deliverance of high quality and safe patient care. Being fully informed of clients plan of care help facilitate and ease communication amongst team members.  It is important we listen to each team member attentively, and talk to one another with respect. At my job we do rounds on all patients. The health care team involving the nurse, doctor, respiratory and physical therapist, case manager, social worker, unit manager and charge nurse discusses plan of care for each patient and go to the client as a group to inform the patient about their plan of care for the day. For patients who are not able to communicate, the plan of care is discussed with the client’s spokes person. Bedside shift report is another useful tool used for communicating with both the nurse and client. During bedside shift reporting, a detailed account of client’s history, why they are in the hospital and what their plan of care is at client’s bedside. Areas that could be changed to improve communication and collaboration at my job; having an onsite translator sitting next to client and translating could be very useful rather than the phone with a translator.  Secondly, giving staff lesser patients gives the staff the opportunity to listen to clients concerns and give other team members undivided attention. NR 351 Week 2 discussion: Interprofessional Collaboration Strategies essay examples. We spend more time documenting rather than caring for the patients. Reducing the amount of documentation will allow the nurse time to get to know their patients better

In conclusion, communication and collaboration can have a truly positive impact on both the patient and the health care system. Communication, teamwork and collaboration is an essential part of patient care. When used effectively, collaboration and communication can optimize patient outcomes and improve the quality of care. Thus, nurses and other disciplines must make this a priority. The dynamics between the nurse and other members of the interdisciplinary team is incredibly important in patient care. When this relationship is optimized, it is undoubtedly professional, positive and potentially life changing. Nurse of the Future Nursing Core Competencies Essay Discussions

The Nurse of the Future Nursing Core Competencies consists of ten essential competencies stemming from nursing knowledge.  Chamberlain College of Nursing (2018) lesson 1 explains how they are a framework for professional nursing practice.  I believe those ten were selected since a strong framework leads to better patient outcomes, patient safety and satisfaction.  “In professional nursing, core competencies are fundamental knowledge, abilities, and skills that enable nurses to provide safe, effective care to other persons in health care settings” (Hood, 2018, p.6).  Of those ten, the two most important to my future nursing practice are patient-centered care and communication.

Patient-centered care is extremely important since we could not be nurses without patients to care for.  In the article “The Role of Patient-Centered Care in Nursing,” it explains how patient-centered care “is seen as a positive movement in the future of health care delivery” (Flagg, 2015. P.76).  I wholeheartedly agree.  How could we not center the care of a patient around them?  I always center care around the patient as well as their family that is involved in their hospital stay.  I work with babies so my communication with the family and interdisciplinary team is how care is centered around the patient.  Nurse of the Future Nursing Core Competencies Essay Discussions. I do this by participating in daily rounds, bedside discussions with family and patient/family weekly meetings.  Communication is how we exchange information.  Without it, we would not know anything about our patients.  Good communication is how we get to know the patients.  Getting to know the patient is how we center care around them.  We also need good communication among the interdisciplinary team.  Everyone needs to be on the same page to keep care centered on the patient and the family providing care for that patient.

Patient-centered care and communication go hand in hand in my opinion.  That is why they are the most important to my nursing career.  I do however feel that all ten competencies are vital to my future nursing career.  I am committed to building on my established nursing framework by increasing my knowledge base through Chamberlain University RN-BSN program. Nurse of the Future Nursing Core Competencies (graded) -posted 1/5/16

–Now that you have examined the Nurse of the Future Nursing Core Competencies, why do you think that these competencies were selected? Which 2 of the 10 competencies will be most important to your future nursing practice and why?

At first glance of the Nurse of the Future Core Competencies the first thing that came to mind was “what do we need this for?” The more I started to read and comprehend what was being read it was like a light bulb went off.

These are all standards that “we” as nurses work with and by every day. In order to function as a nurse, you have to practice as a
    Professional
, if not it would be difficult for one to trust us with their lives. The care that we provide to our patients would mean nothing if it was not
    Patient-Centered
    and focused on
    Safety.
If we cannot keep our patients safe what is the purposed of providing care.
    Communication
is important in everyday life, but it is a HUGE part of caring for our patients, whether it is verbal, non-verbal or physical it gives us an opportunity to develop a trusting rapport with them, not only that, communication offers a sense of healing. In order to help patients with their medical needs we have to have
    Teamwork and Collaborate
with other healthcare professionals. We have to work as a team because we are all trying to accomplish the same goal, helping one get well. As healthcare professionals we function like this day in and day out and it’s a surprise that this plan was developed just a few short years ago. I guess when you are working on bettering something it puts things into perspective and I think that with this NOF NCC it helped to put nursing into a perspective I never thought of. Nurse of the Future Nursing Core Competencies Essay Discussions

All ten of these Core Competencies are very important, needless to say I had a very difficult time finding the 2 that were most important. I find that
    Communication
    and
    Informatics and Technology
are the 2 that really engage my mind. I have been in the nursing field for 9 years and have had the pleasure of working with both the pediatric, adult and geriatric population and
    Communication
has always been at the top of the list. It is not until now that I actually realize it. Whether I am working with a non-verbal child or a comatose geriatric patient I find that there are always ways to
    Communicate
that they understand and feel. When administering eye drops to my comatose/non-verbal patients I always touch their eyelids softly and say “ok I am going to give your eye drops in this/ your eyes”, that way the can anticipate they will feel something go into their eyes instead of just going over and dropping fluid in their eyes. One of my patients would actually take a deep breath after softly touching her eyes as if she anticipating it.
    Information and Technology
has allowed nursing to become easier when it comes down to documenting, reading and submitting orders etc. I grew up in the technology era and love that it is incorporated so well within the nursing aspect.

