    Psychopharmacological Approaches to Treatment

    Introduction

The brain, which is the most complex part of the human body, is responsible for the regulation and function of the human body. It is made up of billions of nerves that play critical roles in the human body (). The human body can reach its maximum potential through the presence of neurons that control numerous functions such as memory, learning, motivation, homeostasis, and emotion. This implies that nerve cells help people to understand their surroundings by initiating activities like smelling, hearing, touching, and sensing. A neuron is a basic unit of the nervous system which is involved in the transmission of information across nerve cells, glands, and muscles (Beaulieu-Laroche, 2018. This paper aims to discuss the anatomy of neurons, functionalities, its components, and the concept of neuroplasticity. NURS 6630 week 1 Assignment: Short Answer Assessment

    Anatomy of the Neuron

A neuron that is the basic unit of the nervous system is a nerve cell responsible for transmitting information to muscles, glands, and other nerve cells. A neuron has six major parts: synapse, myelin sheath, axon, soma, nucleus, and dendrites. Synapses play the role of sending electrical signals to other neurons while dendrites receive electrical signals from nearby cells (Beaulieu-Laroche, 2018). The soma comprises of cell’s organelle while the nucleus contains the DNA of cells. The axon transmits electrical signals from cells to synapses, while the myelin sheath insulates the axon to control electrical signals. Electrical impulse conduction takes place through the axon, cell body, and dendrite pathway. Impulse termination occurs when it reaches the synapse and communication takes place. Examples of impulses include touch, light, and sound. NURS 6630 week 1 Assignment: Short Answer Assessment

    Major Components of Subcortical Structures

    Subcortical structures are neural formations in the brain which perform complex activities like hormone production, pleasure, emotion, and memory. The structures act as nervous system information hubs by relaying and modulating information in various parts of the brain. The major elements of subcortical structures include;

Diencephalon- It is the posterior of the forebrain and is made up of the epithalamus, thalamus, subthalamus, and hypothalamus that perform primary relay and processing of sensory information (Vertes, 2015).

Pituitary glands- It is an extension of the hypothalamus, which consists of the anterior lobe and posterior lobe and is responsible for secreting, stimulating, and releasing hormones. NURS 6630 week 1 Assignment: Short Answer Assessment

Hippocampus- It is also known as the hippocampal formation and is in charge of spatial navigation and long-term memory.

Basal ganglia- It is made up of nuclei and is responsible for fine-tuning moto neuron functions.

Limbic structures- It is composed of cortical, subcortical, and brain areas, and it controls memory, emotion, olfactory, motivation, learning, and body homeostasis. NURS 6630 week 1 Assignment: Short Answer Assessment

Which Component Plays a Role in Learning, Memory, and Addiction?

The cerebrum or the cortex, which is the largest part of the brain, plays a major role in the control of learning. The limbic system, including the cingulate gyri are responsible for learning. Although different parts of the brain may be responsible for different kinds of memory, the hippocampus and amygdala are generally responsible for memory. While, the nucleus accumbens and is responsible for influencing addiction through feelings of pleasure or reward (Surgeon General, 2016). NURS 6630 week 1 Assignment: Short Answer Assessment.

The Function of Glia Cells in the Central Nervous System

Glia cells are also known as neuroglia or glial cells, are non-neuronal cells that are found both in the central nervous systems (CNS) and peripheral nervous system (PNS). Glia cells offer a supporting role to neurons and do not take part in the production of electrical impulses. The main functions of glial cells include the formation of myelin, maintaining homeostasis, and offering support and protection to neurons. The functions of glial cells are depicted from its different types, which include astrocytes, microglia, oligodendrocytes, and Schwann cells located in PNS (de Castro et al., 2015). Astrocytes control the floor of the blood in the brain, normalizes communication amongst neurons, and sustain fluid composition around neurons. Astrocytes also offer assistance to neurons during development by locating their destination and creating blood barriers in the brain. This is essential as it helps protect the brain from toxic substances. Microglia act as hunters, which eliminates dead cells and other waste substances. Oligodendrocyte components of glia cells take part in myelin’s production that insulates axons surrounding neurons and increases action potential in the central nervous system.

Part Of Neurons that are Communicating with each Other

Neuronal communication involves the transmission of electrochemical from one component to another. Synapses refer to the intersection between an axon of one neuron and the dendrite of another neuron where communication takes place (Andreae & Burrone, 2018). With this, the parts of neurons that communicate with one another are axons and dendrites. Axons conduct electrical impulses away from nerve cells to other neurons through a process called an action potential. On the other hand, dendrites comprise neurotransmitter receptors, which are released by neurons. If electrical impulses being released are strong, an action potential will travel down the area of the axon in the terminal buttons to release neurotransmitters in the synapse and completes the process of communication (The University of Queensland, 2017). Neuron communication occurs in the downward direction, which is determined by the action potential NURS 6630 week 1 Assignment: Short Answer Assessment.

    The Concept of “Neuroplasticity.”

