    [**Deanna Freeman-Jones**](https://herzing.instructure.com/courses/47762/users/98707)

**Management of the Patient with Peptic Ulcer Disease**

    **Diagnosis**

The presentation is most consistent with iron deficiency anemia, likely secondary to chronic blood loss or nutritional deficiency. The hemoglobin of 8.5 g/dL and hematocrit of 25% indicate a moderate to severe anemia. The patient’s early satiety, gnawing epigastric pain, anorexia, weight loss, and fatigue raise concern for a possible underlying gastric ulcer or malignancy. Cognitive changes and pallor support symptomatic anemia.

    **Management/Treatment Rationale**

Based on the patient’s condition, my treatment goals include correcting anemia, identifying and treating the bleeding source, and alleviating GI symptoms. Initial management includes oral ferrous sulfate 325 mg three times daily (providing ~195 mg elemental iron daily) for treatment of iron deficiency anemia (Arcangelo & Peterson, 2021). Additionally, proton pump inhibitor (PPI) therapy, such as omeprazole 20 mg orally once daily, will help heal a likely gastric ulcer and reduce gastric irritation from iron, while improving GI symptoms and iron absorption (Bereda, 2022).

    **Prescription**

    1.
_Ferrous sulfate 325 mg PO TID_
_**:**_ Oral; Three times daily; Take on an empty stomach to enhance absorption; avoid dairy, caffeine, or antacids within 2 hours.

    2.
_Omeprazole 20 mg PO daily:_ Oral; Once daily, 30 minutes before breakfast; Do not crush or chew.

    **Patient Education**

I would educate the patient that iron therapy may cause GI upset, dark stools, and constipation – encourage a high fiber diet and hydration. Emphasize the importance of adherence for at least 3 to 6 months until ferritin and hemoglobin normalize. Counsel to rise slowly, manage fatigue, and report any worsening abdominal pain, black stools, or vomiting (Bereda, 2022).

    **Follow up**

I would schedule a follow-up in 4 weeks to repeat CBC, ferritin, and assess response and tolerance. If hemoglobin fails to improve or GI symptoms persist, I would consider referral to gastroenterology for endoscopic evaluation to rule out peptic ulcer disease, gastric malignancy, or other bleeding sources. I would also assess the patient's cognitive status on the return visit

    **Diagnosis**

The most likely diagnosis for this 60-year-old woman is iron deficiency anemia secondary to chronic gastrointestinal blood loss, likely due to gastric ulcer disease or gastric malignancy. Her hemoglobin of 8.5 g/dL and hematocrit of 25% indicate moderate anemia. Associated symptoms including pallor, dry skin, early satiety, gnawing epigastric pain, and mild nausea support the presence of a chronic underlying condition, possibly affecting the upper gastrointestinal (GI) tract. The recent cognitive changes and fatigue are also common in anemia and chronic illness (Lanier et al., 2018). Given her age and constellation of symptoms, it is imperative to rule out gastric carcinoma, which can present similarly and often remains occult until advanced (Sitarz et al., 2018).

    **Treatment Plan and Rationale**

Immediate goals include correcting the anemia, identifying and treating the underlying source of blood loss, and ensuring nutritional support. First, I would order iron studies (serum ferritin, iron, TIBC), vitamin B12, folate, and a fecal occult blood test (FOBT). To evaluate the source of bleeding, the patient requires an urgent esophagogastroduodenoscopy (EGD). A referral to gastroenterology for endoscopic evaluation is warranted to rule out ulceration or malignancy. In the meantime, oral iron supplementation should begin to replenish iron stores. I would start with Ferrous sulfate 325 mg PO once daily, as this provides about 65 mg of elemental iron per dose. Lower-frequency dosing is now preferred to improve gastrointestinal tolerability and absorption (Sendur & Malkan, 2025). Because of early satiety and reduced oral intake, a nutritional consultation is also indicated to assess for caloric and micronutrient deficiencies and to provide guidance on nutrient-dense food intake while further diagnostics are underway.