Week 1: Nurse of the Future Nursing Core Competencies

According to the Massachusetts Department of Higher Education there are ten core competencies known as the Nurse of the Future: Nursing Core Competencies that are crucial for professional nursing practice. The identified competencies are patient-centered care, professionalism, leadership, systems-based practice, information and technology, communication, teamwork and collaboration, safety, quality improvement, and evidence-based practice (Hood, L.J., 2014, p. 7-8). Not only in professional nursing but for student nursing as well, core competencies are essential knowledge, capabilities, and skills that allow nurses to provide safe, effective care to patients (Hood, L.J., 2014, p. 6).

Core competencies are important to provide a guide for all nurses, starting from the beginning of their education and throughout their career. Nursing is a profession where you continue to learn, evidence-based knowledge and sensitivity to variables such as age, gender, culture, health differences, socioeconomic standing, race and spirituality are essential for caring for diverse populations and this global society (Massachusetts Department of Higher Education, 2016, p. 4). I believe these ten core competencies were chosen because it has been shown over time how important each one is to a student nurse, a new-graduate nurse, and even a nurse that has been practicing for many years. NR 351 Week 1 Discussion: Nurse of the Future Nursing Core Competencies

Of the ten core competencies’, the two that I believe to be the most important are communication and patient-centered care. The Nurse of the Future: Nursing Core Competencies states that communication is interacting effectively with patients, families, and colleagues, developing mutual respect and shared decision making, to enhance patient satisfaction and health outcomes (Massachusetts Department of Higher Education, 2016, p.32). In my career as a pediatric nurse communication is one of the more important core competencies that I use and will continue to use in my future nursing practices. Working with children means communicating in different ways. When communicating with children you use pictures to describe pain, point to areas on the body to identify the problem region, going down to eye level, and explaining details in words that they would understand. In pediatrics not only do you have to communicate with the children but their parents as well. Communicating with parents and helping them understand will put children as ease if they see that their parents aren’t in distress.

Patient-centered care is also another tremendously important core competency for my future nursing practice, which is defined as providing compassionate, coordinated, age and culturally appropriate, safe and effective care (Massachusetts Department of Higher Education, 2016, p. 10). Again, as a pediatric nurse it is important to practice patient-centered care. In pediatrics you see children of all ages, ranging from newborns to college age children and knowing how to effectively care for children is extremely important. When working with the younger ages you have to consider all of the medical equipment looking intimidating. NR 351 Week 1 Discussion: Nurse of the Future Nursing Core Competencies. Equipment such as a blood pressure cuff are often explained to children as a machine to measure muscles or just simply giving your arm a hug, a stethoscope is often related to listening to your heart like doc mcstuffins does. At times you even have to care for a child’s beloved stuffed animal to show everyone goes to the doctor. As children get older simply explaining tasks or discussing things you mat have in common. Both of these core competencies are important to me because communication creates clear pathways for patient care and patient-centered care shows that everyone is an individual and requires care focused to that specific person.

    Discussion

Now that you have examined the Nurse of the Future Nursing Core Competencies©, why do you think these competencies were selected? Which two of the 10 competencies will be most important to your future nursing practice, and why?

Week 1: Nurse of the Future Nursing Core Competencies

The Competency Committee of the Nurse of the Future Nursing Core Competencies (NOFNCC) developed a multi-step system to define the core set of nursing competencies (Massachusetts Department of Higher Education, 2010, p. 3). They were to develop a smooth transition for nurses who were planning on furthering their education. Picking the core set of nursing competencies required 3 steps. The first step was making the list of nursing competencies. They obtained their list by looking at other states competencies, current practicing standards, accreditation standards, and national initiatives (Massachusetts Department of Higher Education, 2010, p. 3). Once they had their list of core competencies, they went to stage two which was feedback. They would ask nursing educators, faculty of public or private universities, and nursing practice councils for their opinion. After they made the adjustments they needed they went to step three. The committee did another review of literature but this time they compared their set of core competencies with the national model (Massachusetts Department of Higher Education, 2010, p. 3).

The ten core competencies the NOFNCC in the end decided on were patient-centered care, professionalism, leadership, systems-based practice, informatics and technology, communication, teamwork and collaboration, safety, quality improvement, and evidence-based practice. I believe these competencies were chosen because as nurses we use them every day on the job. It is like a checklist to providing the best care to our patients. They are the guidelines for future nurses, nurses to plan to further their education, and nurses practice in the field. NR 351 Week 1 Discussion: Nurse of the Future Nursing Core Competencies

Of the ten competencies the two most important ones for myself are communication and patient centered care. Patient centered care is defined as “the Nurse of the Future will provide holistic care that recognizes an individual’s preferences, values, and needs and respects the patient or designee as a full partner in providing compassionate, coordinated, age and culturally appropriate, safe and effective care (Massachusetts Department of Higher Education, 2010, p. 10).” Each patient is unique and they each require different care. We can’t treat everyone as if they are all the same. As nurses we need to be able to accommodate to the patients needs and respect their space, whether it be privacy, taking their medication a certain way, or respecting their wishes to have a certain nurse. This reassures our patients that they are our main focus and that they are in control of their care. Our job as nurses is not only to care for our patients; it is to put our patients mind at ease. We are there to reassure them that their needs are met and their questions answered. NR 351 Week 1 Discussion: Nurse of the Future Nursing Core Competencies