Neuroplasticity/brain plasticity is defined as structural and functional changes that the brain adapts through the formation of new neural connections that result in new experiences in an individual’s environment (Voss et al., 2017). The concept of neuroplasticity describes the human brain’s ability to adapt, learn new skills, store information and memory, and being able to overcome traumatic brain injury. Neuroplasticity occurs through sprouting and rerouting processes naturally or through brain plasticity exercises and cognitive training. Sprouting involves the development of a new connection between neurons, while rerouting is the development of a new neural pathway by eliminating damaged nerve cells where new connections are formed between active neurons. During sprouting, neuron pathways are stimulated by repetitive cognitive functions that reinforce synaptic communication amongst neurons. Examples of neuroplasticity include neurogenesis, synaptogenesis, and long-term potential, and depression. For example, during neurogenesis, new neurons are created in the hippocampus and olfactory bulb. In long-term potential, synapses are strengthened by recurring actions like studying aimed at promoting learning and memory. NURS 6630 week 1 Assignment: Short Answer Assessment

    Conclusion

Indeed, neurons are significant elements of the brain that are involved in the transmission of information to other neurons. A neuron consists of synapses, axons, dendrites, myelin sheath, soma, and a nucleus, which helps in neurotransmission. Therefore, the components allow a human being to manage learning, emotion, memory, and homeostasis activities with ease. NURS 6630 week 1 Assignment: Short Answer Assessment

    Mini-Mental State Exam (MMSE)

Health care providers alway use the Mini-Mental State Exam (MMSE)While gathering subjective and objective data to evaluate the mental health status of the elderly. MMSE is a standardized tool used to assess cognitive function changes over time. The 11 items measuring orientation, registration, attention and calculation, recall, ability to follow commands, and language it takes approximately 5 to 10 minutes to administer NRNP 6635 The Psychiatric Evaluation and Evidence-Based Rating Scales Discussion. The maximum score is 30. A score 20 or less may be associated with dementia, and a score of 26 or higher is not associated with dementia.Higher education is associated with higher MMSE scores, even when dementia is present. MMSE has been translated into multiple languages and adapted for many cultures, and in some cases external validity, sensitivity, and specificity have been evaluated (Ball et al., 2019).

This test is often used to help diagnose Alzheimer’s disease and dementia. This quantitative-styled survey scores one’s as either zero (incorrect), one (correct), six (item administered, participant does not answer), or nine (test item not administered/unknown). Out of thirty points, a score that ranges between twenty to twenty-four marks mild dementia; thirteen to twenty defines moderate dementia; and anything less than twelve is classified as severe dementia (Alzheimer’s Association, 2019) NRNP 6635 The Psychiatric Evaluation and Evidence-Based Rating Scales Discussion. This tool is often used to get a baseline on the degree of dementia and used to evaluate the effectiveness of medications that function to delay the progression of the disease. While the MMSE provides useful information that adds to a mental health diagnosis, this test should not be the only information used. The National Institute on Aging (2017) asserts that in addition to neuropsychological tests such as the MMSE, providers also use laboratory tests and brain scans to validate a diagnosis of dementia. NRNP 6635 The Psychiatric Evaluation and Evidence-Based Rating Scales Discussion

The MMSE method has many advantages. The method is popular and it has a large quantity of data that is readily available to find standards that can be used for comparison in various circumstances and settings. It has been made popular because of its standardized structure that can be applied universally that has enabled it to be accepted by many as a measurement unit and a basis for assessing the severity of CI and DEM (Palsetia et al., 2018). Again its structure enables assessment of various cognitive aspects and its rating scale has been accepted globally for measuring mental functioning  It is very quick and easy to practice where it involves less instructions which can be administered in ten minutes and therefore it can be administered with less training and is widely taught in most institutions.. Their scoring system is standardized which makes it easy to calculate the results. The method can be applied using different languages since instructions are simple and can be applied NRNP 6635 The Psychiatric Evaluation and Evidence-Based Rating Scales Discussion

The test method also has several disadvantages. The main disadvantage is the fact that the method is not very useful in identifying mild impairment (Palsetia et al., 2018). It also offers problems in detecting changes that occur in severe dementia. The method does not give accurate information for people who are educated because of the ceiling effect (Palsetia et al., 2018). Furthermore, the method does not offer fine details for impairments such as neglect, aphasia and apraxia that occur for people in post-stroke NRNP 6635 The Psychiatric Evaluation and Evidence-Based Rating Scales Discussion. The method does not put more emphasis for memory loss since it is given only three points while orientation and language are given more most points (Palsetia et al., 2018). This shows that the method is not dedicated to assess the core domain for cognitive ability (Palsetia et al., 2018). Furthermore, the lack of full standardization for the method offers another problem. Three words for the registration stage are critical in the memory stage but there are no criteria for choosing these words. Freedom is given to the examiner to choose these three words of which the choice can be affected by several factors such as frequency of use, familiarity, imaginability others. Other items that are not standardized are drawings and calculations. Besides, some of the instructions require reading and writing skills which limits its use on illiterate people.

In conclusion, MMSE has become a very popular tool for assessing cognitive impairment for older people. Some of the reasons for its popularity is because of shorter instructions, ease of administering and ease of translating the instructions into different languages all over the world. The method fits well for the older generation because it takes a few minutes to complete it. The method reliability was proved to be adequate and its validity was also adequate with a high sensitivity and specificity.

Discussion: The Psychiatric Evaluation and Evidence-Based Rating Scales

Assessment tools have two primary purposes: 1) to measure illness and diagnose clients, and 2) to measure a client’s response to treatment. Often, you will find that multiple assessment tools are designed to measure the same condition or response. Not all tools, however, are appropriate for use in all clinical situations. You must consider the strengths and weaknesses of each tool to select the appropriate assessment tool for your client. For this Discussion, as you examine the assessment tool assigned to you by the Course Instructor, consider its use in psychotherapy.