    **Prescription**

    · Ferrous Sulfate 325 mg orally once daily

    · Instructions: Take with orange juice or a source of vitamin C to enhance absorption. Avoid taking with dairy, calcium supplements, or antacids.

    · Duration: Continue for at least 3 months after normalization of hemoglobin.

Should the patient not tolerate oral iron or if absorption is impaired, IV iron such as iron sucrose may be considered after further evaluation.

    **Patient Education**

The patient should be educated about the cause and consequences of iron deficiency anemia, emphasizing the importance of iron therapy adherence and dietary adjustments. Foods rich in iron  (red meat, leafy greens, legumes) and vitamin C (citrus fruits) should be encouraged. She should also be advised to report any new or worsening symptoms such as black stools, fatigue, chest pain, or shortness of breath. Education should include an explanation that her early satiety and stomach discomfort are concerning for possible upper GI disease, and that a scope test (endoscopy) will help identify the cause of bleeding. Support should also be offered regarding emotional and cognitive changes, as these may improve once anemia is treated.

    **Follow-Up and Referral**

The patient should return for a follow-up in 1–2 weeks to assess her tolerance to iron therapy, repeat her CBC, and review the results of initial lab tests. An urgent gastroenterology referral is needed for endoscopic evaluation. If a gastric malignancy is identified, oncology consultation will be required. Should there be persistent neurocognitive symptoms despite anemia correction, referral to neurology or geriatric psychiatry may be indicated.

**Description:** In your own words, describe the study in less than 200 words. Include study design, participants, intervention if there was one, and major conclusions. Description should be less than 200 words.

Singh et al. (2022) conducted a randomized control trial in India with 220 critically-ill patients receiving mechanical ventilation. No statistically significant differences were found in the demographic and clinical characteristics of the treatment and control groups at baseline. Routine oral care for intubated patients was the use of chlorhexidine in the oral cavity. In addition to the normal interventions used to prevent ventilator-associated pneumonia (VAP), the treatment group received toothbrushing and application of a mouth moisturizer. During the study, the frequency of oral care was determined by a standardized, scored oral assessment. Diagnosis of VAP was made based on fever, abnormal chest X-rays, and positive cultures of the endotracheal tube. The treatment group had lower prevalence of VAP, fewer days of mechanical ventilation, lower mortality, and shorter ICU stays. (129 words)

**Strengths of the study** (15 points)

The strengths of the study include the rigorous design with random assignment to groups, an adequate sample size, and a rigorous intervention. The study had no attrition.

    **Limitations of the study**(15 points)

The study was conducted in one critical care unit in a different country. The researchers also noted that some patients were admitted from other hospitals and the care provided there was not known. Some of these patients may have received antibiotics. They also said that the diagnosis for VAP was non-invasive and thus possibly less accurate. The researchers did not discuss the fidelity of the intervention- was the intervention applied consistently with all patients in the treatment group?

**How do you plan to use the study results in your practice?** (15 points)

I plan to share the results of this study with the unit manager and one of the RNs I work with routinely. We already use chlorhexidine as part of the mouth care for intubated patients. Using the comprehensive oral care intervention would require additional nurse time and the purchase of toothbrushes, toothpaste, and moisturizer, but preventing VAP would be worth it.

    Overview

When performing an evaluation to determine the performance or health of a company, it is important to identify and evaluate any current or potential risks the company might face.

In this assignment, you will perform a risk assessment for TransGlobal Airlines to better understand its business environment.

    Prompt

Perform a risk assessment for TransGlobal Airlines and write a report describing the results.

Specifically, you must address the following rubric criteria:

    ·
**Identification**: Identify two risks the company is facing, including at least one high-impact risk.

    · Identify the balanced scorecard component each risk corresponds to.

    · Classify the risk based on these two types:

    · Operational

    · Strategic

    ·
**Evaluation**: Determine whether the probability and impact of each identified risk is low, medium, or high. Justify your evaluation of the impact and probability of each risk.

    ·
**Mitigation**: Recommend a possible risk-prevention or mitigation strategy for each identified risk.

    What to Submit

Submit a 1- to 2-page Word document using double spacing, 12-point Times New Roman font, and one-inch margins. Sources should be cited according to APA style.