As nurses we need to always remember that we are our patients advocates and the only we can fulfill this role is by gaining our patients trust. To build trust we must keep an open and honest relationship. This entails us to constantly keep our patients up to date with their condition or progress. Communication is defined as, “the Nurse of the Future will interact effectively with patients, families, and colleagues, fostering mutual respect and shared decision making, to enhance patient satisfaction and health outcomes (Massachusetts Department of Higher Education, 2010, p. 32).” Nurses have ranked as the most trusted profession for 15 years in a row. This is only possible because of the relationship nurses build with their patients through open communication. We make sure that our patients feel comfortable to ask questions and voice their concerns. Ineffective communication occurs when there is a lack of clarity or direction in a conversation. This is something as nurses we must try to avoid. To achieve this I believe as nurses we must make sure we look up our patients effectively and we must make sure during handoff we have a good idea of what our patients plan of care is for the following day.

I believe we should treat our patients how we would like or loved ones to be treated. Even though there are some days when it is really hard, going that extra mile to make sure our patient have all their questions answered or making some time to spend an extra 5 minutes with our patient who is anxious makes a huge difference. Nursing is a field where we put our hearts into our work and the core competencies are there to make sure we are able to be the best nurses we can be to our patients and ensure their safety. Spirituality and the Nexus Between Religion and Nursing Practice

The aspect of normal anatomy that is interesting to me involves the normal functioning of the human brain. The brain of the human person is a fascinating creation or evolution (depending on one’s belief). It is in that organ that all other functions and processes of the human body are controlled. Thought itself and the conceptualisation of good and bad reside in the brain. Furthermore, the concept of spirituality is resident in the mind, which itself has a complex interconnectedness with the brain. That the brain is made of millions of neurons is what interests me most. This is because the neurons are not even a continuous strand or fibre of organic material. Rather, each of them is an independent cell that is not directly connected to the next. A gap exists between two adjacent neurons, and it is through this gap that impulses are passed from one neuron to another using chemical carriers known as neurotransmitters. Spirituality Reflective Writing – NURS 323 Pathophysiology. Some of these are acetylcholine, dopamine, and serotonin. When disease strikes, most of the times it normally targets the neurotransmitters to disrupt proper functioning of the brain. Symptoms then manifest as mental disease.

The factors that then cause damage to the normal anatomy and physiology of the brain are factors that disrupt the transmission of impulses, or interfere with the integrity of the neurons. An example of a disease condition that results from the former is the psychosis known as schizophrenia, while for the latter the example is the demyelinating condition multiple sclerosis. Another factor that can also cause damage to the normal anatomy and physiology of the brain is a physical or physiological accident. A physical accident is like a fall or a motor vehicle accident, while a physiological accident is like a stroke (bursting of a blood vessel in the brain).

How We Can Restore the Healing Process

Restoration of the healing process from a holistic nursing perspective will involve both physical therapies and spiritual nourishment. In other words, healing will not be directed only at the physical element which is the brain. Rather, it will be directed at the whole person’s mind, body, and spirit. This is where spirituality and the nexus between it and nursing come in. According to Mauk and Schmidt (2004), health and spirituality have had a close connection for a long time. In fact, the basis of traditional medicine especially in the Orient has been spirituality coupled with the usual physical factors Spirituality Reflective Writing – NURS 323 Pathophysiology. In conventional medical practice, nurses have always been concerned with the spirituality of their patients, particularly those that they know will not live for long as they suffer from terminal illness (Hussey, 2009). As such, holistic nursing is also important in restoring the healing process.

Role of Prayer, Influence of Religion and Culture in Illness

Religion teaches that most of the human afflictions are a result of sinning and a person brings them to themselves. As such, spirituality has it that prayer can undo this and restore health. Spirituality and religion also dictate that God will heal without discriminating against those that have sinned and fallen ill. A good example is the paralytic at Capernaum who was healed by Jesus, even though he had fallen sick because of his own transgressions (White, 2011). Culturally, most of us have been brought up to believe strongly that despite tangible physical cures (conventional medicine), spirituality still plays an important part in the healing process. That is why nurses have to adopt a holistic attitude to patient care. Spirituality Reflective Writing – NURS 323 Pathophysiology

In conclusion, we need to appreciate that the normal functioning of the human body is prone to disruption by illness. The treatment of this illness however depends on both physical measures and spiritual intervention. As nurses, we should therefore always care for our patients holistically by including religious and cultural aspects of therapy and healing.

I am writing to apply for the position of registered nurse for xxx as advertised on your website career page. I have eleven years of experience working as a nurse with different facilities and in different positions, and I believe that my set of skills, knowledge, and experiences identify me as an ideal fit for the position. Cover Letter, Resume, and Portfolio Practicum Assignment Examples

In your job posting, you mentioned that you seek to hire a registered nurse with the capacity to be a team player, work independently, lead others, and develop and apply innovative thinking in solving professional problems. In my position as a nurse for the seven facilities that I have worked for in my eleven years of career as a nurse, I worked psychiatric, peds/OB, geriatric, and med/surg fields. I gained experiences that improved my professional skills, practice proficiency, and capacity, allowing me to take on more responsibilities. Besides that, the varied employment positions allowed me to engage with diverse medical personnel and patients, which improved my awareness and understanding of the concept of teamwork. In addition, I have ten years of experience as an army reservist, a position that allowed me to appreciate the concept of leadership, innovation and critical thinking, qualities that serve me well in my nursing career.