Week 2 initial post Positive and Negative Syndrome Scale

This week’s resources give us as students a plethora of information that can help us to diagnose many different psychiatric disorders that we will experience as advance nurse practitioners. The key is to use the tools that will be the most effective and give us the best outcomes for our clients and their families. The decisions we make will help us to diagnose and treat clients effectively and to determine the best tools to use we will be examining those tools individually and together through this discussion forum. The Positive and Negative Symptom Scale may be a tool that can help with the diagnosis of Schizophrenia, Schizoaffective disorder and Schizophreniform however It seems that the reliability of this tool has not been proven to be consistent with different diagnosticians (Kølbæk et al, 2018). Not because it is ineffective, but due to the time constraints which are not always amenable to the client’s condition in a crisis (Kølbæk et al, 2018).

According to Sadock, Benjamin et al., the psychiatric interview is the most valuable element of the evaluation and care of clients with mental illness (2017). However, without the medical and mental history as well as the examination of the client and questioning of family or people close to the client the diagnosis may be skewed. Many clients with schizophrenia or any diagnosis that has delusional aspects or hallucinations may have knowledge that is corrupt in nature due to the nature of their illness (Andresen et al., 2006). Additionally, a patient may have a medical condition that is causing their psychosis. For this reason, the medical including drug or alcohol use need to be considered and ruled out prior to diagnosing a client with a psychotic disorder (Psychiatry Lecture, 2020).

The Medical history is the first component I feel is important to the interview of a patient that is having psychosis simply to rule out any organic causes for the psychosis such as infection, trauma or tumor. These things can be ruled out by taking completing blood tests, diagnostic imaging, and a system review. Taking care of the client’s physical needs may be the answer to help resolve his psychosis (Sadock et al, 2017).

The psychiatric history of the client and the family is a second component that is especially important with psychotic patients. Knowing the history and how long the client has been suffering with the symptoms will help to determine the diagnosis of the client. If the client has been dealing with his or her current symptoms for more than 6 months, and they have a family history of schizophrenia, then the risk for the patient regarding schizophrenia is much higher (American Psychiatric Association, 2013).

The third component which is particularly important is the history and current use of substances including alcohol, medication, and illegal substances. I believe it is important to talk about all substances including medications the patient is currently taking as well as the amounts they are taking. The client could be taking their prescribed medications incorrectly, causing an exaggerated reaction or side effect which can be remedied easily by educating the client on proper use and dosing (American Psychiatric Association, 2016) NRNP 6635 The Psychiatric Evaluation and Evidence-Based Rating Scales Discussion.

The assessment tool assigned to me is the Positive and Negative Symptom Scale (PANSS). This tool is one of many primarily used for diagnosis by giving a reliable method of determining the severity of the symptomology presented by clients in psychosis (Fleishhacker, 2016). The second purpose of the tools for diagnosis of schizophrenia and other psychotic diagnosis’ is to determine the effectiveness of treatment (Fleishhacker, 2016). The PANSS tool which was published in 1987 (Ostergaard, Opler & Correll, 2017) is considered the gold standard for diagnosis of psychotic mental health issues once medical reasons for the psychosis is ruled out (Takeuchi et al, 2016). The psychometric properties of the PANSS have proven this to be an exceptionally reliable and valid instrument for determining the factors in schizophrenia and other psychotic disorders especially for research (Østergaard, Opler, & Correll, 2017). However, the practicality of using this tool with clients who are presenting in crisis is questionable. It requires 45 to 60 minutes of uninterrupted time with a client to administer this test which makes it impractical in many psychiatric intake settings (Kølbæk et al., 2018.)

This instrument is valuable for research and to help evaluate clients who are stable and able to verbalize their feelings and symptoms. In clinical practice it may be more effective for us as advance practice nurses to use an abbreviated version of this tool such as the 6-item positive and negative syndrome scale (PANSS-6) or the Simplified negative and positive symptom interview (SNAPSI) (Ostergaard, Opler & Correll, 2017).

The PANSS is a 30 item Positive and Negative as well Global symptoms. Positive symptoms are those that are added to a clients’ “normal” These consist of things like hallucinations, delusions, disorganized concepts, hostility, grandiosity, suspicion, and excitement Negative symptoms are when the client is lacking where his or her personality is concerned. For instance, if a client is not socializing with others. Negative symptoms included on the PANSS are blunted affect, emotional withdrawal, poor rapport, apathy or social withdrawal, problems with abstract thinking, no spontaneity and stereotypical thinking. The third factor of the PANSS is the general psychopathology scale. This consist of 16 items which include anxiety, somatic concerns, depression, poor attention, poor impulse control and preoccupation. The lowest that can be scored on any one of the items is 1. So, the lowest score possible is 30 (Kay, Fiszbein, & Opler, 1987).

To have a high degree of reliability with this instrument, there must be training for those rating the clients using said instrument (Opler, Yavorsky, & Daniel, 2017). In the case of the PANSS raters there are 4 principles that are required.

Each item definition and points must be interpreted as literally as possible.

The highest rating possible must be given if it applies.

The reference period and time frame must be considered for each item.

All information given in the interview must be considered for each item, therefore the instrument must be completed after the interview is completed.