    Scenario

You have been asked to evaluate Company A and Company B and make your recommendation for acquiring one or both companies. Based on your initial assessment, you have created balanced scorecards for both companies. You are now ready to analyze the information you have gathered so far about the two companies so that you can compare the costs, benefits, and risks associated with acquiring each company and make a well-informed decision.

In this milestone, you will first analyze the current situation of TransGlobal Airlines using the given data and other sources to understand their business environment. You will also evaluate the performance of Company A and Company B using the balanced scorecards you created in Milestone One.

Using the recommendation you provided in Milestone One, you will develop a strategy to implement the IoT innovation and move ahead in the marketplace.

    Scenario

You work as a middle manager for one of the top U.S. producers of luxury and mass-market automobiles and trucks. To remain a viable competitor and meet the demands of consumers’ desire for technology, your company’s chief technology officer (CTO) has decided to implement the internet of things (IoT) in your company’s product line. You are now leading a cross-functional team to move forward with the company initiative.

In Milestone One, you and your cross-functional team looked at the risks and benefits of discontinuous and incremental innovation, competitors in the market, and your company’s internal capability for moving forward with the IoT innovation. After doing this analysis, you presented your recommendation to various stakeholders about how to proceed. You chose one of the following:

    A. Design a completely new product line, based on the recent technology (discontinuous innovation)

    or

    B. Add recent technology features into one model and then incrementally into the broader product line (incremental innovation)

The CTO has accepted your recommendation. Now you are ready to develop a strategy for taking the innovation to market. In order to develop a strategy, you will need to look at your competitors’ relative strengths in the current marketplace, to understand how your company compares to your competitors, and to identify the total market and the market’s annual growth rate. Finally, you also need to share your thoughts on how to respond to changes in business conditions. Although no one can predict what is going to happen, it is important to think about how flexible your plan is in order to meet unexpected conditions.

    Directions

Using the recommendation you provided in Milestone One, develop a strategic plan to implement the innovation and gain competitive advantage. Use Power BI to analyze the Comparative Growth Data included in the Power BI User Manual linked in the Supporting Materials section. You should also refer to the Sales Forecast, Comparative Growth Data, and Comparative Operating Statistics linked in the Module One Memo Guidelines and Rubric to complete this milestone.

    1. Follow the directions in this document to create the required charts using Power BI. Analyze the
**competitors’ relative strengths** in the marketplace.

    A. Write a short paragraph describing the growth rate of each competitor based on the data included in the comparative growth data sheet.

    B. How much of the market does each competitor now own? Do you see a trend of gaining or losing market share? Present your findings using the comparative growth data sheet and Power BI.

    i. Complete your pie charts and analysis for the market share percentage of cars and trucks now and in 2030.

ii. Complete your pie charts and analysis for the market share percentage of connected cars and trucks now and in 2030.

    C. How do your company's financials look compared to your competitors'?

    2. Identify
**future potential total available market (TAM)** and growth for each chosen product/service and technology.

    B. What is the global TAM for IoT-connected cars and light trucks in billions?

    C. What is the projected compound annual growth rate (CAGR) for cars and light trucks?

    D. What is the projected CAGR for IoT-connected cars and light trucks?

    3. Identify ways to adjust if
    **business conditions change.**

    A. What strategies can you implement if your customers are slow to purchase the innovation?

    B. What actions can you take if one competitor is overtaking all the others, including your company?

    4. Describe the steps your company needs to take from
    **concept** outline
**to launch.** Consider your learnings from the previous two modules to ensure the steps are easy to understand for your team.

    A. What is the size and scope of development that will be necessary to successfully launch the IoT innovation?

    B. Will you require additional capital and personnel to support the development and launch?

    C. How would you approach determining the timeline from the beginning of development until initial product launch?

iv. Consider key milestones, relevant metrics, project phases, and any tools or methods you would use for measuring progress.

    What to Submit

Submit a 4- to 7-page Word document, devoting one page to each critical element described above, along with an introduction and a short executive summary.