When I saw the advertisement for the job opening, I knew that I was a good opportunity to explore a new challenge that would test my nursing skills and knowledge. I have included my resume and portfolio to allow you to learn more about my educational background and work experiences.

Effective Nursing Leadership in the Face of High Nursing Staff Turnover: The Role of Leadership Style in Ensuring Patient Safety

Nursing practice has many issues that affect it. These issues may be related to the work environment, remuneration, or professional development among others. For instance, nurse may find that they are not as productive as they would wish because the work environment they are in curtails their resourcefulness. Most of the time, this kind of scenario is related to oppressive leadership on the part of nurse managers. The nurse may also find that the remuneration they are getting for their services are not congruent with the skills that they have and the workload. Or they may find that the organizational policies towards professional development in terms of training, upskilling, and reskilling are not favorable to them. All these factors normally lead to one undesirable outcome – high nurse turnover. This paper is about this issue of high nurse turnover in nursing practice and the role of the nurse manager in ensuring that patient safety is safeguarded in the face of a reduced workforce. Benchmark – Effective Approaches in Leadership and Management

Nurse turnover in the hospital and other healthcare settings negatively impacts the quality of care that patients receive in such settings. This is because the nurse is the one healthcare professional that is always in contact with the patient and actually delivers the end-product of healthcare institutions. According to Bae et al. (2010), nurse turnover has been shown to negatively impact both the quality and continuity of patient care. Apart from that, they also demonstrated through their study that nurse turnover affects workgroup learning which in turn influences patient outcomes. This position is supported by Antwi and Bowblis (2018) who opine that high nurse turnover is related to lower overall quality of nursing care. Benchmark – Effective Approaches in Leadership and Management According to them, it may also mortality rates in the hospital and other settings like nursing homes. Furthermore, nursing turnover has the effect of lowering effectiveness because new employees are not familiar with an organization’s standard operating procedures and other protocols. In fact, the high turnover may also increase operating costs to the organization; and this is detrimental to the bottom line in fiscal terms. Numerous studies have therefore shown an undisputed statistical association between nurse turnover and health outcomes (Antwi & Bowblis, 2018).

Ensuring Nursing Professional Standards of Practice are Observed

According to standard number 3 of the American Nurses Association’s
    Standards of Professional Nursing Practice
, the registered nurse is tasked with identifying the outcomes that are expected for each individual patient (ANA, 2015). This is in appreciation of the fact that it is the registered nurse who is mandated to plan care for each patient in the care of her team. It is in this nursing care plan that the registered nurse clearly outlines individualised patient outcomes and works towards achieving them together with her team. The team in a typical healthcare setting usually includes other registered nurses, enrolled nurses, and nurse assistants. To achieve the desired patient outcomes, therefore, the registered nurse normally has to take the role of clinical nurse leader and guide care by clinically leading the other team members Benchmark – Effective Approaches in Leadership and Management. This is achieved by delegation of duties. The registered nurse in this case may be administratively involved in the management of the unit or not. That does not exclude her from taking the clinical leadership role. In the face of a shortage of nurses because of high staff turnover, however, the registered nurse who is the clinical nurse leader will find it difficult to maintain professional standards of practice. Yet this is exactly what must be maintained despite all the staffing difficulties.

The answer to the above situation lies in resourceful delegation of duties by the clinical nurse leader, such that the professional nursing standards of practice are respected at all times in the face of the nursing shortage. Care plans must still be made, and individual patient outcomes must still be achieved. The clinical nurse leader can thus delegate appropriate tasks to the other registered nurses in the team because these may not require direct supervision. Other lesser tasks will be delegated to enrolled nurses and both direct and indirect supervision employed depending on their experience. Then the nurse assistants can help the patients with activities of daily living such as feeding and bathing.

Role of the Nurse Manager in a Situation of Nursing Shortage Due to Turnover

In a situation in which there is a shortage of the nursing workforce, the role of the nurse manager becomes very important. This is because patient safety must remain a priority at all times. As such, the nurse manager must find ways of mitigating the negative effects of the nurse shortage. This calls for experience, ingenuity, and resourcefulness. Their roles therefore include maintaining standards of care and patient safety, motivating their remaining staff to carry out the immense task without feeling overburdened, and meeting organizational expectations outlined it its strategic plan. Different nurse managers will adopt different approaches to the situation. There are those that will resort to authoritarianism in order to get things done. This kind of nurse manager will forcefully delegate extra duties to her staff due to the pressure of work and the lack of time to discuss over tasks before allocating them. Of course this approach is counterproductive and can only lead to worsening of the situation as more nurses will leave compounding the turnover crisis Benchmark – Effective Approaches in Leadership and Management. Other nurse leaders will adopt a more consultative and teamwork approach. To begin with, they will put on the gloves themselves and carry out tasks alongside the other staff members. This is the reason why the cl9inical nurse leader should first and foremost be a clinical expert with vast clinical experience. This way, they can fill in where there is a shortage even as they delegate other tasks to the other nurses in the team. This other nurse manager will then make sure that the working environment is conducive despite the pressure of work. The staff members will then feel appreciated and deliver only their best. This is the essence of transformational leadership (Cheng et al., 2016). In doing all these, the nurse managers will be applying the principles of management that are authority and responsibility in that they will be delegating these to the other nurses, even as they retain accountability for all the tasks.