The training for rating this instrument should be included in clinical training (Opler, Yavorsky, & Daniel, 2017).

This instrument and the other many instruments that are available in psychiatric practice helps me to understand there is still so much to learn. I know that this is only the tip of the iceberg and I am looking forward to using the different tools available to us. NURS 6003 Week 1 Discussion Career Goals: Strengths and Challenges Related to Nursing Practice Competencies

I am a Registered Nurse (RN). I reside in Littlerock, Arkansas, I am single with no children, and I am the primary caretaker of my 70-year-old mother. From the most primitive time that I can remember, I wanted to make an impact on the world, mainly through the care of others. NURS 6003 Week 1 Discussion Career Goals: Strengths and Challenges Related to Nursing Practice Competencies. I had practiced as a Registered Nurse, since the year of 2015, before my career as an R.N., I held a Nursing Assistant certificate and a license for Practical Nursing. I work in the Long-Term Care (LTC) setting.

My ultimate goal is to work as a Family Nurse Practitioner. I plan to provide empathetic, compassionate care that is all-inclusive. The care that I expect to deliver will encompass not only physical health but spiritual health as it affects the overall health of a being and a population. I plan to isolate and identify the causes of individual patient imbalances utilizing a root cause analysis and from there articulating and implementing plans for specific issues to educate and promote further health. Focusing on health promotion must include the spiritual and physical well-being of an individual or group, for this shows full support of the being. (Grossmeier, 2019). The ultimate goal is to educate to prevent further circumstances. NURS 6003 Week 1 Discussion Career Goals: Strengths and Challenges Related to Nursing Practice Competencies.

Attending Walden was not an easy choice for me, so many thoughts rushed through my mind as I began to doubt my ability to return to school and obtain a degree. I can honestly say that it was something about the commercial that pulled me in, and after speaking to some of my colleagues who had attended Waldend and receiving only good reports I was almost sold. I then began to do some research, and my nerves began to resurface, and my intuition just told me go for it you can do this, stop being afraid to be the change that you want to see in the world, so I applied NURS 6003 Week 1 Discussion Career Goals: Strengths and Challenges Related to Nursing Practice Competencies.

Walden’s mission substantially aligns with my professional and academic goals, but more so for me with my spiritual goals. By providing me with many opportunities for multiple modes of learning and allowing me not only to learn but also to apply the skills I obtain from the teaching provided. By providing groundbreaking, learner-centered, enlightening curricula that identified and will integrate the information, expertise, and capabilities that I bring into my academic program, it lets me know that Walden will put me first. (Walden University, 2020). It gives me confidence that Walden puts me first. The coursework aligns with the knowledge that I must harness to achieve my goal. NURS 6003 Week 1 Discussion Career Goals: Strengths and Challenges Related to Nursing Practice Competencies I believe this will aid in my professional elevation. I think that the community’s improvement by focusing on an individual’s health, and conditions that are faced day to day is imperative. Knowledge and safety are paramount in the medical field, and by completing the course work provided and by mastering the programs learning outcomes, I feel that I will have fulfilled my definition of success, in this particular arena.

NURS 6003 Week 1 Discussion Career Goals: Strengths and Challenges Related to Nursing Practice Competencies response

I do appreciate your inspiration and defeat you have used to keep up your objectives. I felt we are in a comparable situation. I began this excursion of turning out to be Family Nurse Practitioner in 2017 and could not get the training I required from that college and decided to drop out of the program. Choosing Walden University is the best choice I have made. I originated from West Africa Sierra Leone and charitable association is the fundamental wellsprings of the less advantaged network to endure. The charitable association is the fundamental haven for youngsters that have been misuse and relative that lost their homes during war in my nation. Your conversation on the philanthropic association as your objectives will have an enormous effect on the network, particularly to the less privileged communities. I am curious if you have ever considered expanding your nonprofit organization to Africa? NURS 6003 Week 1 Discussion Career Goals: Strengths and Challenges Related to Nursing Practice Competencies.

NURS 6003 Week 1 Discussion Career Goals: Strengths and Challenges Related to Nursing Practice Competencies Walden University describes social change “as a deliberate process of creating and applying ideas, strategies, and actions to promote the worth, dignity, and development of individuals, communities, organizations, institutions, cultures, and societies” (Walden, 2020 Theme & Mission). As a director of nursing working in the home care agency, I utilize holistic care regularly. Often include communicating with health care providers, family members, patients, advocates, provide health promotion, and using community resources to ensure the quality of care, especially to diversity population. I believe you are on the right track and keep your goals in place. In my opinion, your biggest challenge will be communication with health care providers and family members. Looking at the mission of Walden University that stated that it is “a diverse community of career professionals that can transform as scholar-practitioners so that they can affect a positive social change.” (Walden University, 2020). It is vital that you connect with different health care providers in the public health community to create and educate the less privileged population. With this collaboration, it will create and maintain routine appointments with their primary care providers which in turn can decrease emergency hospitalization, improve the quality of care, and save the cost of medical insurance. Overall, you discussion is very inspiring

    Discussion – Week 1

    COLLAPSE

NURS 6003 Week 1 Discussion Career Goals: Strengths and Challenges Related to Nursing Practice Competencies/. As we progress to a new level of nursing, we have to ask ourselves what is our person misson in our careers?  I have been a nurse for 7 years, and started in a level 1 trauma center in the ER and worked at that facility for a year to get a basis, and then started doing nursing contracts.  Throughout the years I have mainly tried to stay within the emergency department, but I have worked some contracts on the medsurg floor, and some shifts in a PACU.

I recently received by BSN in March 2020.  It took me awhile to get back in the swing of school and paper writing.  I have a previous Bachelors degree in Business management and with those additional credits, I was able to buckle down and finisg my BSN in 6 months, which I was proud of myself for doing.  I decided since I was already in “school mode” that I would continue with my goal in being a nurse practitioner.  Waldon seems to be a good fit, as there is a lot of busy work, but I feel that this is easily doable while working full time. (Waldon 2020)

Through Walden’s mission that states,   “have a deep curiosity, drive, and commitment to solve pressing problems in their field” (Walden University, 2020, Vision, Mission, and Goals) I have noticed that this is also my personal mission statement.  I have always been one to ask why questions.  I am very curious by nature to understand specific processes or how different disease processes can change the outcome of our patients based on their big picture.  I am commied to being the best practitioner that I can be and being able to understand and solve the health care problems I am confronted with.

” The journey to the “ultimate
    achievement
” never ends.”(Plastic Surgery Nursing 2016)  This is an inspirational quote that is now included in my personal mission.  I have always felt that I lived in my sister’s shadow.  I mean she is pretty amazing, but I was never involved in everything she was or as strong academically as we were growing up.  She went to nursing school and loved it and was part of my inspiration to start into the field.  However, as soon as I started, she decided to take her 10 years of nursing experience and continue to medical school.  As she is now in residency, as respects me as a colleague in the field, we have discussed me working with her side by side and working hospital and clinic pediatrics.  This would fulfill my career goal and feel that I have officially made and surpassed my goals. Recommending the Article to inform Professional Practice

The article is appropriate to inform professional practice because the article recognizes the use of theory, guidelines, and evidence to inform nursing practice, and as a result, facilitate provision of safe and high-quality care. accordingly, it is important for the nurse educators, nurses and researchers to work hand-in-hand to generate evidence-based solutions to challenges present in nursing practice (Hussein & Osuji, 2017). This can be achieved by decreasing the theory-practice gap and assisting students and nurses to apply theoretical knowledge and evidence to practice solutions. Part 4: Research Analysis NURS 6003 Assignment: Academic Success and Professional Development Plan. Theories in nursing also focus on guiding decision making in nursing and curriculum design. Approach to Identifying and Analyzing Peer-Reviewed Research

The first step included consulting the library database. The library database had an option to refine my search and generate scholarly articles. After retrieval of the articles, I looked for an editorial statement as well as author instructions at the end of the journal articles. Some of the articles had instructions to authors or editorial statements that indicated that they were peer-reviewed while others did not have. In addition, the Ulrichs Global Serials Directory was also used; this is a database that provided information regarding published journal articles, including the information if the article has been reviewed. Part 4: Research Analysis NURS 6003 Assignment: Academic Success and Professional Development Plan. The journal article that was retrieved and selected was denoted as “refereed,” indicating that it was peer-reviewed. In addition, the selected article had the credentials of the authors, their academic affiliations were listed, the journal article contains an abstract at the start of the article and also had a reference list of sources used in the journal article. All these steps facilitated the identification of the selected peer-reviewed journal article. Part 4: Research Analysis NURS 6003 Assignment: Academic Success and Professional Development Plan.

Strategies Effective in Finding Peer-Reviewed Research

The first strategy is to select a suitable database and interface, to begin with. According to Bramer et al (2018) vital factors to consider when selecting databases to utilize include the presence and the coverage of a thesaurus. For searchers that are medical-oriented, the recall and coverage of Embase, which consists of the MEDLINE database, are considered better when compared to those of MEDLINE database Part 4: Research Analysis NURS 6003 Assignment: Academic Success and Professional Development Plan. Each specific database contains its specific thesaurus with distinct structure and definitions. Embase thesaurus is relatively complex and hence Emtree, which has more definite thesaurus terms when compared to MEDLINE (MeSH) thesaurus is easier, and hence Embase has been recommended to be used first. MEDLINE and Embase are widely available and the accessibility of the searcher determines the primary database and the interface as well (Bramer et al, 2018).

The second step includes selected an advanced search screen. Part 4: Research Analysis NURS 6003 Assignment: Academic Success and Professional Development Plan The use of an advanced search screen enables the researcher to refine the search by choosing a search field for each term that is entered. The fields that can be searched consist of the subject term, title, author, along with the name of the journal. It is important to limit the search in order to ensure that only articles with full-texts are retrieved. The search can also be limited to peer-reviewed journals and data of publication. After retrieving and selecting the journal article, it is important to conduct a critical appraisal in order to ensure the quality of the article (Bramer et al, 2018).