Additional Measures the Nurse Leader and Manager Can Take in the Face of Nurse Turnover

There are additional measures that the nurse leader can institute to make sure that patient safety, professionalism, and standards of practice remain respected. She can make sure that skill mix in her team is correct. Regardless of the shortage, as the nurse and clinical manager she must ensure that the right skill mix among the registered nurses, enrolled nurses, and nurse assistants prevails (Moore, 2012). She must also make sure that she monitors her remaining staff more closely for signs of burnout such as increased irritability. This is because burnout itself will be a danger to patient safety as nurses suffering from burnout have been known to commit medication and nursing errors often.

As stated above, the leadership style that would best address the issue of nurse turnover is transformational leadership. This is a style that ensures motivation to the staff, a conducive and welcoming work environment, and an understanding ear (Cheng et al., 2016). Lack of succession planning has however made it difficult to replicate this kind of leadership, though it remains the best for this situation (Balogh-Robinson, 2012).

    Conclusion

High nursing staff turnover is a menace to all healthcare organizations. It has got detrimental effects to the standard of care that is delivered by the remaining staff since they get overwhelmed with work. To effectively manage its impact, transformational nurse leadership is required. Benchmark – Effective Approaches in Leadership and Management.

CONTEMPORARY ISSUES AND TRENDS: NURSING INFORMATICS – KEY ELEMENTS

    ´Name:

    ´Institution:

    ´
    INTRODUCTION

´Nursing informatics increasingly integrated into nursing practice.

´Involves applying information technology in nursing duties.

    ´Involves aspects of data management.

´Nursing informatics has four proficiency levels:

    ´Beginner

    ´Experienced

    ´Specialist

    ´Innovator

Nursing practice has increasingly been integrated with informatics to result in the conceptualization of “nursing informatics” as a specialization in nursing resources. The concept of nursing informatics involves application of information technology in nursing duties to include practice, management and education with a focus on informatics skills, informatics knowledge; and computer skills. In addition, it involves many aspects of data management such as telenursing, e-learning, intelligent systems, electronic patient records, security, information science, computer science, imaging informatics, information systems, human-computer interaction, decision support systems, patient care, ethics, and data recovery. contemporary issue or trend in nursing. There are four proficiency levels: beginner; experienced; specialist; and innovator (Darvish et al., 2014). The present presentation explores five significant trends concerning nursing informatics.

    ´
    FACT #1: HEALTH CARE PROMOTION

´Nursing informatics applied in research, education, management and clinical areas.

´Increase patient safety through supporting evidence-based practice.

    ´Promote nursing management outcomes.

Nursing informatics is applied in nursing research, education, management and clinical areas, and this has an influence on health care. It increases patient safety through supporting evidence-based practice. In fact, competence in informatics is acknowledged as a prerequisite to improving safety with nursing curriculum promoting informatics tools as critical practice components. Informatics has specifically been used to promote nursing management outcomes to include shift planning and efficient management of health care. Overall, informatics offers access to evidence that supports efforts to improve care quality while supporting evidence-based practice. As such, it is critical for nurses to have the capacity to assess, apply, manage and report data, activities that are best facilitated by informatics tools (Darvish et al., 2014). contemporary issue or trend in nursing

    ´
    FACT #2: ADVANCED DECISION SYSTEMS

    ´Improve speed of decision-support systems.

    ´Helped achieve safe care delivery.

    ´Enhanced critical thinking.

    ´Reduced working time.

Informatics has improved the speed of decision-support systems to help with safe care delivery and enhancing critical thinking. Prior to inclusion of informatics, decision support systems would improve safety but impair critical thinking. In addition, informatics has reduced working time through helping decision systems to replace human knowledge through formulating knowledge. In fact, expert systems have become more common and are used in prescription, decision support, critical treatment plans, and diagnostic support. Examples of such systems can be seen in the intelligent and smart tools that are used to support diagnosis and treatment with the intention of reducing medical harm and errors, and financial loss. It is important to note that these expert systems that include artificial intelligence are not advanced enough as standalone tools to eliminate the need for human expertise. Rather, they require some human expertise to be integrated as part of knowledge management (Darvish et al., 2014) contemporary issue or trend in nursing.

    ´
FACT #3: LEVERAGED INTERNET AND NETWORK

´Unique data networks such as AMN and AJN.

    ´Tele-nursing and telemedicine.

    ´E-education.

Informatics has leveraged the internet and computer networks. In fact, nursing informatics makes use of communication and electronic devices to develop and access unique data networks such as AMN and AJN. Nurses have access to common networks for nursing organization where they develop forums for discussion professional issues while designing online meetings. Besides that, informatics has leveraged the networks to offer medical services from a distance through tele-nursing and telemedicine. In this regard, technological solutions have been developed to facilitate easy remote diagnostic hardware and software designed to facilitate E-nursing services with a focus on remote use of professional knowledge and skills. Through tele-nursing, nurses are no longer restricted by distance when providing counseling, patient monitoring and education. The remote services provision has helped in addressing nursing demand and shortage concerns. contemporary issue or trend in nursing. Besides that, there is a possibility of improving health through E-education irrespective of place and time thus improving self-care behavior and satisfaction among patients (Darvish et al., 2014).