One Resource to Use to Find Peer-Reviewed Research

One resource I would use to locate peer-reviewed research is Ulrich Web. Ulrich Web is a database of journals and can be used to identify journals in specific subject, confirm if the journal is peer-reviewed as well as confirm the database the journal is indexed or the availability of the journal article in full-text form. Ulrich Web provides bibliographic and publisher information of all kinds of periodicals, scholarly and academic journals and it addresses over 900 subject fields (Cooper et al, 2018). Any journal article that is peer-reviewed is denoted as “refereed,” in the Ulrich Web. Part 4: Research Analysis NURS 6003 Assignment: Academic Success and Professional Development Plan

  • NURS – 6003N Transition to Graduate Study for Nursing

    RE:
    Discussion – Week 1
    – NURS – 6003N Transition to Graduate Study for Nursing

I’ve been in the medical field for 40 years, however, have only been a nurse for the past 12 of those years. I started my nursing journey when my children were older and in high school, and the timing was right for our family. Because of the later start into my dream field, I dove into it with both feet and swam at lightening speeds to try to become the best nurse I could be, and what I found in myself was that I enjoy and excel at promoting change. NURS – 6003N Transition to Graduate Study for Nursing I don’t know if that has to do with my age and associated life experiences, or if that was an innate ability in me all along that I was unaware of. I began my nursing career on a medical surgical floor, then transitioned to a hospice unit until it’s closure, and then to ICU. During the past 40 years I have been a CNA, med aide, and clinic manager of a busy urology practice, in addition to the past 12 years as a RN. I always knew I wanted to become a nurse practitioner.

I began researching schools and programs that were entirely online NURS – 6003N Transition to Graduate Study for Nursing. I have several friends who have gone to Walden University in the recent past, and spoke very highly of it, so I put this university on my list. As I was searching, some of the most important facets for me are the reputation and ranking of the school, cost, educational level of the professors, and the vision and mission statements. It was easy to cross off several universities for me due to the absence of the important factors I mentioned. Walden University stayed on the top of my list for quite some time. I can honestly say this university meant all of my criteria. Walden’s mission and vision statements spoke to me for several reasons. What I appreciate most about Walden’s mission statement is that it highlights diversity (Walden University, 2020). The topic of diversity is exceedingly important to me for multiple reasons, and so much that I wanted to be sure I was a part of a graduate program that emphasizes and promotes this topic.

In meeting my personal goals, and reading on Walden’s website in the College of Nursing vision and mission statements, I knew I had found the perfect college for me when I discovered that Walden’s view is not just to feed us knowledge, though that is exceedingly important as a practitioner, but this distinct university desires to move us to become change agents (Walden University, 2020). That is when I knew it was a perfect fit for me and would challenge me as a person and nurse. I want to be challenged to think outside my box, and the knowledge base that I have thus far. This proved evident in the scholar-practitioner portion of the vision and mission statements by stating “have a deep curiosity, drive, and commitment to solve pressing problems in their field” (Walden University, 2020, Vision, Mission, and Goals, para. 3). This is when Walden became my school of choice. It is my belief that as nurses, we are a trusted, revered profession that has a platform to produce change in not only the medical field, but in the world. This is what I desire to be a part of in my life, especially as a practitioner in the future NURS – 6003N Transition to Graduate Study for Nursing.

The word networking can be thought of in a negative connotation, thus associated with being a smoothie type person or schmoozing people just to get a desired outcome (Schmidt, n.d.). I was glad this was brought up in this article because I believe nurses are as far removed from exhibiting salesmen or insurance agent schmoozing behavior as anyone could be (Schmidt, n.d.). Professional networking for nurses is a valuable and useful tool that we should be taking advantage of in order to make professional relationships that may lead to career advancements, increased knowledge base, and develop a more powerful voice to effect change (Schmidt, n.d.) NURS – 6003N Transition to Graduate Study for Nursing.

According to P. Wofford, Founder of Your Nurse Connection, over 70 percent of nursing jobs are secured via professional networking opportunities (Wofford, 2018) NURS – 6003N Transition to Graduate Study for Nursing. We are all aware that social media can be used positively or negatively, and in the case of professional networking, social media is an effective tool (Wofford, 2018).

I personally belong to several Facebook medical groups, and have enjoyed the networking and exchange of knowledge. Other interesting ways to network as a nurse are becoming an influencer on Instagram or become a blogger, volunteer, attend conferences, have a signature look about you, seek out connections, and have business/networking cards available to exchange with connections made (Wofford, 2018). This article suggests not joining more than 10 social media type groups, and always send a follow-up email after making a connection (Wofford, 2018). Conferences are my personal favorite, and one that I’ve utilized frequently in my nursing career. I have always gained knowledge, and used it to empower my peers or those I supervise. It is the conferences I’ve attended on my own that have afforded be to become a change agent in my nursing career NURS – 6003N Transition to Graduate Study for Nursing.

I have chosen the psychiatric mental health nurse practitioner program, not only because I enjoy this field, but because it needs and deserves change. It was my belief that the school I chose would have a great deal to do with the effectiveness of the changes I will be able to execute for the greater good of my profession. Becoming a scholar practitioner will enable and equip me to be able to effect the changes I hope to be a part of in my community regarding mental health advocacy. I have networked with a psychologist in the past who has given me great insight into the challenges surrounding the stigmas associated with the mental health industry, and I believe that has helped me by forming me into a passionate, and non-judgmental nurse to advocate for those struggling with mental health issues. NURS – 6003N Transition to Graduate Study for Nursing Response

I admire the fact that you have been in this field  so long, I truly think it takes a special person to put the care of others in the forefront. I am curious to know more about your time on the hospice unit as this is something I have wanted to do, but I felt that I may not be able to function with the thought that my patient would eventually pass on. While I do have an understanding that Hospice care is not only comfort care, and it does factor in holistic care. I don’t think I fully understand every thing that Hospice encompasses. I would think that it would aid in improving the quality of life that is left? maybe you could expound on that for me. NURS – 6003N Transition to Graduate Study for Nursing

I will not say that I did much research in schools but I did trust my nursing  counterparts when it came to the school that put them first.