    ´
    FACT #4: REVOLUTIONIZED NURSING EDUCATION

´Nursing informatics capabilities needed to meet present and future health care needs.

    ´Addresses practice challenges.

    ´Addressing national surveillance needs.

    ´Address nurse education challenges

    ´Centralized solutions for nursing problems.

The national advisory council on nurse education and practice have repeatedly highlighted nursing informatics as an important component for meeting the prevailing and future health care needs. It particularly notes that informatics can help in addressing practice challenges in nursing to include telehealth, care planning tools, data capture tools, computerized provider order entry, bar code administration, patient monitoring systems, and electronic health records. In addition, it helps with addressing national surveillance needs, improving care access and data collection efficiency, tracking patient data, reducing medical errors, and reducing the impact of nurse personnel shortage. Besides that, informatics helps in addressing nursing education challenges through faculty development, information technology inclusion in curricula, simulation to delivery education, and E-learning to delivery education. Overall, informatics is credited with centralizing solutions to nursing problems and supporting nurse involvement for optimal technology use with a focus on increasing health information technology skills, reducing faculty shortage, and improving the capacity of nursing schools (Darvish et al., 2014). contemporary issue or trend in nursing

    ´
FACT #5: NEED FOR SPECIALIZED EDUCATION

´Health technology developing and growing fast.

´Nurses can take advantage of the technologies through effective use.

´Used to improve nursing care quality and outcomes.

´Education programs strengthened to implement information technology.

Health technology is developing and growing at a fast rate. Nurses can take advantage of the new technologies only if they can use them effectively. In fact, effective use allows nurses to take advantage of information technology to improve nursing care quality and outcomes. This can only become a reality if nurses are strengthened to implement information technology tools in all aspects of their profession (Darvish et al., 2014). contemporary issue or trend in nursing

    ´
    LOOKING FORWARD

´Potential of nurse informatics cannot be overestimated.

´Enhances quality of nursing domains outcomes.

´Nurses are positioned to interact more frequently with medical technology.

´The frequent use requires informatics capabilities to improve productivity.

    ´Nurses should have informatics capabilities.

The potential for informatics, as a component of information and communication technology application, cannot be overestimated as they enhance the quality of nursing domains outcomes. As primary care providers with the most communication with patients, nurses undoubtedly interact more frequently with technology. Through using technology, they leverage their informatics capabilities to improve productivity thus allowing them to gain more decision-making authority. As such, there is a need to ensure that nurses gain informatics capabilities.

Not every person will answer these questions the same so it’s important to review how others respond to these questions.

    Health Care Team Member Role

T.M. is a registered nurse that works as a case manager at an outpatient Oncology clinic in La Jolla California. The typical patient population consists of patients with brain tumors and metastatic cancers and this is often the last treatment option before hospice. T.M seems to be very passionate about her job in oncology and stated that, “oncology has a very special place in my heart as a health care team member” (T. Macare, personal interview, Feb. 22, 2018) Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

“Oncology specialty is the reason why I pursued nursing” (T. Macare, personal interview, Feb. 22, 2018). The author was impressed with the dedication that T.M has for oncology. Due to this sometimes being the last step before hospice for these patients the author deeply admires her love for her position. This can be a very difficult time for these patients and having a compassionate nurse that deeply loves what they do can have a huge impact on them.

    Professionalism

T.M. stated that, “professionalism pertains to how an individual conducts and presents themselves to others” (T. Macare, personal interview, Feb. 22, 2018). Furthermore, “it pertains to one’s appearance, behavior, and competence” (T. Macare, personal interview, Feb. 22, 2018). “It starts with looking neat, being reliable and getting to work on time, being accountable for their actions, and maintaining an ethical practice” (T. Macare, personal interview, Feb. 22, 2018).

“As a nurse I have taken a pledge to do no harm, maintain confidentiality, and continue to serve to those in my care” (T. Macare, personal interview, Feb. 22, 2018). The author took a similar pledge and thinks that this pledge should show in the professionalism of all nurses. Professionalism can have different meanings to people but the author agrees with what T.M. has included in what she thinks professionalism is. How a person acts to how they look can impact the way a person views a nurse’s professionalism Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

    Stewards of Health Care

T.M. believes that the leaders in her organization are stewards of health. “They are engaged and committed to their profession and their team members” (T. Macare, personal interview, Feb. 22, 2018). “They advocate for patient and nursing safety, patient and employee satisfaction, and enhancement of collaborative practice for doctors and nurses” (T. Macare, personal interview, Feb. 22, 2018).

Leaders can be stewards of health care for many reasons. They can influence staff and patients and are always looking for ways to improve safety as well as improving the personal development of staff. For the profession, the focus will be on refining the roles, creating positive and healthy practice environments, and upgrading regulation and standards to support the future work of the nurse” (Haase-Herrick, 2005, p. 115). “In stewardship of the self, nurse leadership will develop a sound cadre of future nurse leaders, engage in continual learning, and role model balance” (Haase-Herrick, 2005, p. 115). Having leaders that are stewards of health care can foster the development of other great leaders. Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

    Leadership

When in a leadership position it’s important that leaders possess certain skills and characteristics. T.M. expressed that skills of a leader are very important. “I cannot emphasize enough the importance of a leader exercising professional advocacy, authenticity, power, and influence when working with colleagues” (T. Macare, personal interview, Feb. 22, 2018). She also expressed that, “strong leaders need to be authentic in their motives, expressing of feelings, and need to show genuine care towards their team members as well as collaborate not only with their team members but with other disciplines” (T. Macare, personal interview, Feb. 22, 2018) Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

T.M. concluded with, “they foster growth and inspire team members to commit to their cause” (T. Macare, personal interview, Feb. 22, 2018), which the author strongly agrees with. Leaders can have an immense impact on their team members as well as other team members that they interact with. They can lead other by the example that they set.

    Conclusion

Depending on what a person’s role is as a health care team member there will be different answers to these questions. Professionalism and leadership will not be viewed as the same thing by every person. T.M has a strong understanding of what professionalism and leadership is. She’s very passionate about her role as a nurse and feels that she has great leaders that are committed to their team. Introduction

In early March 2020, I interviewed with M.K, a registered nurse working in a nursing home. The interview was broadly classified into four sections which will be discussed in this paper. M.K is a pleasurable person and it is easy to see why she would choose to work with the frail in society. Her compassion and empathy towards people and not just toward patients make her much suitable for her type of work. Additionally, M.K was not hesitant to discuss her background (which will not be included in this interview summary) which made it all the more clear on her choice of career path. The following section of this paper summarizes the interview replies from the interviewee Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

What is your Role as a Health Care Team Member?

M.K stated that her role was that of an RN working in a nursing home. Her main job is to care for the needs of senior patients. At the most basic, she ensures that her patients maintain acceptable personal hygiene levels; receive proper nutrition; have clean beds, and all prescribed medications are taken and treatment sought for any illnesses. Advanced responsibilities include drawing blood, administering medication, giving IVs, and monitoring vital signs (M.K, personal interview, March 4, 2020; McCloskey, Donovan, Stewart, et al., 2015) Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

M.K also stated that she oversees the activities of licensed practical nurses and nursing aides and assistants working in the nursing home (M.K, personal interview, March 4, 2020; Backhaus, Verbeek, Rossum, et al., 2015). She also oversees the medical histories of patients as well as the overall health of the patients. She does this to ensure that the individual needs of each patient are met with the best possible care. Some of these duties are those that she only can do and cannot delegate to the LPNs or nursing assistants and include consulting with the patient’s supervising doctor, monitoring the blood sugar levels, administering oxygen to patients, and commencing intravenous infusions (M.K, personal interview, March 4, 2020) Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

How do you define professionalism and how does professional responsibility influence your work?

M.K first pointed out that professionalism goes beyond politeness and wearing a uniform. She said that professionalism to her means encompassing a set of values that enable elevation of quality of patient care. These values also improve on the judgments, standards, and methods that guide her and her team daily. A professional environment helps her team to develop skills that prove to be valuable to not only the individuals but also the team as a whole (M.K, personal interview, March 4, 2020; Tanaka, Taketomi, Yonemitsu, et al., 2016) Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

Professional responsibility has influenced M.K’s work by first propelling her to pursue a higher education level; she believes that by so doing, she creates further opportunities to enhance her core values of professionalism. Besides furthering her education, professionalism has sharpened her cognitive skills in that she focuses on continuous learning on professional conduct and proceeds to apply the same in her workplace. This has helped her make decisions and prioritizes what is important in an effective way Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

Do you consider leaders in your organization stewards of health care? Why or Why not?

M.K pointed out that she considers leaders in her organization as healthcare stewards. “Stewardship in nursing calls for new care delivery models and the evolving nature of the nurses’ work needs to be addressed in every aspect.”( M.K, personal interview, March 4, 2020) Leaders at her workplace continuously work at collaborating with other professionals in the nursing home care industry to improve the work environment not just for the particular nursing home she works, but every nursing home in the US (M.K, personal interview, March 4, 2020; Manojlovich & Ketefian, 2016). Management at the nursing home she works often visits other homes for benchmarking purposes. Additionally, the nursing home is in the process of seeking international accreditation in service delivery Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

Other than benchmarking, the leadership at M.K’s workplace, leaders attend and participate in dialogues that aim at improving safety of patients as well as their organizational staff; meeting the needs of the aging workforce in the industry; increasing autonomy for NP; increasing respect for professional nurses’ contribution to the field; clarifying the nurses’ caring work, and enhancing the collaborative practice of the healthcare multidisciplinary team (M.K, personal interview, March 4, 2020) Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

Is it important to you that leaders exercise professional advocacy and authenticity as well as power and influence when working with colleagues? Why or why not?

To answer this question, M.K stated that ‘influence cannot exist outside of advocacy and for a leader to be effective in change advocacy as well as for better communities, individuals, and society a large; leaders have to be engaged and also engage with colleagues.” (M.K, personal interview, March 4, 2020) Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview. She went further and said that influence can be viewed as the power that a person exerts to bring about a change and more so, a positive change that affects the nursing profession and advances the importance of nursing in healthcare. (M.K, personal interview, March 4, 2020).

M.K also stated that it is important for leaders to stand up and advocate for a moral good; voicing the needs, challenges, and concerns of disadvantaged people; and collaborating with groups and individuals alike who need assistance in their preferences and rights (M.K, personal interview, March 4, 2020; Shillam & MacL1an, 2018) Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

Comparison of Peer versus Leader’s Perspectives

M.K stated that she goes a step further to ensure that her team works in cohesion. To achieve that, she ensures every team member has a voice by allowing for open communication void of criticism. M.K stated that her main reason for doing the leadership class was to better improve work cohesion at the nursing home (M.K, personal interview, March 4, 2020). On the other hand, A.B focuses more on patient-centered care that involves patients in decision making.