I also read your post and noted your article on professional networking. There was once a time when I just went to work and did my job limiting my conversation to patients, families, doctors and fellow nurses. I didn’t realize prior to becoming a nurse exactly how important it would be to my overall career to have effective communication and networking. I can agree that by networking it opens the door to many other opportunities and aids in securing jobs. Though I did not realize how much planning and preparation went into effective networking. And according to your presented article when networking one should have their career goals in mind (Schmidt, n.d.).

I also belong to a few Facebook groups that pertain to medical topics, I love how colleagues can come together without putting each other down and spread love and knowledge also, I believe that I have missed many networking opportunities by being ill prepared and not being exactly conscious of my overall goals in the past. I did not began my journey as goal oriented as I have been as of lately. I believe my energy was somewhat misplaced, that is why I am always happy to see someone who has all of their plans lined out such as you. Reading your post I became even more inspired to get my ducks in a row.

I also commend you on your entrance into the Mental Health practitioner program, you are correct, I do not feel enough focus is placed on mental health as a lot of the issues go undiagnosed, and in some ethnicities there is a stigma assigned to seeking help when it comes to mental health it has been viewed at times as being weak. While I considered the ACAGNP program I feel as though I would like to extend my care across and entire lifespan and not be limited by a specified degree, my whole career has been spent with the elderly, I feel its high time for me to spread some love to the rest of the population.

     Discussion – Week 1

    COLLAPSE

Hello my name is Caylisa Handy. I have been a nurse for almost two years in February. I have accomplished numerous awards and accolades in such little time in my career. I am the chair of shared governance and currently working on my RN3. I love what I do serving and advocating for my patients. I currently work on a orthopedic, med surg, trauma, and surgical floor, where we see a lot of gun shot wounds, falls, etc. Previously, I worked at an OBGYN office as a Medical Assistant. After completing nursing school, I decided I would change specialties to gain experience in a different area of nursing NURS – 6003N Transition to Graduate Study for Nursing.

As I have been a student my entire life, literally I have been in school since I graduated high school in 2008; I had the desire to be a labor and delivery (L&D) nurse, however, as I stated before after nursing school I decided I would start with med surg and later return to L&D. I have learned through this journey of nursing that you have to have a strong support system. What I love about my company is that they support, train, and equip us to grow in our career. With that being said I was undecided at first which career path I wanted to take. During the time I was a medical assistant, I was leaning toward Nurse Practitioner in Women’s Health. After I became a nurse I thought maybe leadership would be my clinical ladder pathway. After long thought and research, I finally decided that Family Nurse Practitioner (FNP) was the way I wanted to go. I chose Family Nurse Practitioner because I want to help prevent patients from going to the hospital and them manage their health as much as possible. NURS – 6003N Transition to Graduate Study for Nursing

I am looking forward to becoming an FNP and I chose Walden University because I felt like I would have the support and push I needed to complete this program, pass the boards, and be confident and equipped enough to go out and serve my patients as a Nurse Practitioner. Walden’s mission to produce scholarly graduates that will be agents of social change and providing a diverse and innovative learning environment relates to my academic and professional goals. (2019-2020 Walden University, 2020b). Walden’s MSN program outcomes relates to my academic and professional goals by exemplifying integrity, diversity, professionalism and encouraging me to be creative and enforcing what I learn into my career field. (2019- 2020 Walden University, 2020a).
NURS – 6003N Transition to Graduate Study for Nursing

I understand having a good support system helps you be more successful, not only in life but also in your career. Having a mentor or a team that supports you, keeps you focused and accountable of reaching your goals. A study also shows that having that extra support and accountability, boosts a student’s competency and confidence (Rush et al., 2019). My academic  team will consist of my student advisor and my instructors for each class. I plan on utilizing my student advisor as a resource here at Walden. I plan on utilizing my instructor to help challenge me in each course so that I can reach my full potential as a student. My professional team will consist of my current Director and a Physician Assistant from my previous job. I have built a rapport with them and I feel that they will also push and challenge me to reach my goals. The Physician Assistant can help me based on her clinical experience and may be my preceptor. Discussion – Week 1

My Nursing Career began two years ago as a Psychiatric nurse
. I currently work in a bariatric surgical care unit. It’s always been my passion to care and serve others since I was younger, however, it was not until my sophomore year of high school that I made the decision that I’d like to be in the healthcare field after I joined HOSA. HOSA’s mission is to empower future health professionals (HOSA 2020). They started me on my professional health care journey and it is an honor to continue this journey with Walden University.

NURS – 6003N Transition to Graduate Study for Nursin
    g. Walden’s mission to create diversity in the professional healthcare community correlates with my goal to learn, implement and develop skills as a Psychiatric practitioner. Walden’s focus on leadership and clinical skills development, data management, and the overall empowerment of its students with tools to provide quality social changes drew me to the institution.