M.K also stated that she tries on all occasions, to align her attitude with the organization’s wider goals as well as with practical standards (M.K, personal interview, March 4, 2020). A.B on the other hand,  believes that although meeting organizational goals and ensuring that all facility staff members are aware of the said goals is critical in the success of a healthcare facility, the focus should be on patient satisfaction first. When patients are cared for in a manner that allows their participation, then better outcomes are realized. These outcomes would lead to better success for the organization. Consequently, when an organization performance improves, employees are motivated to work which again improves on patient satisfaction Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

At the end of the interview it was clear that M.K has a passion for her job. She has taken to pursuing further education to make her a better leader and one that advocates for the disadvantaged. Also, M.K. views professionalism as a critical component of enhanced patient care delivery and better collaboration within her team. Her main drive on self-improvement is to improve the lives of others, a noble and admirable path to take. A.B on the other hand is more focused on involving patients in decision-making and in all levels of their care and treatment as the best way to achieve optimal patient outcomes. The two interviews depict a common ground as regards professionalism and the characteristics that are deemed of importance for a leader to possess Professional Identity and Stewardship – Part I: Peer & Part II: Leadership Interview.

The Importance of Nursing Informatics and the Multidisciplinary teams

As a healthcare professional, it is imperative to have excellent communication skills. Nurses should always know what is going on with their patients, too many times; nurses miss urgent orders due to a lack of communication. Nursing informatics is essential; having computerized charting makes it easier for the nurse to check orders and to know the plan of care.

According to YoeniTo, improve the quality of nursing care, we developed and evaluated an Electronic Medical Record (EMR) program designed to maximize productivity and efficiency in our nursing documentation system. Electronic medical records are useful communication tools that can alert anyone on the interdisciplinary team of the patient’s progress. An electronic document can share relevant patient information among care providers to coordinate care, aid in decision-making, and achieve improvements in quality (Burnett, 2016). Benefits of EMRs are they provide a virtual state of the art multidisciplinary rounding record, so when doctors are not available, they can log on and access what every interdisciplinary team discussed. EMRs increases efficiency’s, reduce readmit rate due to patient having access to their medical records online, decreases in readmissions, and have fewer mistakes and redundancies then paper charting. However, I believe that the patients should always have a physician on call available to answer patients’ questions to cut down on unnecessary ER visits. The Importance of Nursing Informatics and the Multidisciplinary teams.

Consistently getting team members together, for instance, requires constant coordination; with the electronic medical record, doctors can pull up labs and diagnostic tests with patients and families. Computers have proven to be a great communicator between teams in the health care setting. Nursing informatics provides a haven for better patient care when dealing with Large multidisciplinary teams and deliver better outcomes for patients.

Nurses must be able to provide safe care and treatment to patients. Most of my career was working in a Level one Trauma Center Emergency Department (ED) in downtown Phoenix, Arizona. In the ED,

we had one system that we used for charting, and the rest of the hospital had a different charting system. If our patient was admitted to the hospital, we had to print off the ED visit note and send it with

the patient for the nurses receiving the patient. This created problems. Just a few of these were: if the patient had medications that were ordered in the ED, but were not given for whatever reason, the

receiving nurse was not aware unless the sending nurse specifically gave it in report, if the patient had STAT lab orders, the ED nurse would not see these as the admitting providers would put them in the

inpatient admitting orders, and at times the admitting provider would discontinue medications that the ED nurse had orders to give by the ED provider before sending them to the floor. The Importance of Nursing Informatics and the Multidisciplinary teams

By getting input from ED staff, providers, and inpatient nurses, the nurse informaticist (NI) advocated for the team to administration. They helped implement the ED charting system that was the same

system that the rest of the hospital had. The NIs include staff in change decisions to ensure the change would not have negative effects on the workflow and to increase staff buy-in (Tyler, 2019, p. 19). The

NI’s trained a staff member on each shift to be a superuser and helped during the transition to the new program. As stated by Tyler (2019), ”The informatics nurse is essential to the success of super users

in any organization” (p. 19). The super users had direct communication with the NI’s and were able to alert them to issues or ideas for a smoother process recommended by the staff.

NI’s trained the providers and the ED staff on using the new system. When the providers needed changes in the system for different documentation within the system, the NI collaborated with the charting

company, and the changes were made.

One strategy recommendation for improvement would be to have the NI come to the unit once a week for any suggestions or complaints and have a suggestion box for those that may be busy or not

present when the NI was there. The form placed in the box would have an answer section that would go back to the person that wrote it with a week return response timeframe. The Importance of Nursing Informatics and the Multidisciplinary teams

Nursing informatics have the benefit of their ability to interact with various clinicians, clinical practice knowledge and can relate to staff with the experience the workflow change (Mastrian & McGonigle,

2018, p. 257). As we rely on technology in so many aspects of our lives, NI’s are a necessity in healthcare. The use of NI’s and their specialty will enable hospital staff to have the most recent evidence-based

practice, patient safety with alerts and check-offs in the electronic medical record (EMR), and track specific trends good and bad for change and education. Nursing executives can use nursing informatics

to help with things like budgeting tools and trending costs and savings (McGonigle, Hunter, Sipes, & Hebda, 2014, p. 324).