My passion for serving others is among the best qualities a healthcare professional can have. I enjoy volunteering at a local shelter in my area, especially during health fairs. It provides me with the opportunity to give back to my community and educate the vulnerable population on how to care for themselves and seek needed medical attention. This aligns with one of Walden’s social change initiatives (Walden 2020). I believe, with the help of Walden’s institution that I will be able to create a better healthcare environment. Not only for myself but for patients and generations to come.

My Goals and Walden University Vision and Mission

I have been a nurse for a few months now, and I knew this was the right time to advance my career in other to achieve my academic, career, and life goals. I became interested in the field of nursing during my last semester of my first undergraduate degree. I knew I wanted to be an advocate for public health issues; I did not realize that nursing could open so many opportunities to achieve that goal until my great friend and professor encouraged me to investigate nursing. I choose the Psychiatric Mental Health Nurse Practitioner (PMHNP) program because I wanted to be challenged. I wanted to be part of the solution in a field that I believe is underrepresented. After a year of shopping around for various PMHNP schools, I choose Walden University for several reasons. The first time I heard about Walden was through my older sister, who is in her final year at Walden. I liked how she was able to pace herself, continue working, and caring for her family, all while earning a PMHNP degree. Upon further research, Walden’s mission to create a diverse group of scholars achieve their career goal meant there was a spot for a Nigerian woman like me (Walden University, n.d.). Walden’s vision is what truly grilled me in as I share the same view about education and its ability to transform minds that can advance our society (Walden University, n.d.).

Another reason why I choose Walden was that their College of Nursing Vision which stated that the school seeks to provide “academically rigorous and culturally and contextually relevant,” (Walden University, n.d. para. 5). Like I mentioned above, I choose the PMHNP program because it is a challenging field, and I needed a school that will equally match that. According to their vision, Walden’s school of nursing will provide me with an environment where I can be an expert in my field of study while making it relevant to our culturally diverse world. My Goals and Walden University Vision and Mission Discussion Essay

Master of Science in Nursing (MSN) Outcome

One of the outcomes for the MSN program at Walden is to create health care providers who can implement ways to provide cost-effect care to reduce any healthcare disparities (Walden University, n.d.). The Centers for Disease Control (n.d.) supported that health care professionals can implement strategies to decrease healthcare disparities by using evidence-based practices. Ending disparities in healthcare apply to one of the reasons I chose the PMHNP program. I want to be in health care to bridge the gap between what kind of care patients can receive based on whether they live in a rural or urban area, unbiased of their race, gender, age, and medical needs. Another outcome for the MSN program that I relate to is the need for Walden to use its graduates to educate the public about their health (Walden University, n.d.). I believe that nurses can play an integral part in educating the public on safe health choices that prevent injuries and promote a healthy lifestyle.

    Vision for Social Change

Since my first undergraduate degree, I have made it my mission to fight for social justice for women and children. It is awe-inspiring that Walden also has the vision to bring about social changes through the education their students will receive. The university vows to inspire and celebrate their students who create solutions to challenging issues in our world (Walden University, n.d.). Walden is the place for me to be and continue to nurture my hunger for developing changes in communities of interest. According to the Minnesota Depart of Health (2015), only 10 percent of Nurse Practitioners licensed in Minnesota, where I live, are found in rural areas. With my career as PMHNP at Walden, I intend to become part of that 10 percent. I want my time at Walden to reflect the steps I take in the future as a human being and as a Psychiatric Mental Health Nurse Practitioner.

    Discussion – Week 1

Being able to serve patients with mental health disorders for about fifteen years is one of my accomplishments that I really applaud. Even though this is not an easy job, using holistic nursing to stimulate the healing process has brought to my attention the need to continue my degree as a Psychiatric Mental Health Nurse Practitioner (PMHNP). My Goals and Walden University Vision and Mission Discussion Essay Since I have been having such a great experience with patients with mental illness, I have decided to work and reach my goals until retirement in the highest quality of care of Psychiatric Mental Health Institution. In the light of all that, I have chosen to advance my career at Walden University as a referral by my friend who is also my coworker to fulfill those dreams

Walden University Vision –Mission –goals- social change-outcomes

According to Walden University’s vision-mission statement, “Knowledge is judged worthy to the degree that it can be applied by its graduates to the immediate solutions of critical societal challenges, thereby advancing the greater global good”. The university provides a various community of career professionals with the opportunity to transform themselves as “scholar-practitioners so that they can effect positive change”. Walden University provides a wide range of educational opportunities for career learners. The university “provides research that fosters critical thinking skills that leads to professional excellence. Walden supports ethical scholar practitioners that will become civic and professional role models for the society.” (Walden University, n.d.). From students’ success enrollment specialists to Professors, the University provides great quality customer service. That is another reason why I have picked Walden University, because its vision statement relates to the type of program I was looking for.

With that being said, upon completing this program as a professional Psychiatric Mental Health Nurse Practitioner, I believe that education I receive from the Walden MSN Program will enable me to create a positive attitude that will be useful within my work environment to better serve the community in which I am working for My Goals and Walden University Vision and Mission Discussion Essay. It will also help me to assess the mental health needs of different populations to finally solve the problem of patients with different mental health illness living in our community and around the world. Walden University gives its students the possibility of networking. As suggested by experts, I am positive that networking with students who chose a similar career path as me could be an instrument in advancing my career (blog.bluepipes.com).   Without any doubt, after advancing my education to this noble profession, I will continue to improve patients’ outcomes in delivering excellent services in any challenging Psychiatric mental health institution.
